Arterial complications of the thoracic outlet syndrome: fifty-five operative cases.

Between January 1, 1969, and December 31, 1984, 55 operative procedures were carried out in 47 patients to correct subclavian-axillary artery lesions resulting from compression at the thoracic outlet. The most common causes of compression were a long cervical rib (27) and an anomalous first rib (15). Presenting features included claudication, vasomotor phenomena, digital gangrene, and acute limb-threatening ischemia. A combined supraclavicular and infraclavicular approach was preferred. Decompression was best achieved by excision of the cervical rib and the first rib and division of all soft t…

Arterial Thoracic Outlet Syndrome Treated Successfully with Totally Endoscopic First Rib Resection.

Thoracic outlet syndrome (TOS) is a constellation of signs and symptoms caused by compression of the neurovascular structures in the thoracic outlet. TOS may be classified as either neurogenic TOS (NTOS) or vascular TOS: venous TOS (VTOS) or arterial TOS (ATOS), depending on the specific structure being affected. The basis for the surgical treatment of TOS is resection of the first rib, and it may be combined with scalenectomy or cervical rib resection. Herein, we describe a case of arterial thoracic outlet syndrome which was successfully treated with totally endoscopic video-assisted thoracos…

[Compression syndrome of the upper thoracic outlet (thoracic outlet syndrome)--report on a 13-year-old girl].

The thoracic-outlet-syndrome, a symptom complex consisting of arterial, venous and neural disorders of the upper extremity, is caused by compression of the neurovascular structures between the clavicle and the first rib. Other reasons are congenital abnormalities like a cervical rib, an abnormal first rib, acquired anatomic lesions like fractures and traumata or functional factors. The younger-and middle-age groups are most commonly involved. The symptoms are depending upon the point at which compression occurs and include pain, paresthesias, numbness, fatigue, pallor, coolness or swelling and…

[Removal of the 1st rib in thoracic outlet syndrome. Is it helpful? Is it safe?].

A thoracic outlet syndrome is only rarely caused by osseous anomalies. Instead, it is a soft-tissue disease, where the C8 and T1 roots or the proximal inferior trunk of the brachial plexus are compressed by fibromuscular anomalies between the inferior cervical spine and the first rib or pleura. Thoracic outlet syndrome is a clinical diagnosis. If surgery is recommended, it should be concentrated on the fibromuscular anomalies and not on the first rib. Transaxillary removal of the first rib carries an important risk of serious complications. Among these, brachial plexus injury is the most frequ

First rib resection for vascular complications of thoracic outlet syndrome.

The surgical approach to vascular complications of the thoracic outlet syndrome remains controversial. When present, removal of a cervical rib alone has produced disappointing results. Our experience of 29 consecutive first rib excisions over a 5-year period is presented. Of 20 cases with uncomplicated subclavian artery compression 19 were cured, and of six cases with aneurysm or thrombosis five were improved. Of 12 cases with neurological symptoms nine were cured and two were improved. It is suggested that first rib excision is the essential primary treatment for patients with arterial sympto…

Thoracic outlet compression syndrome: current concepts and surgical experience.

Current concepts of the diagnosis of thoracic outlet compression syndrome are presented together with the surgical experience and results obtained in 45 patients. Conservative therapy with exercise, moist heat and physical therapy is advised in patients with mild, non-disabling symptoms, whereas surgical therapy, resection of the first rib, cervical rib, prolonged transverse process or anomalous first rib is recommended in patients with persistent and disabling symptoms. The complications are minor but some serious events with injury to the subclavian artery, vein and brachial plexus have been…

[Surgical treatment of thoracic outlet syndrome].

We present three cases with longstanding true neurogenic thoracic outlet syndrome. All patients had aching pain in the shoulder, arm and ulnar border of the hand. On examination, we found atrophy of the hand muscles. Electromyography revealed signs of compromised function of the inferior trunk of the brachial plexus. At surgery, we found and severed a fibrous band that compressed the inferior trunk. Postoperatively, the pain subsided and fine hand movements improved. One patient had no cervical rib, however, in the two other cases we found rudimentary cervical ribs. Magnetic resonance imaging …

INTRODUCTION AND IMPORTANCE: Supernumerary ribs are very rare. They may occur at any level of the spine. We present here a case of an unusua … a supernumerary rib. CLINICAL DISCUSSION: A review of the literature reveals a global incidence of cervical ribs ranging from 0.04 % up to 4.5 %, intrathoracic ribs in about 50 cases to date and very few reports on supernumerary ribs in the lumbar and sacral region. We were unable to find any similar cases of supernumerary ribs in the sternum. CONCLUSION: Su …

[The thoracic outlet compression syndrome and its vascular complications].

The costoclavicular compression syndrome and the cervical rib syndrome can cause arterial complications of the subclavian artery: mural thrombosis, occlusion and aneurysma. These complications can result in peripheral emboli or cerebrovascular insufficiency. Such sequelae can be avoided by timely surgical treatment of these compression syndromes. The therapy of choice is transaxillary rib resection. The same axillary approach can also be used for directly reconstructing the subclavian artery.

[Surgical treatment of superior thoracic outlet syndrome].

The authors present immediate and long-term results of treatment of 117 patients with superior thoracic outlet syndrome (STOS). There were different reasons for compression of neu- rovascular fascicle in outlet of the thorax. The costaclavicular syndrome was a reason in 48 patients, additional cervical ribs had 36 patients. Skalenus syndrome was noted in 26 cases, rudimentary cervical ribs or hypertrophy of cervical vertebrae C7 had 7 patients. Raynaud's syndrome took place in 19 cases. The required volume of diagnostic procedures and surgical treatment of STOS were determined according to the…

A radiological study of vertebral and rib malformations in children with myelomeningocele.

Postmortem radiographs from 112 infants with thoracolumbosacral or lumbosacral myelomeningoceles were surveyed for associated vertebral and rib malformations. Sixty-four exhibited a variety of anomalies including hemivertebrae, fused vertebral bodies, fused vertebral arches, fused ribs, absence of ribs and absence of vertebral bodies. Anomalies were commonly associated with the cranial end of a dysraphic spina bifida region; in addition hemivertebrae and rib fusion exhibited peak incidences centred around the seventh cervical and fourth thoracic vertebra respectively. Fusion of vertebral arch

A Hypertrophic Anterior Scalene Muscle and the Passage of a Subclavian Artery Through its Fibres: The Location of Possible Entrapment.

OBJECTIVE: The presence of cervical ribs, 1st rib anomalies, cervical muscle hypertrophy and repetitive motion are possible aetiologies of subclavian artery (SCA) entrapment and/or compression. Thoracic outlet syndrome of the arterial type may appear with symptoms of hand pain due to the aneurismal part of the compressed SCA. The current cadaveric case describes a hypertrophic right-sided anterior scalene muscle (ASM) and the possible entrapment of the right SCA (RSCA) passing through its fibres. Furthermore, the branching pattern of the entrapped vessel is analysed. CASE REPORT: A hypertroph…

Experience of supraclavicular exploration and decompression for treatment of thoracic outlet syndrome.

The purpose of this study was to assess the symptomatic outcome of patients with thoracic outlet syndrome who underwent decompression of the thoracic outlet. In our unit we prefer the supraclavicular approach, performing anterior scalenectomy with excision of fibrous bands or cervical ribs if present. Operative details were gained by theater logbook and case note review. Over a 6-year period, 31 patients (37 limbs) underwent thoracic outlet decompression. Of the 37 affected limbs, the indications for surgery were a combination of both neurological and vascular symptoms in 24 patients (65%), ne…

Unilateral chest wall paradoxical motion mimicking a flail chest in a patient with hemilateral C7 spinal injury.

After a lower hemi-cervical spinal cord injury, a patient presented with a left hemiplegia and on the same side a unilateral chest wall paradoxical motion mimicking a flail chest. X-rays demonstrated a left hemilateral C6 injury but no rib fractures. We demonstrated that the paradoxical motion was due to the action of the diaphragm acting on the rib cage with intercostal respiratory paralysis on the side of hemiplegia.
