Malignant hyperthermia and its implications in general dentistry.

Malignant hyperthermia (MH) is an uncommon and potentially life-threatening pharmacogenetic disorder. This abnormality in muscle metabolism can be triggered by a variety of agents (particularly general anesthetics and stress), resulting in a rapid heart rate increase, muscle rigidity, acidosis, temperature elevation, rhabdomyolysis, and renal failure. Immediate discontinuing of triggering agents, oxygenation, cooling, and dantrolene are necessary to treat an episode. MH-susceptible patients often indicate a positive family history of experiencing an adverse event during anesthesia. Few diagnos

An interdisciplinary approach to treatment planning in implant dentistry.

The practice of implant dentistry requires an interdisciplinary approach that integrates the knowledge, skills and experience of all the disciplines of dentistry into a comprehensive treatment plan. This article outlines a comprehensive interdisciplinary treatment philosophy designed for developing the foundation of optimal aesthetics in implant dentistry. Cases are presented to illustrate the utility of interdisciplinary treatment in which specialists are recruited to enhance and improve a patient's dental function and aesthetics.

Evidence-based dentistry: a model for clinical practice.

Making decisions in dentistry should be based on the best evidence available. The objective of this study was to demonstrate a practical procedure and model that clinicians can use to apply the results of well-conducted studies to patient care by critically appraising the evidence with checklists and letter grade scales. To demonstrate application of this model for critically appraising the quality of research evidence, a hypothetical case involving an adult male with chronic periodontitis is used as an example. To determine the best clinical approach for this patient, a four-step, evidence-ba…

Intravenous conscious sedation in children for outpatient dentistry.

The use of general anaesthesia for dental treatment in the NHS outside hospitals has changed over time. Although deaths are uncommon during or immediately after general anaesthesia for dental treatment, they are more likely to occur than with other methods of pain and anxiety reduction, such as local anaesthesia and conscious sedation. Inquiries into recent anaesthetic deaths in dental practice have been critical of the standard of care provided in areas such as pre-operative assessment, monitoring, resuscitation and transfer to specialist critical care facilities.

Occupational health problems in modern dentistry: a review.

Despite numerous technical advances in recent years, many occupational health problems still persist in modern dentistry. These include percutaneous exposure incidents (PEI); exposure to infectious diseases (including bioaerosols), radiation, dental materials, and noise; musculoskeletal disorders; dermatitis and respiratory disorders; eye injuries; and psychological problems. PEI remain a particular concern, as there is an almost constant risk of exposure to serious infectious agents. Strategies to minimise PEI and their consequences should continue to be employed, including sound infection co…

[Development and clinical application of ultrasonic osteotomy in dentistry].

PURPOSE: Gestational diabetes mellitus (GDM) is a metabolic disorder of pregnant women that develops during their second or third trimester. As an important organ in carbohydrate and lipid metabolism, the liver may play a role in GDM pathophysiology. This meta-analysis examined the conflicting relationship between liver enzyme levels and GDM risk while emphasizing the probable use of liver enzymes as a biomarker for early detection and prevention of GDM. METHODS: Literature from PubMed, EBSCO, Google Scholar, and ScienceDirect was systematically reviewed through August 2025. A total of seven

One of the greatest figures in the development of the dental profession to the high status it enjoys today is all but forgotten. Jonathan Ta … day, he set the standards for modern dental periodical literature that have done so much to elevate dentistry that today it stands on a par with medicine as a truly science-based profession. He served dentistry in many capacities: president of the American Dental Association, founder of the National Association of Dental Faculties, and founder of the Nation …

[Lasers in dentistry. 5. The use of lasers in periodontology].

This literature review shows that the Nd:YAG laser and the Er:YAG laser can be beneficially used in periodontology for subgingival curettage and gingivectomy. Studies demonstrated that subgingival laser treatment can result in significant reduction of the initial levels of periodontal pathogens. It is beneficial to perform laser curettage under water irrigation prior to scaling in order to destroy of the structure of the calculus and its attachment to the root surface. During laser curettage sufficient haemostasis is obtained, which improves detection of subgingival calculus. Nd:YAG laser ging

Adverse reactions associated with the use of eugenol in dentistry.

Eugenol is a material commonly used in dentistry with few reported side effects. It is not however, a bio-friendly material when in contact with oral soft tissues. It can produce both local irritative and cytotoxic effects, as well as hypersensitivity reactions. Here we report on two cases of adverse local reaction to eugenol, contained within a temporary restorative material and a temporary cementation material respectively, which illustrate these problems.

Allergy to local anaesthetics in dentistry. Myth or reality?

BACKGROUND: Local anesthetics (LA) are frequently used in dentistry. Although these drugs are usually well-tolerated, they can sometimes provoke adverse reactions of various types and severity. The true incidence of LA allergic reactions is unknown. The objectives of this study were (i) to evaluate the incidence of immediate adverse events in subjects requiring local anesthetic injection in order to receive dental treatment; (ii) to assess the incidence of anaphylactic allergic reactions among those recorded as adverse events and (iii) to analyze the relationship between the atopic antecedents…

The impact on dentistry of recent advances in the management of hypertension.

Dentistry has played an important role in the detection of patients with hypertension. Patients found to have high blood pressure at or beyond defined levels should be referred for a medical diagnosis and indicated treatment. Once the hypertensive condition is under control, oral and dental evaluation and treatment can be initiated. Beginning in 1976, the percentage of the general population in the United States with undetected hypertension declined steadily. However, this decline reversed, beginning in 1994. In addition, fewer than 50% of the patients who are aware of their hypertension have …

Drug-related methods for alleviation of stress in dentistry.

For many patients, dental procedures can cause a great deal of anxiety and fear. These emotions are part of a stress reaction, which, in physiologic terms, can pose a serious risk, particularly to patients with pre-existing cardiopulmonary disorders. In each case the dentist must decide whether anxiolytic stress-reducing treatment is indicated, and which method should be used. Besides certain psychotherapeutic techniques, numerous drugs (neuroleptics, barbiturates, beta-blockers, and benzodiazepines) are available for this purpose. This paper reviews these types of drugs and offers recommendat

Geriatric dentistry and prevention: research and public policy.

Changing demographics, including the increase in life expectancy and the growing numbers of elderly, has focused attention on the need for dental research activities to be expanded for geriatric dentistry. The elderly are at greater risk for oral disease, since gains in longevity result in more medically compromising conditions or systemic disease with oral manifestations. Also, as edentulism decreases and as more teeth are retained by the elderly, the pattern of oral diseases and the treatment of dental conditions will be altered. Barriers to self-care and professional care must be removed, a…

Glass inserts. A new dimension in restorative dentistry.

Pediatric restorative dentistry has traditionally involved the placement of a stainless steel crown following a pulpotomy in primary molars. The increased exposure to systemic and topical fluorides has significantly decreased smooth surface caries, yet occlusal pit and fissure caries remains a problem. Occlusal caries involving the pulp of primary molars, leaving the supporting walls intact, may be restored conservatively with the use of glass inserts. The glass inserts provide the major volume of the restoration, therefore a minimal amount of posterior composite resin is necessary. The glass …
