How to influence health providers.

In spite of the recognition that Helicobacter pylori is responsible for most duodenal ulcer and peptic ulcer and that it is the underlying cause of most gastric cancer, little or no attention has been paid to its effect upon public health and in particular no measures have been put in place to address them. For over 20 years relatively inexpensive and noninvasive testing and treatment have been available, but no country has introduced a "screen-and-treat" policy, and relatively little research has been devoted to the identification of a vaccine for the disease. Health-care providers and doctor

Bioterrorism: the need to be prepared.

Postal distribution of anthrax spores in October 2001 in the USA resulted in cases of pulmonary anthrax. In consequence, interest and concern about terrorist attacks on civilian populations using biological weapons have increased, particularly when one recent authoritative assessment suggested that an attack using some form of unconventional weapon on a Western city was 'inevitable'. This article reviews the steps necessary to minimise the probability of a successful attack. Despite best endeavours, the possibility remains that significant numbers of casualties will arise, emphasising the need

[Fast prophylaxis for last-minute travelers. Which measures are still possible 1 week before traveling?].

Vaccinations or booster injections against tetanus, diphtheria and polio shortly before leaving on a journey are both possible and to be recommended. Active hepatitis-A-vaccination can also be applied immediately prior to the journey, and offers better protection than gamma globulins. As a rule, vaccinations against hepatitis B, yellow fever and typhoid must be given one to four weeks before the journey. Effective malaria prophylaxis for last-minute travellers is always possible. In addition to mandatory "exposure prevention", effective chemoprophylaxis is also recommended for travellers to tr

[Eradication of contagious diseases].

Based on analysis of eleven-year intense epidemiological intervention against smallpox, a number of findings and demands ensued which should be met by an infectious disease to be included into the programme of eradication or elimination. The author mentions several episodes from the programme of smallpox eradication in which he participated as a member of a WHO team. Part of the paper is a detailed explanation of the terms eradication and elimination. The main part of the article is a characteristic of infections where the global programme of eradication or elimination is underway. At present

General practice awareness of notifiable infectious diseases.

The Acheson Report concluded that the process of infectious disease notification in England and Wales was unsatisfactory and recommended that it should be reviewed. However, the success of any notification system will depend on the knowledge and motivation of general practitioners, who are responsible for a large proportion of infectious disease notifications. A district-wide telephone survey was conducted in Croydon among general practitioners to assess the level of awareness of which diseases are on the statutory notification list. Respondents' opinions were also sought on the composition of

Recommendations of the International Task Force for Disease Eradication.

This report summarizes the conclusions of the International Task Force for Disease Eradication (ITFDE), a group of scientists who were convened by a secretariat at the Carter Center of Emory University six times during 1989-1992. The purpose of the ITFDE was to establish criteria and apply them systematically to evaluate the potential eradicability of other diseases in the aftermath of the Smallpox Eradication Program. The ITFDE defined eradication as "reduction of the worldwide incidence of a disease to zero as a result of deliberate efforts, obviating the necessity for further control measur

[Evaluation of the information provided by the system of compulsory notification of diseases].

In order to assess activities of epidemiological surveillance resulting from the statutory notification system, a total of 17,394 notification records of eight infectious diseases (brucellosis, bacillary dysentery, typhoid fever, viral hepatitis, meningococcal infection, rickettsioses other than exanthematous typhus, pulmonary tuberculosis, and tuberculosis of other organs) together with 10,503 epidemiological surveys submitted to the "Servei Territorial de Salut Pública" of the province of Barcelona between 1982 and 1986 were reviewed. In notification records, data to locate physicians were t
