The objective of this study was to document the pattern of bowel gas decompression and mediastinal shift in infants with congenital diaphragmatic hernia (CDH) before delayed surgical correction. The setting was the Neonatal Intensive Care Unit, Royal Alexandra Hospital, Edmonton. The design was a retrospective review of radiographs and charts of all infants with CDH between January 1990 and December 1993. Bowel gas in the upper and middle one third of the hemithorax was classified as grade III and grade II, respectively. Bowel gas in the lower one third of the hemithorax or absent bowel gas wa

Fourteen patients with severe chronic venous ulceration and constricted lower limbs underwent an operation to release the constriction and to ligate the perforating veins in the calf. At 4-year follow-up 6 patients remained healed. Two further patients were healed when last seen 3 years and 19 months after surgery. The operation is suggested as a salvage procedure for selected patients, when all other methods of treatment have failed.

BACKGROUND: Neurovascular compression (NVC) is a primary etiology of trigeminal neuralgia (TN) and hemifacial spasm (HFS). Despite Magnetic Resonance Tomographic Angiography (MRTA) being a useful tool for 3D multimodal fusion imaging (MFI) in microvascular decompression (MVD) surgery planning, it may not visualize smaller arterial vessels and veins effectively. We validate a novel computed tomography angiography and venography (CTA/V) - diffusion tensor tractography (DTT) -3D-MFI to enhance the MVD surgical guidance.
METHODS: In this prospective study, 80 patients with unilateral primary TN o

PURPOSE: Hydrocephalus after craniovertebral decompression (CVD) for Chiari I malformation (CM-1) is a well-recognised complication. The mainstay of management involves medical management with high-dose steroids and/or acetazolamide, repeated lumbar punctures, external ventricular drainage (EVD) or insertion of a ventriculoperitoneal shunt (VPS). Endoscopic third ventriculostomy (ETV) has only been used a handful of times to treat this type of hydrocephalus with controversial outcomes. Not much is known about this or the technical nuances of this procedure. We report our experience.

OBJECTIVE: The authors sought to determine how the temporal proximity of lumbar epidural spinal injection prior to surgery impacts clinical outcomes (e.g., 30-day readmission, postoperative complications, CSF leak) in patients undergoing lumbar decompression without fusion.
METHODS: The authors queried their institutional registry to identify patients who underwent elective lumbar decompression for spondylotic pathology between January 2019 and March 2022 at multiple centers within the same hospital network. Patients were divided into groups based on the time between their surgical date and t

PURPOSE: No clear consensus exists on which anterior surgical technique is most cost-effective for treating cervical degenerative disk disease (CDDD). One of the most common treatment options is anterior cervical discectomy with fusion (ACDF). Anterior cervical discectomy with arthroplasty (ACDA) was developed in an effort to reduce the incidence of clinical adjacent segment pathology and associated additional surgeries by preserving motion. This systematic review aims to evaluate the evidence regarding the cost-effectiveness of anterior surgical decompression techniques used to treat radiculo

BACKGROUND: Accurate diagnosis of carpal tunnel syndrome is key to successful treatment. At our center a treatment pathway was recently established for patients with carpal tunnel syndrome. Within this pathway patients are assessed by their community practitioner. Patients who fail to improve with conservative treatment and have a positive nerve conduction study are put forward for surgery without a specialist assessment. Recent literature has shown nerve conduction study may produce a high rate of false-positive results when used to diagnose carpal tunnel syndrome. The aim of this study was t

OBJECTIVES: This study aimed to investigate the feasibility of segmentation-independent volume rendering (SI-VR) in visualising the root entry zone (REZ), and to explore the influence on the management of vascular compression syndromes (VCSs).
METHODS: Two hundred and twenty patients with VCSs were recruited in this prospective study from July 2015 to May 2019. SI-VR was reconstructed based on inverted 3D fast spin echo T2WI. They were assigned to the experimental group and control group randomly. Patients in the experimental group would accept extra evaluation based on SI-VR before microvasc

BACKGROUND: Most studies recommend urgent decompression (e.g., within 48-72 h) of the symptomatic onset of a cauda equina syndrome. As patients in our area typically underwent >3 months delayed surgery for cauda equina syndromes due to disc disease/stenosis, we asked whether surgery was still worthwhile.
METHODS: This was a retrospective analysis of 12 patients (2012-2018) who underwent delayed surgical decompression for cauda equina syndromes secondary to lumbar disc herniations and/or degenerative lumbar canal stenosis.
RESULTS: After a mean postoperative duration of 8.22 months, nine pati

BACKGROUND: Lumbar spinal stenosis (LSS) is frequently associated with postural instability. Although several studies evaluated patients' functional impairments, underlying sensorimotor mechanisms are still poorly understood. We aimed to assess the specific set of postural control deficits associated with LSS during spontaneous and externally perturbed stance and evaluated post-surgical changes in postural behavior.
METHODS: We analyzed postural control in eleven LSS patients (age 69 ± 8 years) pre- and post-laminectomy, correlated experimental data with functional tests and patient-reported

Treatment for craniocervical junction lesions associated with osteogenesis imperfecta (OI) has been described, but there are divergent views on operative procedures and preoperative and postoperative therapies due to the small number of cases. It has been suggested that a major procedure such as combined anterior and posterior surgery with concomitant ventriculoperitoneal (VP) shunting is required for OI associated with basilar impression (BI). However, here we report a case with a good outcome after posterior decompression fusion only. The patient was a 29-year-old woman with OI (Sillence typ

OBJECT: If, when, and how children with arachnoid cysts should undergo surgery has been a matter of debate. In the present study the authors describe long-term clinical and neuroimaging results in children with intracranial arachnoid cysts, treated in accordance with the authors' policy of performing surgery in most of these cases.
METHODS: The study included 48 pediatric patients (age < 16 years of age) who underwent surgery for treatment of an arachnoid cyst at Haukeland University Hospital between January 1987 and September 2004. Forty-one patients underwent cyst fenestration to the basal

OBJECTIVE: We have used three different approaches, namely, the infratentorial lateral supracerebellar approach, the lateral suboccipital infrafloccular approach, and the transcondylar fossa approach, for microvascular decompression for treatment of trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia, respectively. Each approach is a variation of the lateral suboccipital approach to the cerebellopontine angle (CPA); however, each has a different site of bony opening, a different surgical direction, and a different route along the cerebellar surface.
METHODS: The infratentor
