Improved osteoinduction of cortical bone allografts: a study of the effects of laser perforation and partial demineralization.

Massive cortical bone allografts have been found to incorporate slowly into host bone and thus are subject to complications such as nonunion, fatigue fracture, and infection. To better understand and improve the process of osteoinduction in these types of bone grafts, a new experimental model was developed with use of diaphyseal cortical bone grafts from rat tibiae that were prepared by partial demineralization and drilling of 0.33 mm diameter holes with a pulsed, 2.94 microns wavelength, erbium:yttrium-aluminum-garnet laser. Six types of grafts were analyzed: untreated (Type I), demineralized…

Evidence for preservation of cortical bone mineral density in patients on continuous ambulatory peritoneal dialysis.

It is still unclear whether dialysis modality, i.e., continuous ambulatory peritoneal dialysis (CAPD) versus hemodialysis (HD) specifically affects bone mineral density (BMD). To answer this question, 34 patients on HD and 25 on CAPD were matched for age, sex, height, and body weight with 125 normal subjects. BMD was measured using dual-energy X-ray absorptiometry (DXA; Hologic QDR 1000/W) at the lumbar spine (trabecular bone), the femoral neck (mixed cortical and trabecular bone), the distal tibial diaphysis (cortical bone), and the epiphysis (trabecular bone) in all subjects. No significant …

Study of the distribution of oral ciprofloxacin into the mucosa of the middle ear and the cortical bone of the mastoid process.

This multicentre study evaluates the distribution of ciprofloxacin into the tissue structures of the middle ear following multiple dosing of … ese concentrations were as follows: middle ear mucosa (n = 16): 5.54 +/- 3.46 micrograms/g (3-4 h): cortical bone of the mastoid process (n = 21): 1.07 +/- 1.29 micrograms/g (4 h). The measurements carried out 12 h after the last dose show that concentrations of ciprofloxacin in the middle ear mucosa were still at least as high as the minimum inhibitory concent …

Differences and similarities in cortical bone of the femur between donors with and without type 2 diabetes.

For a given BMD, adults with type 2 diabetes (T2D) have greater fracture risk than adults without the disease. To test the hypothesis that T2D lowers the fracture resistance of human cortical bone by negatively altering the bone matrix quality, we acquired cadaveric femurs from 120 female and male donors >50 yr old: 60 without diabetes (Ctrl) and 60 with T2D for ≥10 yr. We scanned a cross-section from each diaphysis using ex vivo micro-CT (μCT), followed by cyclic reference point indentation (cRPI: 0-10 N for 20 cycles) and impact micro-indentation on the medial surface. From the medial quadra…

Intraskeletal Variation in Cortical Bone Quantity in a Medieval Italian Sample: A Multivariate Exploratory Approach.

Bioarcheologists interpret skeletal health by examining variability within and between individuals. Studies of bone loss have generated contradictory and conflicting results regarding the onset and severity of age-related bone loss on a global and temporal scale, perhaps due to mismatched methodologies. Intraskeletal comparisons of bone tissue prove challenging precisely because of heterogeneous baselines in quantity and remodeling of cortical bone throughout the skeleton, as well as evolutionary histories and environmental impacts on growth and development. Here we analyze cortical bone indic…

A 2-Stage Approach to Alveolar Cleft Repair with Combined Cortical and Cancellous Bone Grafting.

BACKGROUND: Achieving optimal efficacy in secondary alveolar bone grafting (SABG) poses a considerable challenge for patients with alveolar … eal sealing during the initial stage, followed by particulate cancellous bone marrow and mandibular cortical bone grafting in the second stage after a 3-month interval. Six months after surgery, the SABG was evaluated quantitatively and qualitatively using computed tomography. RESULTS: A total of 81 patients (median age, 14 years; 49 men and 32 women) were inc …

Evaluation of trabecular bone and mandibular cortical thickness in adults with different vertical facial types.

OBJECTIVES: This study investigated differences in trabecular structure and mandibular cortical thickness in adults related to vertical facial type (VFT), sex, and their interactions. STUDY DESIGN: Lateral cephalometric radiographs (LCRs) and panoramic radiographs (PRs) of 256 patients were reviewed. The VFT classification into low-angle, normal, and high-angle groups was determined using angular and linear measurements on LCRs. Fractal dimension (FD) values and mandibular radiomorphometric indices (RMIs) were calculated on PRs. RESULTS: Two-way analysis of variance revealed significant diff

Influence of optical transmissivity on signal characteristics of photoacoustic guided waves in long cortical bone.

Long cortical bone allows axial transmission of ultrasonic guided waves, which has been utilized for osteoporosis evaluation. Benefiting structural and molecular sensitivity, photoacoustic has been used for tissue composition characterization. However, photoacoustic guided waves (PAGWs) in long cortical bone as well as the influence of optical transmissivity on PAGWs have not been thoroughly investigated. In the study, the influence of optical transmissivity on the signal characteristics of PAGWs was experimentally studied with a 1064 nm pulsed laser ultrasonic system and a tunable laser syste…

Reduced trabecular bone mineral density and thinner cortices in men with distal forearm fractures.

Men with distal forearm fractures have reduced bone density, an increased risk of osteoporosis and of further fractures. The aim of the study was to investigate the structural determinants of these observations using quantitative CT (qCT). Ninety six men with low-trauma distal forearm fracture and 101 age-matched healthy control subjects were recruited. All subjects underwent a quantitative CT on a standard 64-slice whole body CT scanner. These were analysed on Mindways QCT PRO™ Software to generate volumetric and geometric data at the lumbar spine, femoral neck and total hip, ultra-distal and

Usefulness of cortical thickness ratio of the third metacarpal bone for prediction of major osteoporotic fractures.

OBJECTIVE: Patients with rheumatoid arthritis (RA) are at high risk for osteoporotic fractures. We developed an index called the third metacarpal cortical thickness ratio (CTR), which reflects bone mineral density (BMD) in RA patients. A longitudinal study was conducted to verify the usefulness of CTR during the follow-up period. METHODS: Patients with RA who underwent dual energy X-ray absorptiometry (DXA) and hand X-ray simultaneously were monitored for disease activity and activities of daily living at 3-month intervals, and BMD and CTR were measured at 1-year intervals. Mean CTR during fo

Cortical bone adaptation to a moderate level of mechanical loading in male Sost deficient mice.

Loss-of-function mutations in the Sost gene lead to high bone mass phenotypes. Pharmacological inhibition of Sost/sclerostin provides a new drug strategy for treating osteoporosis. Questions remain as to how physical activity may affect bone mass under sclerostin inhibition and if that effect differs between males and females. We previously observed in female Sost knockout (KO) mice an enhanced cortical bone formation response to a moderate level of applied loading (900 με at the tibial midshaft). The purpose of the present study was to examine cortical bone adaptation to the same strain level…

Evaluation of myostatin as a possible regulator and marker of skeletal muscle-cortical bone interaction in adults.

INTRODUCTION: Bone mass was recently reported to be related to skeletal muscle mass in humans, and a decrease in cortical bone is a risk factor for osteoporosis. Because circulating myostatin is a factor that primarily controls muscle metabolism, this study examined the role of myostatin in bone mass-skeletal muscle mass interactions. METHODS: The subjects were 375 middle-aged community residents with no history of osteoporosis or sarcopenia who participated in a health check-up. Cortical bone thickness and cancellous bone density were measured by ultrasonic bone densitometry in a health chec…

Curettage through a wide cortical window for treatment of a primary aneurysmal bone cyst of the patella.

A 27-year-old man presented with intermittent right knee pain for 1 year with no previous trauma. Physical examination revealed only tenderness over the patella. Typical fluid-fluid levels were visible on magnetic resonance imaging (MRI), which highly suggested aneurysmal bone cyst (ABC) of the patella. After removal of a large window of thin cortical bone, curettage and bone grafting followed by cerclage wiring was performed. Histology confirmed the initial diagnosis of primary ABC of the patella. At the final follow-up visit at 71 months after surgery, the patient had normal joint activity w…

The Assessment of Skeletal Muscle and Cortical Bone by Second-generation HR-pQCT at the Tibial Midshaft.

BACKGROUND: Peripheral quantitative computed tomography (pQCT) is the current densitometric gold-standard for assessing skeletal muscle at the 66% proximal tibia site. High resolution peripheral quantitative computed tomography (HR-pQCT) is a leading technology for quantifying bone microarchitecture at the distal extremities, and with the second-generation HR-pQCT it is possible to measure proximal limb sites. Therefore, the objectives of this study were to: (1) assess the feasibility of using HR-pQCT to assess skeletal muscle parameters at the 66% proximal tibia site, and (2) test HR-pQCT ske

Meaning and Limitation of Cortical Bone Width Measurement with DentaScan in Medication-Related Osteonecrosis of the Jaws.

Mandibular cortical bone measurement with x-ray imaging is known to be a potentially useful tool in the detection of dimensional changes caused by bisphosphonate. The primary purpose of this study was to assess the meaning and limitation of cortical bone measurement with computed tomography (CT) in patients with medication-related osteonecrosis of the jaw (MRONJ). The investigators obtained DentaScan images of the mandible from 15 patients with MRONJ, 15 patients with a history of antiresorptive agent administration without symptoms of MRONJ (non-MRONJ), and 15 control subjects. The cortical b…

Glucocorticoid effects on changes in bone mineral density and cortical structure in childhood nephrotic syndrome.

The impact of glucocorticoids (GC) on skeletal development has not been established. The objective of this study was to examine changes in volumetric bone mineral density (vBMD) and cortical structure over 1 year in childhood nephrotic syndrome (NS) and to identify associations with concurrent GC exposure and growth. Fifty-six NS participants, aged 5 to 21 years, were enrolled a median of 4.3 (0.5 to 8.1) years after diagnosis. Tibia peripheral quantitative computed tomography (pQCT) scans were obtained at enrollment and 6 and 12 months later. Sex, race, and age-specific Z-scores were generate

Changes in cortical bone response to high-fat diet from adolescence to adulthood in mice.

UNLABELLED: Diabetic obesity is associated with increased fracture risk in adults and adolescents. We find in both adolescent and adult mice dramatically inferior mechanical properties and structural quality of cortical bone, in agreement with the human fracture data, although some aspects of the response to obesity appear to differ by age. INTRODUCTION: The association of obesity with bone is complex and varies with age. Diabetic obese adolescents and adult humans have increased fracture risk. Prior studies have shown reduced mechanical properties as a result of high-fat diet (HFD) but do no…

Time-lapsed assessment of microcrack initiation and propagation in murine cortical bone at submicrometer resolution.

The strength of bone tissue is not only determined by its mass, but also by other properties usually referred to as bone quality, such as mi … f SR CT for 3D visualization and quantification of the different ultrastructural phases of the intracortical bone porosity. We particularly postulate the necessity of 3D imaging techniques to unravel microcrack initiation and propagation and their effects on bone mechanics. Result of maxillomandibular fixation using intraoral cortical bone screws for condylar fractures of the mandible.

PURPOSE: Treatment of mandibular condylar fractures is not standardized. The maxillomandibular cortical bone screw fixation technique carries many advantages. The aim of this work was to evaluate this technique for routine method. MATERIALS AND METHODS: Fifty patients treated by maxillomandibular fixation (MMF) by use of cortical bone screws from 2004 to 2006 were retrospectively analyzed. In our maxillofacial surgery unit in Bordeaux, France, our indication is to treat extra-articulated fractures without severe displacement by MMF. RESULTS: The mean time required for MMF was 13 minutes, and…
