Syphilitic osteomyelitis in a patient with HIV and cognitive biases in clinical reasoning: A case report.

RATIONALE: Diagnosing multifactorial, multidimensional symptoms unexplained by presumptive diagnosis is often challenging for infectious disease specialists. PATIENT CONCERNS: We report a rare case of a 30-year-old Japanese bisexual man with a history of virally suppressed human immunodeficiency virus and syphilis infections who developed chest pain and an erosive lesion under the lower midline jaw. DIAGNOSIS: Imaging examinations revealed erosive lesions on the sternum and left the ninth rib. Biopsy and polymerase chain reaction testing of sternal tissue specimens were noncontributory. Howe

Mapping the Social Interaction Architecture of Clinical Reasoning in Morning Report: A Mixed-Methods Social Network Analysis.

INTRODUCTION: Morning Report (MR) is a workplace-based learning activity in which clinical reasoning develops through interaction among learners and clinical teachers. Although previous studies have examined the educational value of MR, less is known about how clinical reasoning is socially constructed through patterns of communication and participation. This study aimed to investigate the interactional architecture of MR using Mixed-Methods Social Network Analysis (MMSNA). METHODS: We conducted a qualitatively driven data transformation mixed-methods study using verbatim transcripts from 20 …

Fuzzy cognitive explainable AI framework integrating ResNet-50 and causal clinical concept reasoning for skin lesion detection.

Melanoma is one of the most aggressive forms of skin cancer, and its early detection from dermoscopic images remains a significant clinical challenge. Deep learning (DL) convolutional neural networks (CNNs) have achieved remarkable results in medical image analysis. However, their inherent opacity is a barrier to trustworthy clinical adoption. Clinicians need not only accurate predictions but also clear, understandable reasons behind those predictions. Although post-hoc explainability techniques such as Grad-CAM and SHapley Additive exPlanations (SHAP) are available, but there is a critical ga

Case-based reasoning for clinical trial recruitment tools in oncology: When you need patients to find patients.

BACKGROUND: Patient recruitment for clinical trials remains a major challenge, with 86% of trials failing to meet enrollment targets on time. In over 77% of cases, recruitment difficulties stem from matching problems between trials and patients. Case-Based Reasoning (CBR) offers a distinct patient-to-patient approach by determining eligibility through comparison with previously enrolled patients, yet this methodology remains underexplored in contemporary oncology trial matching despite its potential advantages. OBJECTIVE: To compare the performance of two CBR approaches-random forest (RF) and

Artificial intelligence (AI)-assisted diagnosis of skin diseases: From image classification to dermatology-specific multimodal clinical reasoning.

Artificial intelligence (AI) in dermatology has moved beyond the early paradigm of single-image classification. Dermatological diagnosis is … entral argument: the field must progress from lesion recognition to dermatology-specific multimodal clinical reasoning. We summarize major advances in convolutional neural networks, dermoscopic benchmarks, clinical-image datasets, large language models, vision-language systems, and dermatology foundation models. We also analyze challenges that are particularly relev …

Can radiology requisition quality reflect clinical reasoning? Insights from a RI-RADS evaluation of emergency CT referrals.

OBJECTIVES: To determine radiology requisition quality using reason for exam imaging reporting and data system (RI-RADS), and the associated clinical variables, and whether it can reflect the clinical reasoning quality in emergency CT referrals. MATERIALS AND METHODS: This single-center retrospective study randomly selected emergency CT referrals between 01 January 2024 and 31 December 2024. One radiologist scored the requisition quality using the RI-RADS, and assessed the clinical reasoning quality, evaluating the extent to which the differential diagnoses on the requisition form matched the…

Clinical Reasoning: Acute Monocular Vision Loss in a Patient With Ipsilateral Extracranial Chronic Internal Carotid Artery Occlusion.

We report the case of a middle-aged man who presented with acute painless monocular vision loss. His medical history was remarkable for chronic total occlusion of the ipsilateral internal carotid artery (ICA) and a recent carotid endarterectomy (CEA) on the contralateral ICA. In a stepwise multidisciplinary approach assessment, we review the differential diagnosis of acute vision loss and investigate how the patient's intracranial and extracranial hemodynamic reorganization after chronic ICA occlusion may affect the clinical reasoning. Early complications of CEA and the differential diagnosis …

Clinical Reasoning: A 47-Year-Old Man With an Upper Respiratory Infection, Acute Confusion, Dysarthria, and Ataxia.

A patient presenting with acute confusion, dysarthria, and appendicular ataxia with gait instability warrants a broad differential including emergent consideration of acute ischemic or hemorrhagic stroke. Moreover, in acute to subacute presentations, a wide array of etiologies including infectious causes, toxins, or autoimmune conditions may be considered. This article features a 47-year-old man who presented acutely with confusion, severe dysarthria, left upper extremity dysmetria, and unsteady gait. In this case, these neurologic signs were preceded by symptoms of an upper respiratory infect

Physiotherapists' perceptions on using a multidimensional clinical reasoning form during psychologically informed training for low back pain.

PURPOSE: Building clinical reasoning skills is important to effectively implement psychologically informed practice. We developed a multidimensional clinical reasoning form (CRF) to be used by physiotherapists in a psychologically informed practice training programme for low back pain. In this paper we describe the development of the CRF, how the CRF was used in the training, and present an evaluation of physiotherapists' perceptions of the CRF. METHODS: Qualitative semi-structured interviews were conducted with ten physiotherapists purposively sampled in primary care. Data were gathered thro…

Clinical Reasoning: A 57-Year-Old Man With Stepwise Progressive Paraparesis, Sensory Loss, Urinary Retention, and Constipation.

We present the case of a 57-year-old man with protein S deficiency and left leg deep vein thrombosis (DVT) 5 years earlier, who developed stepwise progressive bilateral lower limb weakness, numbness/paresthesia, gait imbalance, hesitancy of micturition, and constipation in the setting of recurrent left common femoral DVT treated with apixaban. Symptoms amplified with Valsalva, corticosteroids, and postlumbar puncture, with longitudinally extensive midthoracic T2-hyperintense lesion extending to the conus associated with hazy holocord enhancement on magnetic resonance imaging (MRI), raising sus

Clinical Reasoning: a girl presenting with stiffness episodes during sleep, cafe-au-lait spots, and flecked retina.

A 4-year-old girl who had been born of normal pregnancy and delivery and had an unremarkable family or personal history was referred to a neuropsychiatric department because of the appearance of peculiar nocturnal episodes. Parents described that their child abruptly became stiff during sleep. These episodes usually ranged from 20 to 40 seconds, and after that the child continued to sleep. Initially she presented 1 episode per week, but there was a progressive increase in frequency up to 3 to 4 times per night. The child never presented similar episodes while awake. Her examination revealed so

[Clinical reasoning and decision-making in practice. A young boy with fever, pancytopenia and an enlarged spleen].

A 5-year-old boy presented with fever and fatigue after a holiday in northern Italy. On physical examination a marked splenomegaly was found. Laboratory investigations showed a pancytopenia as well as several markers suggesting an autoimmune disease. The splenomegaly and pancytopenia continued to progress despite treatment with prednisolone and intravenous immunoglobulins. One and a half years after presentation, the spleen had grown to such an extent that it was causing mechanical problems. Splenectomy was performed for diagnostic and therapeutic purposes. Histological investigation of the sp

[Clinical reasoning and decision making in practice. A 39-year-old woman with somnolence, hypertension and haemolysis].

A 39-year-old woman was admitted with somnolence, severe hypertension and thrombotic microangiopathy. Both malignant hypertension and thrombotic thrombocytopenic purpura (TTP) were considered. Immediate therapy was instituted to treat both diseases because of severe clinical deterioration. Eventually, TTP was considered less likely due to the presence of grade IV hypertensive retinopathy (papilloedema and soft exudates) and a normal Von Willebrand factor-cleaving protease level. Differentiating TTP from malignant hypertension can be difficult as both diseases have similar clinical, laboratory
