A Case of Confusion in an Obese Patient Treated With Daptomycin: Neurotoxicity.

Daptomycin (DAP) is a cyclic lipopeptide antibiotic with bactericidal activity against gram-positive bacteria. The most common adverse reaction is myotoxicity characterized by rhabdomyolysis. Other reported adverse reactions include gastrointestinal symptoms, skin lesions, bleeding, and pulmonary involvement. Neurotoxicity is rare and its mechanism remains partially elucidated. We report a case of confusion consistent with DAP-induced neurotoxicity. A 73-year-old obese man was treated with DAP 9 mg/kg for methicillin-resistant Staphylococcus aureus (MRSA) bacteremia associated with foot osteit…

Not to be confused, this is a pleural fibroma: Case report and literature review.

Solitary fibrous tumors of the pleura are rare mesenchymal tumors. The diagnosis is often made incidentally. We report the case of 65 year old patient consulting for dry cough evolving for 1 year. The clinical examination revealed a mattness of the right hemi thorax. The chest X-ray detected a right lower lobar opacity. The CT scan showed a right lateral basal mass, connected at an obtuse angle with the wall. A CTguided biopsy with an anatomopathological and immunohistochemical examination were performed, thus confirming the diagnosis of a solitary fibrous pleural tumor. The evolution after tr

Dazed and Confused: Altered Mental Status in an Adolescent Male.

BACKGROUND: Absence status epilepticus (ASE) is a form of generalized nonconvulsive status epilepticus. ASE is characterized by impairment in consciousness, which can vary widely, making the diagnosis more difficult. The typical patient with ASE will be confused yet responsive and in a "trance-like state" with delayed speech, clumsy gait, and the ability to perform simple tasks after prompting. With treatment, typical ASE has an excellent prognosis and does not appear to be associated with significant neuronal damage. CASE PRESENTATION: An 11-year-old boy with history of febrile seizures pres

Images of the month: Verruciform xanthoma: an easily confused disease.

Verruciform xanthomas occur primarily in the mouth, but sometimes in the anogenital region, which may cause clinical confusion with genital warts. We present a case where a 70-year-old man presented with an asymptomatic warty lesion on the scrotum and verruciform xanthoma was confirmed by histopathological results.

MALPOSITIONED LMA CONFUSED AS FOREIGN BODY IN NASAL CAVITY.

We present a case of confusing white foreign body in the nasal cavity detected during Endoscopic Sinus Surgery (ESS) in a 35-yr-old male which turned out to be a malposition of classic laryngeal mask airway (LMA). Although malposition of LMA is a known entity to the anesthesiologist, if ventilation is adequate, back folded LMA in nasal cavity might not be recognized by the surgeon and lead to catastrophic consequences during endoscopic sinus surgery. In principle, misfolding and malpositioning can be reduced by pre usage testing, using appropriate sizes, minimizing cuff volume, and early ident

[The incubus phenomenon: A sleep-related condition, not to be confused with psychosis].

BACKGROUND: The incubus phenomenon is a paroxysmal condition characterised by sleep paralysis coinciding with a sensed presence and the frightening sensation that something or somebody is exerting pressure on the thorax. AIM: To present an overview of the literature on the incubus phenomenon and to provide practical advice regarding diagnosis and treatment. METHOD: To obtain information for our paper we carried out a search in PubMed and the historical literature. RESULTS: The incubus phenomenon is a relatively unknown condition, even though, world-wide, it has a lifetime prevalence rate of

Giant cerebral cysticercosis in an infant confused with a thalamic glioma.

Neurocysticercosis is a common parasitic infection of the central nervous system. Intraparenchymal giant cysticercosis has been described in literature, but this is a rare report of a thalamic giant cysticercosis in a young child where the diagnosis could be made on follow-up. A 1½-year-old male child presented with seizures, hemiparesis, and features of raised intracranial pressure. Initial neuroimaging findings of thalamic swelling with minimal edema and contrast enhancement with choline peak on magnetic resonance spectroscopy were attributed to thalamic glioma. Subsequent imaging revealed a

Confusing medical terms: disease that may or may not exist.

Patients often quote diseases or illnesses that either do not exist per se or are hard to prove that they exist. Often symptoms are vague and, therefore, difficult for patients to qualify in a language clinicians can understand, interpret and act upon. Physicians often perpetuate this by giving 'diagnoses of exclusion', or using poor explanations, oversimplifications, conflicting diagnostic criteria or vague historical terms that have now evolved into something else. However, the history taker must be able to interpret the subtle language barrier that exists between doctor and patient. In this

Transient neurologic syndrome: a benign but confusing clinical problem.

Spinal anesthesia has been a safe and popular anesthetic option for patients undergoing outpatient surgical procedures of the trunk and lower extremities. Occasionally, after a spinal anesthetic, patients can develop moderate-to-severe pain in the back, hips, and legs without neurologic deficit. They will often present to the emergency department with complaints of pain and require an extensive diagnostic workup to rule out other more ominous possibilities such as spinal hematoma, infection, or nerve injury. After a negative workup and with a history of recent spinal anesthetic the patient wil

Terminology of lower urinary tract symptoms. Helpful or confusing?

An established standardized terminology is necessary for communication of scientific information, and for prevention of mistreatment and misdiagnosis. Terminology concerning the lower urinary tract has been much discussed; in particular, the meaning of terms like lower urinary tract symptoms (LUTS), urgency, frequency and nocturia, overactive bladder (OAB), and detrusor overactivity (DO). It is natural and desirable that all suggested definitions are subject to criticism, and it is important that discussions for improvement of the existing terminology continue.

[IgG4-related disease-the diagnostic confusion and how to avoid it].

Since Hamano et al. have first reported serum IgG4 elevation in sclerosing pancreatitis in 2001, various systemic disorders have been report … s granulomatosis, lymphoma, cancer, and other existing conditions is necessary. To avoid diagnostic confusion, simpler and more scientific names should be used where disease-specific pathogenesis or markers have been ascertained.
