OBJECTIVE: The COVID-19 pandemic profoundly disrupted outpatient services with challenges including cancellations and delays in consultations, diagnostics, and surgeries. The Acute Hand Program at Trillium Health Partners examined the pandemic's impact as a catalyst for improving an outpatient model of care.
METHODS: In 2020, the Acute Hand Program redesigned its care model. This multiyear, quality improvement plan included seven change initiatives ranging from physical space reconfiguration to workflow, admission and discharge criteria revisions, and leveraging community resources.
RESULTS:

BACKGROUND: Xerostomia (dry mouth) is a common yet severe problem, causing difficulty with eating, speaking, dental caries, and discomfort. Access issues to dental services, including cost and availability of dental practitioners, can delay timely diagnosis and management of dry mouth. Thus, there is a need for innovative approaches to manage xerostomia, involving non-dental primary care health practitioners.
METHODS/DESIGNS: This proof-of-concept study assessed the feasibility of a co-designed oral health education resource to enable pharmacists to support their clients in managing xerostomi

BACKGROUND: The increased number of older adults living alone has created a substantial care need. However, the utilization rate of services and facilities to meet these needs are surprisingly low. Many of older adults experience difficulties accessing these services, although it remains unclear how these obstacles impede access to services. This study explored the obstacles and difficulties experienced by urban older adults in seeking community care.
METHODS: A phenomenological study was carried out and participatory observation and in-depth interviews were employed to investigate the proces

BACKGROUND: In developing countries, satellite-based technology can aid critical telemedicine applications and other digital health services in critically underserved areas. Affordable, high-speed broadband services can and should be accessible to all citizens. Remote locations are necessary to support various critical services, including education and training, telehealth applications, remote patient monitoring, and warning systems, particularly during disasters. Currently, however, these services are limited to urban centers, leaving rural areas without access to specialized health care serv

Obstetric complications remain a leading cause of preventable maternal and neonatal morbidity and mortality in low- and middle-income countries (LMICs). Persistent barriers to facility-based antenatal care (ANC), including geographic remoteness, transportation costs, socio-cultural norms, and fragmented health systems, have prompted the large-scale expansion of community-based ANC models designed to bring preventive and early-detection services directly to underserved populations. This systematic review evaluated the impact of community-based ANC models on the prevention and early detection of

Community Based Organizations (CBOs) are important health system stakeholders with the mission of addressing the social and economic needs of individuals and groups in a defined geographic area, usually no larger than a county. The access and success efforts of CBOs vary, depending on the integration between health care providers and CBOs but also in relation to the community participation level. To achieve widespread results, it is important to carefully design an efficient network which can serve as a bridge between the community and the health care system. This study addresses this challeng

Limited research exists regarding the most aggressive forms of hepatoblastoma. Cell lines of the rare subtypes of hepatoblastoma with poor prognosis are not only difficult to attain but also challenging to characterize histologically. A community-driven approach to educating parents and families, regarding the need for donated tissue, is necessary for scientists to have access to resources for murine models and drug discovery. Herein, we describe the currently available resources, existing gaps in research, and the path to move forward for uniform cure of hepatoblastoma.

Across diverse microbiotas, species abundances vary in time with distinctive statistical behaviors that appear to generalize across hosts, but the origins and implications of these patterns remain unclear. Here, we show that many of these macroecological patterns can be quantitatively recapitulated by a simple class of consumer-resource models, in which the metabolic capabilities of different species are randomly drawn from a common statistical distribution. Our model parametrizes the consumer-resource properties of a community using only a small number of global parameters, including the tota

The use of prescribed medication was examined in 34 people with learning disability who underwent a planned resettlement from a hospital into the community. No significant differences were found in the number of subjects receiving antipsychotic, antimuscarinic, antidepressant, and anticonvulsant medication, lithium preparations, and non-psychoactive medication, just prior to discharge and 1 year after discharge. This study indicates that long-stay hospitalised adults with moderate, severe or profound mental retardation can successfully be relocated into the community--in spite of suffering fro

BACKGROUND: The increasing availability of voluntary assisted dying (VAD) signifies a major shift in how communities perceive and experience death. Access to VAD can be seen as a new step in the shared meaning-making through which communities respond to death and dying and a new opportunity to engage communities in developing death literacy. The Australian Capital Territory (ACT) VAD legislation took effect in late 2025.
OBJECTIVES: This paper describes the initial co-creation of a multi-phase project (Getting Ready) seeking to develop end-of-life choices education in partnership with ACT com

Posttraumatic stress disorder (PTSD) significantly influences individuals' daily life, yet little is known about their experiences of occupational participation and the supporting resources they use. To explore the lived experiences of occupational participation among adults with PTSD and identify resources that support their participation in everyday life. Employing a qualitative design grounded in descriptive phenomenology, remote semi-structured interviews were conducted with 10 community-dwelling adults diagnosed with PTSD. Data were processed through reflexive thematic analysis. Analysis

AIMS AND OBJECTIVES: Examined the levels of burden and the coping strategies experienced by caregivers of persons with schizophrenia in the community. It aims (1) to provide mental healthcare professionals with more information about caregiver stressors and (2) to recommend more effective community resources and support.
BACKGROUND: A caregiver's burden increases due to negative coping skills and lack of resources. However, insufficient studies of caregiver burden and coping skills accentuate the already inadequate understanding of the field. This lack of understanding often leads to frequent

OBJECTIVE: The study tests a community- and data-driven approach to homelessness prevention. Federal policies call for efficient and equitable local responses to homelessness. However, the overwhelming demand for limited homeless assistance is challenging without empirically supported decision-making tools and raises questions of whom to serve with scarce resources.
MATERIALS AND METHODS: System-wide administrative records capture the delivery of an array of homeless services (prevention, shelter, short-term housing, supportive housing) and whether households reenter the system within 2 years

Previous studies have shown that socioeconomic factors play an important role in multiple myeloma (MM) health outcomes. We postulated that the type of treatment facilities and their volume of cases also affect overall survival, utilization of various therapies including palliative care services in newly diagnosed MM. Using the National Cancer Database (NCDB), we analyzed 174,551 newly diagnosed MM participants from across the country. We found that at high volume facility centers (over 90th percentile of new patient volume from 2004 to 2016), the median overall survival (OS) was 62.3 months ve

This study investigates the complexities faced by emergency managers in wildfire-prone areas to uncover pressing issues and potential solutions. Four themes are discerned through three focus group discussions with emergency managers from nine counties across California. First, there is unequal access to resources for both risk assessment and response, with counties that have fewer resources facing significant challenges in effectively managing wildfire risks. Second, effective risk communication depends on the available resources and the unique characteristics of each community. Participants s

Exposure to a persistent terrorist threat constitutes a continuous traumatic situation (CTS) that can severely impact one's mental and physical health. For older adults living in CTS, this risk is compounded by the challenges of aging. This vulnerability is significantly heightened for older adults living alone in CTS, who typically experience increased social isolation and loneliness. Past research has explored internal coping resources that help these individuals deal with their stressful circumstances. Nevertheless, external coping resources have been hardly explored, making it difficult to
