The main determinants of community participation in disease control programmes are identified and a framework with eleven variables is developed. Attention is drawn to the political background, community characteristics, the managerial capacity of the provider and the epidemiology of the disease. The framework is designed to guide health professionals in the systematic assessment and monitoring of participation in disease control programmes. The endemic mycoses--histoplasmosis, blastomycosis, and coccidioidomycosis-pose a common problem in the differential diagnosis of the atypical pneumonia syndrome. Patients admitted to hospital with chest infections were examined serologically to see if these were due to Legionella pneumophila. In a rural community of 80,000 people 69 patients were identified as having a diagnosis of Parkinson's disease. After interview and examination we found that 55 met the generally accepted diagnostic criteria for Parkinson's disease, 4 had possible Parkinson's disease, 6 had essential tremor, 2 had dementia and 2 had other conditions. Rehabilitation in the Philippines has in the past been based on the Western model, with an emphasis on hospital departments located in the major cities. This approach is inappropriate for the majority of disabled people in the Republic as 70% of the population live in rural areas. A community-based programme was devised using local volunteers who had simple training. These volunteers can identify and support disabled people in their own villages, avoiding long journeys and expensive institutional care.

A questionnaire was prepared to asses community pharmacists' perceptions of their abilities to recognize the common signs and symptoms of cancer and their preferred methods for obtaining further education. The preferred treatment of hyperthyroidism remains controversial. Most of this data is derived from large, university-based medical centers. We report here our experience with treatment of hyperthyroidism in a community setting. This involves 144 patients with hyperthyroidism who were seen over a 10 year period at Michigan State University Clinical Center and were treated in the community hospitals and private physicians' offices, and by community surgeons. Follow-up data were available on 119 of these patients; 105 of them were hyperthyroid because of Graves' disease and multinodular goiter.
