Synthesis and in vitro antifungal activity of isoniazid-derived hydrazones against Coccidioides posadasii.

Coccidioidomycosis is a potentially severe infection caused by dimorphic fungi Coccidioides immitis and Coccidioides posadasii. Although guidelines are well established, refractory disease is a matter of concern in the clinical management of coccidioidomycosis. In the present study three isoniazid-derived hydrazones N'-[(E)-1-(4-methoxyphenyl)ethylidene]pyridine-4-carbohydrazide, N'-[(E)-1-(4-methylphenyl)ethylidene]pyridine-4-carbohydrazide, and N'-[(E)-1-(phenyl)ethylidene]pyridine-4-carbohydrazide were synthesized and evaluated for antifungal activity against C. posadasii. Susceptibility as…

An atypical morphologic presentation of Coccidioides spp. in fine-needle aspiration of lung.

Infection due to Coccidioides spp., a dimorphic fungal pathogen, usually presents as a chronic pulmonary disease, occasionally with pulmonary nodules. On cytology, large spherules filled with endospores are typically seen. We report an unusual case of coccidioidomycosis in a 39-year-old female from an area nonendemic for Coccidioides and without other known risk factors for infection. Fine-needle aspiration of the patient's cavitary lung lesion revealed Coccidioides spp., which demonstrated atypical delicate septate hyphal forms and chains of conidia, with none of the large spherules typical o…

Acute eosinophilic pneumonia complicating Coccidioides immitis pneumonia: a case report and literature review.

Though peripheral eosinophilia commonly accompanies infection with Coccidioides immitis, pulmonary eosinophilia in patients with C. immitis and multilobar infiltrates has been reported in only three prior cases to our knowledge. To enhance clinicians' suspicion of an unusual complication of C. immitis pneumonia and the treatment implications, we present a patient with an initial diagnosis of acute eosinophilic pneumonia [based on bronchoalveolar lavage (BAL) fluid analysis and transbronchial biopsy] followed several days later by a diagnosis of accompanying C. immitis pneumonia when BAL fluid …

Unusual forms of immature sporulating Coccidioides immitis diagnosed by fine-needle aspiration biopsy.

Coccidioidomycosis is an endemic infection acquired by inhalation of the spores (arthroconidia) of the thermally dimorphic fungus, Coccidioides immitis. The arthroconidia transform into spherical cells called mature spherules in the lung. Immature spherules and other atypical forms of immature C immitis have rarely been found in vivo. We report on a case that presented unusual forms of immature sporulating C immitis in a fine-needle aspiration specimen. A 36-year-old Chinese woman, living in New Jersey for the past 10 years, presented with fever, night sweats, hemoptysis, and an abnormal chest…

Disease surveillance in recombining pathogens: multilocus genotypes identify sources of human Coccidioides infections.

Molecular surveillance of pathogenic microbes works by genotyping isolates with DNA fingerprinting techniques and then using these genotypes … populations. We illustrate the usefulness of this technique by inferring the source populations for Coccidioides isolates recovered from patients treated outside the endemic area of Coccidioides sp., the etiological agents of human coccidioidomycosis, but with a travel history including visits to one or more endemic areas.

Sequence, expression and functional analysis of the Coccidioides immitis ODC (ornithine decarboxylase) gene.

The ornithine decarboxylase (ODC) gene of the human respiratory fungal pathogen, Coccidioides immitis (Ci) was cloned, sequenced, chromosome-mapped, and expressed in Escherichia coli (Ec). The genomic, cDNA and translated sequences are presented. Transformation of an ODC null mutant strain of Ec (EWH 319) with the Ci ODC gene was conducted to confirm function of the protein encoded by the fungal gene. Activity of the enzyme by the bacterial transformant was inhibited by 1, 4-diamino-2-butanone (DAB), a known inhibitor of eukaryotic ODC. Temporal expression of the Ci ODC gene during the parasit…

Immunogenicity of a 48-kilodalton recombinant T-cell-reactive protein of Coccidioides immitis.

The outcome of coccidioidomycosis depends to a large extent on the effectiveness of the T-cell-mediated immune (CMI) response to the fungal pathogen. For this reason, identification of Coccidioides immitis antigens which stimulate T cells is important for understanding the nature of host defense against the organism and essential for the development of an effective vaccine. Here we describe the immunogenicity of a 48-kDa T-cell-reactive protein (TCRP). The antigen is expressed by parasitic cells and localized in the cytoplasm. It stimulates the proliferative response and production of gamma in…

Mapping of a Coccidioides immitis-specific epitope that reacts with complement-fixing antibody.

We have previously cloned the cDNA fragment that encodes the complement fixation antigen of Coccidioides immitis. The recombinant protein was highly sensitive in detecting CF antibody in sera from patients with coccidioidomycosis but was not specific to C. immitis, as evidenced by its reactivity with sera from patients with histoplasmosis and, to lesser extent, blastomycosis. We undertook this study to determine if the epitope(s) that reacts with CF antibody is the same or differs from the epitopes that are shared with Histoplasma capsulatum and Blastomyces dermatitidis. PCR-generated CF/chiti…

Antigen complex of Coccidioides immitis which elicits a precipitin antibody response in patients.

The occurrence in patients of elevated levels of immunoglobulin M (IgM) precipitin antibody to Coccidioides immitis antigens, which are commonly detected by the immunodiffusion-tube precipitin (TP) assay, is suggestive of primary nondisseminating coccidioidomycosis. We previously demonstrated that the concanavalin A-bound mycelial culture filtrate plus lysate preparation is a source of at least two TP antibody-reactive antigens (TP-Ags), which were identified by sodium dodecyl sulfate-polyacrylamide gel electrophoresis as 120- and 110-kDa fractions. Evidence is presented here that the crude fi…

[Immunochemical properties of the extracellular hydrolases (protease and alkaline phosphatase) of Coccidioides immitis].

Extracellular hydrolases (protease and alkaline phosphatase) of the coccidioidal fungus possessed antigenic properties and caused production of the corresponding antibodies. Phosphatase-antiphosphatase-substrate system apparently has future prospects for the elaboration of immunobiochemical methods for the diagnosis of coccidioidomycosis.

Man-in-the-barrel syndrome caused by cervical arachnoiditis in refractory coccidioidal meningitis.

We report a case of refractory coccidioidal meningitis presenting with bilateral upper-extremity weakness in a man-in-a-barrel distribution caused by cervical arachnoiditis, adhesive bands, and spinal cord compression. The patient was treated sequentially with fluconazole, other azole therapy, liposomal amphotericin B, intrathecal amphotericin B, and olorofim, with only partial disease control. Neuroimaging demonstrated progressive cervical leptomeningeal inflammation, cord tethering, and CT myelography illustrated adhesive arachnoiditis. Surgical lysis of adhesions and placement of a spinal O

Coccidioidal Meningitis in an Immunocompetent Patient: A Rare Central Nervous System Infection.

Coccidioidomycosis is a systemic fungal infection caused by Coccidioides immitis and Coccidioides posadasii. Although well recognized in parts of the southwestern United States, it remains underdiagnosed in Mexico and Central America. While most cases present as self-limited pulmonary infection, approximately 1% of immunocompetent individuals develop disseminated disease, with coccidioidal meningitis (CM) being the most severe manifestation. We describe a 46-year-old immunocompetent male from Oaxaca, Mexico, who presented with progressive headache, neurocognitive decline, and altered mental st…

Disseminated Coccidioidomycosis and Coccidioidal Meningitis Hospitalization, Texas, USA, 2016-20231.

We analyzed inpatient data from Texas, USA, to characterize hospitalizations for disseminated coccidioidomycosis and coccidioidal meningitis from 2016-2023. Geographic mapping revealed a substantial disease effect in both western Texas and metropolitan areas. Our findings suggest the need for enhanced surveillance and increased healthcare provider awareness regarding coccidioidomycosis in Texas.

Trapped Below: Abdominal Pseudocysts Associated With Ventriculoperitoneal Shunts in Coccidioidal Meningitis.

Ventriculoperitoneal (VP) shunts are the mainstay for cerebrospinal fluid diversion in patients with refractory coccidioidal meningitis (CM) … than previously reported in patients with hydrocephalus due to other etiologies. Evidence of active Coccidioides infection was present in 71% of patients. Imaging typically revealed fluid collections at the distal catheter tip, and most patients underwent surgical intervention targeting the distal shunt. Despite intervention, recurrent shunt failures occurred …

Emergence of Coccidioides posadasii in an asymptomatic Ecuadorian patient with diabetes: A case report.

Coccidioidomycosis is a potentially serious but poorly studied mycosis in Ecuador. It is not a notifiable infection; therefore, its true incidence and prevalence in Ecuador remain unknown. Because primary lung disease due to coccidioidomycoses is typically self-limiting, it does not usually require treatment. Here, we present the first case of Coccidioides posadasii infection in an asymptomatic patient with diabetes without a history of travel to an endemic area. This is the first case reported in Ecuador in the last half-century.

Development of a rapid lateral flow assay for detection of anti-coccidioidal antibodies.

Coccidioides spp. are dimorphic fungi that are capable of infecting human and non-human mammals and can cause diverse manifestations of coccidioidomycosis or Valley fever (VF). In combination with clinical symptoms and radiographic findings, antibody-based diagnostic tests are often used to diagnose and monitor patients with VF. Chitinase 1 (CTS1) has previously been identified as the seroreactive antigen used in these diagnostic assays to detect anticoccidial IgG. Here, an indirect enzyme-linked immunosorbent assay to detect IgG to CTS1 demonstrated 165 of 178 (92.7%) patients with a positive…

Coccidioidal Meningitis in New York Traced to Texas by Fungal Genomic Analysis.

A child developed hydrocephalus. Sixteen months later, it was discovered to be a complication of coccidioidal meningitis. The infection's source was uncertain until genomic analysis of the fungal isolate identified its origin to be a visit to Beeville, Texas. Improved national reporting of cases of coccidioidomycosis might reduce diagnostic delays.

Coccidioides immitis and posadasii; A review of their biology, genomics, pathogenesis, and host immunity.

Coccidioides immitis and C. posadasii are two highly pathogenic dimorphic fungal species that are endemic in the arid areas of the new world, including the region from west Texas to southern and central California in the USA that cause coccidioidomycosis (also known as Valley Fever). In highly endemic regions such as southern Arizona, up to 50% of long term residents have been infected. New information about fungal population genetics, ecology, epidemiology, and host-pathogen interactions is becoming available. However, our understanding of some aspects of coccidioidomycosis is still incomplet…
