Formula for the progressive reintroduction of fat into breast milk in infants with chylothorax.

Chylothorax in infants is a potentially life-threatening condition, often resulting from injury during cardiac surgery. Traditionally, breastfeeding is discontinued. Recently, feeding with defatted human milk has been safely introduced. However, methods for fat reintroduction are not standardized. We developed a mathematical formula to facilitate individualized, progressive reintroduction of fat into skimmed breast milk. Fat and protein content were analyzed by Fourier-transform infrared spectroscopy (FTIR). Defatting was performed by centrifugation at 3000 rpm at 2°C for 15 min. The formula c…

Neonatal Course and Long-Term Outcome in Children With Congenital Chylothorax.

BACKGROUND: Congenital chylothorax is a rare pulmonary developmental disorder with high associated morbidity and mortality. A limited number of cases have been described, and clinical manifestations and outcomes vary. METHODS: This study aims to delineate the neonatal and long-term outcome of patients with congenital chylothorax by retrospective and prospective data collection at Ghent University Hospital in Belgium between 1992 and 2022. RESULTS: Eighteen patients with congenital chylothorax were included, seven of whom had a pathology-confirmed diagnosis of congenital pulmonary lymphangiec…

Lymphocytes and immunoglobulins in peripheral blood and lymphatic fluid of neonates with chylothorax.

BACKGROUND: Neonatal chylothorax is associated with high morbidity and mortality, partly due to increased infection risk from lymphocyte depletion and hypogammaglobulinemia. However, data specific to chylothorax cohorts are limited. This study aimed to investigate lymphocyte subsets and immunoglobulin levels in neonates with congenital and acquired chylothorax. METHODS: We retrospectively enrolled 18 neonates with chylothorax admitted to our NICU between January 2023 and January 2025. Inclusion criteria were term or preterm infants with congenital or acquired chylothorax and paired peripheral…

Chylothorax and chylopericardial tamponade following lobectomy and lymphadenectomy: a rare presentation.

BACKGROUND: Although postoperative chylothorax following lung cancer surgery is rare, it is a recognized complication in 0.25-3% of patients. However, cases of cardiac tamponade caused by chylopericardium after lung cancer surgery are extremely rare. CASE PRESENTATION: We describe hitherto unreported sequelae of chyle leak following lobectomy and systematic mediastinal lymph node dissection (SLND) causing pericardial tamponade and cardiovascular compromise. The patient was successfully treated with minimally invasive surgical repair and ligation. We also discuss the development of chyloperica…

Scourge of tuberculosis: A rare case of simultaneous chylothorax and chylous ascites.

Tuberculosis (TB) is a rare cause of chylothorax and chylous ascites. This is a case of simultaneous TB-chylothorax and chylous ascites in a 20-year-old patient who had been diagnosed with disseminated Multi-Drug Resistant (MDR) Tuberculosis two years ago. Abdominal distention with horseshoe-shaped dullness was found on examination. Abdominal ultrasound revealed gross ascites and bilateral gross pleural effusion. Pleural fluid analysis was positive for chylomicrons and revealed elevated Protein, Albumin, ADA, and Triglyceride. GeneXpert was negative and no growth was seen on culture. Lymphosci…

Chylothorax and chylous ascites due to juvenile paracoccidioidomycosis: a case report.

Paracoccidioidomycosis (PCM) is a systemic fungal infection caused by Paracoccidioides species. Chylothorax is a rare complication of PCM. A 16-year-old adolescent presented daily fever, lymphadenomegaly, sweating, weight loss, ventilatory-dependent pain, and dysphagia, which confirmed PCM. During treatment, the patient developed chylothorax and chylous ascites. Chronic inflammatory and fibrotic lymphadenopathy may obstruct lymphatic vessels, resulting in the extravasation of lymph into the abdomen or pleural cavities. Chylothorax is one of several complications of PCM and can lead to respirat…

Post-operative Chylothorax in Patients with Repaired Transposition of the Great Arteries.

Patients with dextro-transposition of the great arteries (d-TGA) require surgical repair as neonates. These patients are at risk for post-operative chylothorax. We sought to describe the presentation, imaging, and outcomes after intervention for patients with d-TGA with post-operative chylothorax. A retrospective chart review was performed in patients with repaired d-TGA who were referred from 1/1/2013 to 4/1/2020 for evaluation of chylothorax. Patient history, lymphatic imaging, and interventional data were collected. Impact of intervention on lymphatic drainage was evaluated with a student's…

Chylothorax as a complication of congenital myotonic dystrophy: A retrospective cohort study.

BACKGROUND: The association between congenital myotonic dystrophy (CMD) and chylothorax is poorly recognized. AIMS: To evaluate the proportion of chylothorax in infants with CMD compared to its prevalence in infants without CMD. STUDY DESIGN: Single-center, retrospective, cohort study. SUBJECTS: Infants managed at a neonatal intensive care unit from 2003 to 2019. OUTCOME MEASURES: CMD was diagnosed using Southern blot for the quantification of CTG repeats in the dystrophia myotonica protein kinase (DMPK) gene. Prenatal and postnatal data of eligible infants with CMD were collected from the…

Congenital Chylothorax: Common and Uncommon Findings in an Infant with Chylous Effusions.

Congenital chylothorax is defined as an abnormal accumulation of lymphatic fluid in the pleural space. It is a rare condition in the neonate that causes significant respiratory, nutritional, and immunologic problems resulting in a high mortality rate. Presented here is a case of congenital bilateral chylothorax in a preterm infant. Fetal ultrasound at 33 weeks' gestation showed polyhydramnios, bilateral pleural effusions, ascites, and subcutaneous edema. Fetal pleuracentesis was done on the right side with 118 mL of fluid aspirated. This article addresses the management of this difficult disea…

A Case of Bilateral Spontaneous Chylothorax with Respiratory Syncytial Virus Bronchiolitis.

A case of bilateral spontaneous chylothorax with respiratory syncytial virus (RSV) bronchiolitis has never been reported. We report the case of a 7-month-old boy born at 33 weeks gestation with a history of Down syndrome, atrial septal defect, pulmonary hypertension, and chronic lung disease, hospitalized due to RSV bronchiolitis who developed bilateral spontaneous chylothorax with exacerbation of pulmonary hypertension (PH). The patient died after 9 weeks of mechanical ventilation and treatment for PH. The autopsy showed acute infectious signs, a chronic interstitial lung disease with pulmona…

Anastomosis of the thoracic duct and the azygos vein for the treatment of recurrent chylothoraxes.

Thoracic duct ligation is a widely accepted treatment option for chylothorax. This case report describes another treatment approach for recurrent chylothoraxes after thoracic duct ligation. A 40-year-old woman had chylothoraxes after thoracic duct ligation. She underwent a secondary surgery to ligate the thoracic duct. Three days later, the number of chylothoraxes increased compared with presurgery, and we anastomosed the thoracic duct and azygos vein after releasing the thoracic duct ligation. The patient recovered smoothly and was followed up for 2 years. This case shows that anastomosis of …

Tuberculosis-Associated Chylothorax: Case Report and Systematic Review of the Literature.

Tuberculosis (TB) is a rare cause of chylothorax. We describe a case and the results of a systematic review of all reported cases of TB-chylothorax. We identified 37 cases of TB-chylothorax. The symptoms at presentation were constitutional (85.7%; 30/35), dyspnea (60.6%; 20/33), and cough (54.5%; 18/33). Chylothorax developed subsequent to the diagnosis of TB in 27.8% (10/36) of the patients, after a median of 6.75 weeks (IQR 4-9). Chylothorax developed during an immune reconstitution syndrome (IRS) in 16.7% (10/36) of the patients, including immunocompetent ones. TB was disseminated in 45.9% …

Prenatal factors associated with neonatal survival of infants with congenital chylothorax.

OBJECTIVES: Congenital chylothorax is a rare disease and prognostic factors are key element in properly informing parents. This study aimed at determining the prenatal factors associated with neonatal survival in a cohort of liveborn infants with congenital chylothorax. STUDY DESIGN: Observational monocentric cohort study including all liveborn neonates consecutively admitted for congenital chylothorax. RESULTS: Neonatal mortality was 32% (16/50). Prematurity (or birth weight), persistence of hydrops at birth and the absence of thoracoamniotic shunt procedure were significantly associated wi…

Association of chylothorax and direct pleura involvement in a case of Waldenström's macroglobulinaemia.

An 82-year-old male was hospitalised for dyspnoea, hypoxaemia and general fatigue; a predominant left chylothorax was revealed. Previously, he had been diagnosed with Waldenström's macroglobulinaemia (WM). Chylothorax complications in patients with WM are rare events and only six such cases have so far been reported. The most common malignant causes of chylothorax are through mediastinal adenopathy. Direct infiltration of the pleura by tumour cells is the most likely cause with this patient, and for this reason, we believe that this case is an instructive one. Chemotherapy induced rapid and pe…

Investigational lymphatic imaging at the bedside in a pediatric postoperative chylothorax patient.

Chylothorax is a rare but serious complication in children who undergo heart surgery. Its pathogenesis is poorly understood, and invasive surgical treatments are considered only after conservative management fails. Current diagnostic imaging techniques, which could aid decision making for earlier surgical intervention, are difficult to apply. Herein, we deployed near-infrared fluorescence (NIRF) lymphatic imaging to allow the visualization of abnormal lymphatic drainage in an infant with postoperative chylothorax to guide the choice of surgical management. A 5-week-old male infant, who develop…

A staged management of prolonged chylothorax in a patient with yellow nail syndrome.

Spontaneous chylothorax remains an unusual condition with diverse aetiologies and non-unified management strategies. Owing to the rarity of the condition, the evidence of management remains from case reports and limited retrospective studies. This case represents a difficult-to-manage chylothorax secondary to yellow nail syndrome with initial failure of both surgical and conservative treatment methods. Pleurovenous shunting represents a surgical management approach allowing the patient to live with and control an ongoing chyle leak.

A novel treatment approach for paediatric Gorham-Stout syndrome with chylothorax.

AIM: To expand the treatment options in paediatric Gorham-Stout syndrome (GSS) when conventional therapy is ineffective. METHOD: Two children with biopsy confirmed GSS, a rare disorder with progressive lymphangiomatosis, were treated with a combination of interferon-α-2b, low anticoagulant, low molecular weight heparin, radiotherapy and surgery. RESULTS: The combined therapy resolved the symptoms in the acute phase, and both patients have since been free of symptoms for >2 years. CONCLUSION: The successful addition of a low anticoagulant, low molecular weight heparin (tafoxiparin) to the tr

Gorham-Stout syndrome with chylothorax in a six-year-old boy.

Gorham-Stout syndrome, also called "disappearing bone disease, diffuse cystic angiomatosis of bone, disseminated lymphangiomatosis, Gorham's vanishing bone disease, phantom bone disease or idiopathic massive osteolysis, is a rare disease of unknown etiology and pathogenesis. It is characterized by rapidly progressive localized massive osteolysis associated with proliferation of vascular structures of benign origin in which the absence of new bone formation is representative. When it is complicated by chylothorax, the prognosis is poor. The authors report a 6-year-old boy with Gorham-Stout synd…
