Causality between rheumatic diseases and iron-deficiency anemia: A 2-sample, 2-step mediation Mendelian randomization investigation.

Growing evidence suggests that patients with rheumatic disease are associated with iron-deficiency anemia (IDA). However, the causal relationship between rheumatic disease and IDA risk remains unclear. To investigate this, we conducted a Mendelian randomization (MR) study using genetic variants from the large genome-wide association studies. In this study, we primarily investigated common rheumatic diseases, which include rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), Sjogren syndrome (SS), systemic sclerosis (SSc) as exposures, with IDA as the outcome and iron status as the me

Calcifediol in the management of vitamin D deficiency-related skeletal and extraskeletal diseases: overview and clinical cases.

As well as being essential for musculoskeletal health, vitamin D is involved in numerous other physiological processes. Poor vitamin D status is linked to a wide range of diseases, including cardiovascular disease, autoimmune conditions, pulmonary disorders and upper respiratory tract infections. While optimal target concentrations of serum 25-hydroxyvitamin D (25(OH)D) for health maintenance or therapeutic purposes are still the subject of debate, there is reasonable agreement that serum 25(OH)D levels <50 nmol/L (20 ng/mL) constitute vitamin D deficiency and that severe deficiency states (se

IL-12Rβ1 deficiency corresponding to concurrency of two diseases, mendelian susceptibility to mycobacterial disease and Crohn's disease.

BACKGROUND: The interleukin-12 receptor β1 (IL-12Rβ1) deficiency is a primary immunodeficiency (PID), affecting the immunological pathway of interleukin 12/interferon- γ (IL12/IFN-γ) axis and interleukin 23 receptor (IL23R). Defect in this pathway is mainly affecting the cellular immunity-related disorders. IL-12Rβ1 is a receptor chain of both the IL-12 and the IL-23 receptors and thus, deficiency of IL-12Rβ1 abolishes both IL-12 and IL-23 signaling. MATERIAL AND METHODS: In this study, we performed whole exon sequencing and confirmatory Sanger sequencing in IL-12Rβ1. Evaluation of the IL12/I

PARK7/DJ-1 dysregulation by oxidative stress leads to magnesium deficiency: implications in degenerative and chronic diseases.

Disturbed magnesium (Mg(2+)) homoeostasis and increased levels of OS (oxidative stress) are associated with poor clinical outcomes in patients suffering from neurodegenerative, cardiovascular and metabolic diseases. Data from clinical and animal studies suggest that MD (Mg(2+) deficiency) is correlated with increased production of ROS (reactive oxygen species) in cells, but a straightforward causal relationship (including molecular mechanisms) between the two conditions is lacking. The multifactorial protein PARK7/DJ-1 is a major antioxidant protein, playing a key role in cellular redox homoeo

Arterial Calcification Due to Deficiency of CD73 (ACDC) As One of Rheumatic Diseases Associated With Periarticular Calcification.

In 2011, St Hilaire et al (N Engl J Med. 2011;364:432-442) identified mutations in the ecto-5'-nucleotidase (NT5E) gene, which encodes CD73, in members of 3 families with symptomatic arterial and joint calcifications. The deficiency of CD73 involves the extracellular adenosine metabolism that influences inorganic pyrophosphate and phosphate metabolism and leads to tissue calcification. Herein, we report an additional case with arterial calcification due to deficiency of CD73. Genetic analyses revealed that the patient was a compound heterozygote of mutations in the NT5E gene. The present case

Vitamin deficiencies and Alzheimer's disease: evidence and implications for supplementation.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by beta-amyloid (Aβ) deposition, hyperphosphorylation of tau protein (pTau), mitochondrial impairment and neuroinflammation. Several risk factors, such as aging, genetics, cardiovascular diseases (CVD) and lifestyle, concur to the onset of the disease. Among modifiable risk factors, micronutrient intake has gained attention for its potential role in preventing or slowing down disease progression. In this narrative review, we summarize current evidence linking vitamin deficiencies to the onset and progression of

Biallelic TXNIP deficiency is associated with a multisystemic metabolic disease.

OBJECTIVE: Thioredoxin-interacting protein (TXNIP) is a protein involved in redox metabolism, but also a key regulator of glucose and lipid metabolism in preclinical models. To date, four patients with biallelic loss-of-function variants in TXNIP have been described, presenting with lactic acidosis and variable hypoglycemia, hepatomegaly, developmental delay and seizures. However, the role of TXNIP in human metabolism and its mechanistic effects across different organs are not fully understood. METHODS: We clinically, biochemically and genetically characterized a cohort of six additional indi

Prime Editing for p47phox-Deficient Chronic Granulomatous Disease.

Chronic granulomatous disease (CGD) is a severe monogenic immunodeficiency caused by damaging variants in genes required for microbicidal NADPH oxidase activity. Autosomal recessive p47phox-deficient CGD (p47-CGD) is predominantly caused by a two-nucleotide deletion in exon 2 (delGT) of NCF1. We developed PM359, an autologous CD34+ hematopoietic stem-cell therapy in which prime editing is used to correct delGT. Two participants received PM359 after myeloid conditioning with busulfan: neutrophils and platelets engrafted promptly in both patients. Adverse events were consistent with myeloid cond

Molybdenum Cofactor Deficiency Type A disease in Northern Israel.

BACKGROUND: Molybdenum cofactor deficiency (MoCD) is a group of three autosomal recessive disorders caused by deficiency of the de novo metabolic synthesis of molybdenum cofactor. Most patients present within the first weeks of life with intractable seizures and progressive encephalopathy. Type A is the most common form caused by pathogenic variants in MOCS1 gene that result in deficiency of the first enzyme, cyclic pyranopterin monophosphate synthase. OBJECTIVES: To characterize MoCD type A clinical features, disease course, neuroradiology, and genetic features in Northern Israel. METHODS:

GPD1 deficiency-a rare, overlooked cause of liver disease.

Transient infantile hypertriglyceridemia is one of the diseases that should be considered in case of unexplained elevated liver enzymes, hypertriglyceridemia and hepatosteatosis. We report 2 siblings with novel homozygous variants in the GPD1 gene with transient infantile hypertriglyceridemia. Two siblings born from consanguineous marriage were referred due to hepatomegaly, elevated transaminases and fatty liver. After excluding other possible causes of fatty liver and elevated transaminase levels; whole-exome sequencing (WES) was performed on genomic DNA isolated from the peripheral blood sam

Vitamin D insufficiency and deficiency: in search of a bone disease.

The definition of deficiency or insufficiency of vitamin D remains challenging because currently used thresholds of <30 nmol/L and between 30 and either 50 or 75 nmol/L are not consistently associated with clinical or biochemical abnormalities suggestive of secondary hyperparathyroidism, compromised bone volume, microarchitecture, or osteomalacia. Indeed, there is little convincing clinical, biochemical, bone densitometric, microarchitectural, or histomorphometric evidence that 25-hydroxyvitamin D [25(OH)D] levels between 30 and 75 nmol/L in community dwellers identify a vitamin D 'insuffic

Invasive Meningococcal Disease in a Patient With Complement 7 Deficiency.

Neisseria meningitidis can cause invasive meningococcal disease (IMD). Individuals with primary complement deficiencies are at a higher risk of developing IMD. However, cases of IMD associated with complement deficiency have rarely been reported in Japan. In this case, a 35-year-old Japanese man presented with fever and a spreading skin rash. Blood culture identified N. meningitidis serogroup B. The patient was treated with antibiotics and fully recovered. Genetic analysis revealed a deficiency in complement component 7. This case highlights that IMD can occur in young, otherwise healthy indiv

Vitamin D Deficiency, Chronic Kidney Disease and Periodontitis.

Vitamin D has important anti-inflammatory, anti-microbial properties and plays a central role in the host immune response. Due to the crucial role of the kidneys in the metabolism of vitamin D, patients with chronic kidney disease (CKD) are prone to vitamin D deficiency. The resultant reduction in the production of calcitriol, the activated form of vitamin D, in patients with CKD is responsible for exacerbating the existing renal impairment and periodontal inflammation. Recent evidence suggests a bidirectional, causal relationship between periodontitis and renal functional status. Both conditi

Micronutrient Deficiency and Muscular Status in Inflammatory Bowel Disease.

Micronutrient deficiencies are common in inflammatory bowel disease (IBD). The aim of this study was to evaluate micronutrient deficiencies and identify muscular status of patients with IBD. From June 2019 to October 2021, a total of 105 patients with IBD were enrolled prospectively. To obtain objective data, micronutrients were measured in the patients' serum, and body composition analysis was performed using bioelectrical impedance analysis. There were 51 patients with ulcerative colitis (UC) and 54 with Crohn's disease (CD), while the gender ratio (M: F) was 54:51. The average age was 37 ±

Testosterone Deficiency in Sickle Cell Disease: Recognition and Remediation.

Hypogonadism is common in men with sickle cell disease (SCD) with prevalence rates as high as 25%. Testicular failure (primary hypogonadism) is established as the principal cause for this hormonal abnormality, although secondary hypogonadism and compensated hypogonadism have also been observed. The underlying mechanism for primary hypogonadism was elucidated in a mouse model of SCD, and involves increased NADPH oxidase-derived oxidative stress in the testis, which reduces protein expression of a steroidogenic acute regulatory protein and cholesterol transport to the mitochondria in Leydig cell
