Anticholinergics in palliative medicine: an update.

Anticholinergics, or antimuscarinic drugs, are drugs that competitively inhibit the action of acetylcholine at muscarinic receptors, leading to a blockade of the actions of the parasympathetic nervous system at sites where overactivity can lead to increased symptom burden. Successful blockade of the parasympathetic nervous system ultimately leads to decreased production of secretions in the salivary, bronchial, and gastrointestinal tracts. These effects are often used for several symptoms that originate due to parasympathetic nervous system overactivity, such as the "death rattle" and malignan

Neurogenic bladder in children: basic principles, new therapeutic trends.

Diagnosis of neurogenic bladder is straightforward in children with myelomeningocele. However, recognition is more difficult in patients with occult dysraphism or central nervous system disorders since clinico-anatomical correlations are poor. Careful clinical examination and urodynamic exploration are mandatory for diagnosis and follow-up. Even if urinary leak is the first symptom, the main goal of the pediatric surgeon must be to preserve the upper urinary tract. The ideal protection strategy consists of ensuring that micturition is voluntary and complete and that the bladder capacity is suf

Contribution of the serotonergic system to myopia and the therapeutic effects of atropine and pirenzepine.

Muscarinic-cholinergic antagonists, such as atropine, are widely used to treat myopia, yet their mechanism of action remains unclear. Recent chick studies suggest atropine may act via a non-muscarinic pathway, specifically antagonism of serotonergic signalling. This study investigates the conservation of this mechanism across different models of myopia (form-deprivation myopia (FDM) and lens-induced myopia (LIM)), and across different muscarinic agents (i.e., pirenzepine). We also examined the role (and ocular distribution) of serotonergic signalling in myopia onset. Chicks undergoing FDM or L…

[A new approach to antischizophrenic therapeutics: D2 dopamine receptor partial agonists].

Since chlorpromazine was described, antischizophrenic therapeutics have been enriched with various other D2 dopamine receptors antagonists, either neuroleptics (which develop extrapyramidal side effects) or non neuroleptic antipsychotics (devoid of extrapyramidal side effects when used at effective doses against psychotic expressions). The latter drugs display such properties on account of several associated pharmacological activities (D3 receptors antagonism more efficient than D2 antagonism; antagonism at cholinergic muscarinic receptors; antagonism at 5HT2 serotonin receptors). All these an

A neuropharmacological study of the periventricular neural substrate involved in flight.

This paper reviews results obtained in experiments concerning the neurochemical characteristics of the substrate involved in the control of flight reactions and the induction of aversive effects in the rat. These experiments investigated the behavioural effects produced by microinjecting into the periaqueductal grey matter (PAG) or the medial hypothalamus (MH) compounds known to interfere with the functioning of some neurotransmitter systems known to exist in these structures. The data obtained show that: the activity of the substrate involved in the production of flight reactions is tonically

Vagally mediated effects of brain stem dopamine on gastric tone and phasic contractions of the rat.

Dopamine (DA)-containing fibers and neurons are embedded within the brain stem dorsal vagal complex (DVC); we have shown previously that DA modulates the membrane properties of neurons of the dorsal motor nucleus of the vagus (DMV) via DA1 and DA2 receptors. The vagally dependent modulation of gastric tone and phasic contractions, i.e., motility, by DA, however, has not been characterized. With the use of microinjections of DA in the DVC while recording gastric tone and motility, the aims of the present study were 1) assess the gastric effects of brain stem DA application, 2) identify the DA r

Network and intrinsic contributions to carbachol-induced oscillations in the rat subiculum.

Low-frequency network oscillations occur in several areas of the limbic system where they contribute to synaptic plasticity and mnemonic functions that are in turn modulated by cholinergic mechanisms. Here we used slices of the rat subiculum (a limbic area involved in cognitive functions) to establish how network and single neuron (intrinsic) membrane mechanisms participate to the rhythmic oscillations elicited by the cholinergic agent carbachol (CCh, 50-100 microM). We have found that CCh-induced network oscillations (intraoscillatory frequency = 5-16 Hz) are abolished by an antagonist of non

5-HT1-, but not 5-HT2-serotonergic, M1-, M2-muscarinic cholinergic or dopaminergic receptors mediate the ACTH/cortisol response to metoclopramide in man.

The possible mediation of dopaminergic, muscarinic cholinergic and/or serotonergic receptors in the response of ACTH/cortisol to metoclopramide (MCP) was evaluated in 27 normal men. All subjects were tested with MCP (10 mg in an intravenous bolus plus placebo or saline, NaCl 0.9%, control test). For the other tests (experimental tests), the men were divided into three groups of 9 subjects each. One group was tested with MCP in the presence of the dopaminergic agonist bromocriptine (5 mg p.o. 3 h before MCP), another group was tested with MCP plus the M1- and M2-muscarinic-cholinergic antagonis

Neurobiological background for the development of new drugs in schizophrenia.

Psychopharmacology of schizophrenia has remained static for many years because the mechanisms explored have been basically monoaminergics, primarily focused toward the modification of dopaminergic function and, later on, serotonergic. In fact, most of the antipsychotics introduced in clinical practice in the last years have been antagonists or selective agonists of these receptors (D(2)/5-HT(2)). The exploration of other receptor pathways, and in particular those additionally involved in the action of the paradigmatic "atypical" antipsychotic clozapine (ie, cholinergic and noradrenergic), has

On the presence in the cerebral cortex of muscarinic receptor subtypes which differ in neuronal localization, function and pharmacological properties.

The existence in rat frontal cerebral cortex of subtypes of muscarinic receptors was investigated by using as a receptor-mediated functional response the release of neurotransmitters from isolated nerve endings. Synaptosomes prelabeled with [3H]choline or [3H]dopamine were depolarized with 15 mM KCl. Acetylcholine (ACh) concentration-dependently decreased the release of [3H]ACh and increased that of [3H]dopamine. Both actions of ACh were counteracted by the classical muscarinic antagonists atropine and quinuclidinyl benzylate. The two antagonists did not discriminate between the muscarinic pre

Rabbit blastocysts accumulate [3H]quinuclidinyl benzilate in vitro.

Day-6 rabbit blastocysts were able to accumulate [3H]quinuclidinyl benzilate (QNB) from their environment. This accumulation was reduced approximately 50% in the presence of 1.5 x 10(-4) M atropine (an accepted antagonist for ligands which bind to muscarinic cholinergic receptors). The accumulation of QNB was sensitive to temperature and was apparently saturable. In the presence of 2 nM QNB, Day-6 blastocysts accumulated 30.3 +/- 2.0 fmoles per blastocyst. When the cellular elements alone were examined, lesser amounts of specific binding were detected. Owing to the complexity of this multicomp

Pharmacological evaluation of methylcarbamylcholine-induced drinking behavior in rats.

Methylcarbamylcholine (MCC), a structural analog of carbachol (an acetylcholine agonist), has been reported to be a specific nicotinic cholinergic receptor ligand. MCC produces a robust polydipsic response shortly following central administration. The purpose of the present study was to pharmacologically characterize this increase in drinking behavior. Male Wistar rats were implanted with intracerebroventricular (ICV) cannula guides directed at the left lateral ventricle. Following a recovery period, animals were injected ICV with saline or various doses of MCC (3-60 micrograms) and water cons

Glucose and physostigmine effects on morphine- and amphetamine-induced increases in locomotor activity in mice.

Recent findings indicate that glucose antagonizes several behavioral effects of cholinergic antagonists and augments those of cholinergic agonists. For example, scopolamine elicits increased locomotor activity, an action which is attenuated by glucose and by combined treatment with glucose and physostigmine at doses which are individually without effect. Opiate and catecholamine agonists, such as morphine and amphetamine, also elicit hyperactivity. The present study examined interactions of glucose and physostigmine with morphine- and amphetamine-induced hyperactivity. Mice received saline, mo…

[Neural mechanisms underlying stress-induced enhancement of cocaine craving behaviors].

Stress potentiates craving for addictive drugs including cocaine. To elucidate neural mechanisms underlying this effect of stress, we developed an experimental paradigm combining cocaine-induced conditioned place preference (CPP) with a restraint stress. Acute restraint stress exposure immediately before posttest significantly increased cocaine CPP scores. It has been suggested that the extracellular noradrenaline (NA) level is increased by stress in the laterodorsal tegmental nucleus (LDT), which sends cholinergic projections to dopamine (DA) neurons in the ventral tegmental area (VTA), and m

Blood flow in the sciatic nerve is regulated by vasoconstrictive and vasodilative nerve fibers originating from the ventral and dorsal roots of the spinal nerves.

Anesthetized rats were subjected to repetitive electrical stimulation of either the ventral or dorsal root of the spinal nerves between the 11th thoracic and 2nd sacral spinal segments. The response of nerve blood flow (NBF) in the sciatic nerve was examined using laser Doppler flowmetry. For all nerve fibers stimulation was for a 10-30-s period at a supramaximal intensity. (1) Stimulation of the T11-L1 ventral roots produced an increase in mean arterial pressure (MAP) and a biphasic NBF response was comprised of an initial increase and a subsequent decrease. The initial increase was a passive

The effect of chronic atropine treatment on salivary composition and caries in rats.

Many instances of salivary dysfunction in humans can be traced to the use of medications that have hyposalivary side-effects. In this study, atropine, a cholinergic antagonist, was administered chronically to rats by use of osmotic mini-pumps. Steady-state blood levels, similar to levels obtained in human multiple oral dosing, were thus maintained. Atropine delivered in this manner for 24 days was found to decrease protein concentration of parotid saliva (p less than 0.05) elicited by pilocarpine, and to increase smooth-surface caries scores (p less than 0.05) in rats fed a cariogenic diet.
