[Cytomegalovirus infection and inner ear deafness].

Bilateral sensorineural hearing loss of patients is correlated with elevated antibody titres against cytomegaly virus in serum in children as well as in adults. Unilateral deafness correlates with mumps infection. This is shown in a German population as well as in a Mediterranean group of patients. There is no evidence between antibody titer of Coxsackie virus, adenovirus and herpes virus or toxoplasma gondii interaction and sensorineural hearing loss neither unilaterally nor bilaterally.

Congenital middle-ear deafness: CT study.

Computed tomography (CT) was used to study 25 patients with congenital conductive hearing loss and normal external auditory canals. Deformities were subdivided according to ossicular, fenestral, and cholesteatomatous origin. Isolated ossicular deformities were found in 14 patients (five bilateral), cholesteatoma in eight, oval-window nondevelopment (with ossicular deformity) in one, and normal studies in two (congenital stapes fixation at the level of the annular ligament). Ossicular deformities may be subdivided into incudostapedial disconnections into incudostapedial disconnections (most com

Word deafness: one hundred years later.

Since its original description the diagnosis of word deafness has been greatly expanded. Confusion has arisen with regard to the usage of the related terms pure word deafness, auditory agnosia, and cortical deafness. Three new cases of word deafness are presented including one case with CT and necropsy correlation. These cases are compared with 34 previously reported cases of various cortical auditory disorders. Our review establishes that patients with word deafness who have had formal testing of linguistic and non-linguistic sound comprehension and musical abilities always demonstrated a mor…

Immune complex-associated deafness: preliminary communication.

During the last year patients presenting with progressive bilateral or sudden sensorineural deafness of unknown aetiology have been investigated for possible abnormal immune activity. Twenty-six cases are reported in which significantly raised circulating immune complexes were present, together with 2 cases of clinical autoimmune deafness where the complexes were normal. Thirty-six control subjects were also studied. Following a review of the clinical features, the pathogenesis of this new association with sensorineural deafness is discussed. In some cases the aetiology is probably autoimmune,…

Acoustic neuromas presenting as sudden deafness.

Caution is required in diagnosing even apparently "obvious" causes of hearing loss. A patient is described who came to medical attention through referral from a lawyer for sudden deafness allegedly caused by exposure to noise in an industrial setting. Thorough evaluation revealed an acoustic neuroma. Review of the literature suggests that presentation of acoustic neuromas as sudden deafness is more common than generally recognized. Clinicians must be alert to this possibility and diligent in their evaluation of any unilateral ear symptoms.

Histopathology of neurosensory deafness in sarcoidosis.

Sarcoidosis is an idiopathic systemic granulomatous disease which occasionally causes fluctuating auditory and vestibular dysfunction. The temporal bones from a 32-year-old man deaf for 5 years from CNS sarcoidosis were examined histologically and compared with other nervous system tissues. It was found that the acoustic, vestibular, and facial nerves were involved in a striking perivascular lymphocytic infiltration resulting in myelin and axonal degeneration. The cochlear and labyrinthine neuroepithelium and stria vascularis had degenerated. It is hypothesized that neurosensory deafness and v…

[Course and prognosis in sudden deafness].

Follow up audiograms were done at periods up to 15 years after sudden deafness on 69 patients. The average value of the pure tone threshold continued to decrease linearally at about 1.15 dB per year. In a few cases (21%) however, improvement in hearing was observed even up to 12 years after sudden deafness. 28.6% of the patients recovered later without having shown any improvement in hearing during treatment with infusions. On the other hand 8.3% of the patients had a relapse. The immediate improvement in hearing following in patient treatment showed no correlation to the subsequent audiogram.…

[Rubella deafness. Audiometric, ophthalmologic, epidemiological aspects].

Case-reports of 75 children, with deafness due to an embropathy from maternal rubella during the first months of pregnancy, are reviewed. Emphasis is placed on the frequency of interauricular functional asymmetry, of pigmented retinitis alone or associated with a cataract, and of dysmaturity at birth and persistence of overall delayed development. Also stressed are the large proportion of women born outside the French Metropolitan area who contracted rubella during pregnancy, and the very high frequency of errors in interpreting clinical and biological signs in suspected rubella, leading to an…

Sensorineural deafness in early acquired syphilis.

A 36 year old male developed bilateral sensorineural deafness as the chief manifestation of secondary syphilis. Cerebrospinal fluid showed pleocytosis. Treatment with penicillin and prednisone resulted in good recovery of hearing. Initial recovery seemed dependent on corticosteroids. Deafness can complicate acquired syphilis in both early and late stages of the disease and may be its sole manifestation. Early acquired syphilitic deafness is usually the result of a meningitis affecting the eighth nerve and responds well to treatment. These features are contrasted with those of late acquired syp…

Mondini's deafness. A review of histopathology.

From a histopathological survey of 12 ears (7 cases) of Mondini's deformity and deafness, the following conclusions or interpretations are drawn: 1) Mondini's bony deformity diagnosable by polytomography does not necessarily indicate Mondini's deafness or x-ray diagnosis of Mondini's deafness cannot be specifically relied upon. 2) The pars superior (semicircular canals and utricle) may be absent (aplastic) or the canals may be enlarged leading to interesting questions regarding vestibular physiology in such patients. 3) Enlarged endolymphatic duct and deficient utriculoendolymphatic valve are …

Congenital choanal atresia and nerve deafness.

Four cases of bilateral congenital choanal atresia are presented. Three of them (75 per cent) were shown by Electric Response Audiometry (ERA) tests to have, in addition, congenital nerve deafness; and two had other congenital anomalies. Thus, congenital nerve deafness should be considered as a possible congenital anomaly associated with choanal atresia. Early diagnosis and treatment of congenital choanal atresia can prevent death from asphyxia during the neonatal period, but after surgical correction of the atresia the possible existence of additional anomalies has to be determined and treate…

Electrocochleography of ears with mumps deafness.

Electrocochleography was performed on 16 affected ears of 15 deaf patients whose deafness had resulted from the mumps. Although pure-tone audiometry showed no response in each case, the cases of mumps deafness can be classified into the following three types of cochlear impairment according to the cochlear microphonic (CM) response: (1) no action potential (AP) response but a well-developed CM response, with impairment at the neural level and probable functioning of Corti's organ; (2) absence of both the AP CM responses, with severe impairment of both the neural regions and Corti's organ; (3) …

Follow up study of sudden deafness.

It is significant to follow up patients with sudden deafness, because the change of hearing level after a long-term period and a differential diagnosis from Meniére's disease must be studied. We reported the follow-up study of 27 patients with sudden deafness who were admitted to the Department of Otolaryngology of the Second Tokyo National Hospital. The duration of follow-up ranged from 6-58 months, with a mean duration of 23.7 months. In the Complete Recovery Group there was little change of mean hearing loss during the follow-up period. We coud demonstrate that the once recovered hearing le…

Scleritis and perceptive deafness: case report.

There is some evidence in the past decade that a similar syndrome to Cogan's syndrome may exist in which a scleritis substitutes for the nonsyphilitic interstitial keratitis. A case is reported of a female patient, aged 39, with nonrheumatoid scleritis and associated bilateral perceptive deafness.

Sickle cell anemia and sudden deafness.

Little is known about the relationship between sickle cell anemia and hearing loss. The case reported here illustrates the potential hazard to hearing during a sickle cell crisis. The audiometric findings are suggestive of possible cochlear and neural involvement.

Familial nephrosis, nerve deafness, and hypoparathyroidism.

Two male siblings with nephrotic syndrome, nerve deafness, and hypoparathyroidism are described. Each child, one at five years of age and the other at eight years, died in renal failure. At autopsy the parathyroid glands were absent in one child and hypoplastic in the other one. Two twin male siblings presented with similar findings and died at the age of three years. At autopsy their parathyroid glands were fibrotic, and glomerular basement membranes were thickened. This may be the first recorded association of familial nephrosis, nerve deafness, and hypoparathyroidism. The mode of transmissi…
