Finite element analysis of a novel implant distribution to support maxillary overdentures.

PURPOSE: To evaluate the biomechanics of a novel implant placement distribution and compare it with that of conventional maxillary overdenture support using three-dimensional finite element analysis (FEA). The application of zirconia implants in the context of this novel design was also evaluated. MATERIALS AND METHODS: Detailed FEA models were created to analyze the loading responses of two different distributions of implants to support maxillary overdentures. The two implant distributions were as follows: the conventional design (D1) included four unsplinted implants in the premolar regions

Reconstruction of severely resorbed edentulous maxillae using osseointegrated fixtures in immediate autogenous bone grafts.

A surgical technique for rehabilitation of severely resorbed edentulous maxillae using fixed prostheses or overdentures supported by osseointegrated fixtures in immediate autogenous corticocancellous bone grafts from the ilium is described. The results of the first 23 consecutively treated patients are reviewed. The mean observation time was 4.2 years (range 1 to 10 years). A total of 124 fixtures was originally placed into the grafts, supplemented with 16 fixtures inserted later into seven of the jaws. Throughout their observation period, 17 of the patients had continuously stable prostheses.

Oral implants in dependent elderly persons: blessing or burden?

BACKGROUND: Implant-supported (partial) dentures may raise problems in patients who have become dependent on others for daily oral health care. Dental hygienists and general dental practitioners, as well as care providers, volunteer aiders and even health care insurance companies, should anticipate the growing demand for specific oral health care for patients provided with implant-supported (partial) dentures. OBJECTIVE: To report three cases of dependent patients and to present recommendations to prevent or resolve implant-related oral problems. MATERIALS AND METHODS: The three case reports

Two-year follow-up of osseointegration and rehabilitation in a patient with oral and maxillofacial arteriovenous malformations.

The case of a patient with high-flow arteriovenous malformations (AVMs) affecting the mandible and lower face is presented. The patient had undergone surgical resection, sclerotherapy, and transcatheter embolization over the course of 16 years. As a result, all teeth except for the lower left third molar were missing, and a mass of arteriovenous malformed soft tissue covered the residual alveolar ridge. Implant placement surgery was performed in his mandible after treatment of the AVMs. Osseointegration of the implants placed in the mandible affected by the AVMs was evaluated. Rehabilitation i

Dental rehabilitation using an implant-carrying plate system in a severely resorbed edentulous maxilla: a case report.

This clinical article describes a case of dental rehabilitation using an implant-carrying plate system (EPITEC) for a patient with severely resorbed edentulous maxilla and microstomia. In this case, the presence of microstomia prevented bone augmentation procedures through an intraoral approach. Treatment using 2 endosseous implants inserted in the canine regions and an implant-supported overdenture was planned. However, endosseous implants were not feasible on the right side because of insufficient available bone volume. An implant-carrying plate system was then utilized on the right side. Fo

The reverse zygomatic implant: a new implant for maxillofacial reconstruction.

This case report describes the rehabilitation of a patient who had been treated with a hemimaxillectomy, reconstruction with a latissimus dorsi vascularized free flap, and radiotherapy for carcinoma of the sinus some years previously. Limited jaw opening, difficult access through the flap to the bony site, and the very small amount of bone available in which to anchor the implant inspired the development and use of a new "reverse zygomatic" implant. For this treatment, site preparation and implant insertion were accomplished using an extraoral approach. The implant was used along with two othe

Interim rehabilitation of an edentulous situation with an overdenture retained by short implants during a staged vertical and horizontal ridge augmentation: a dental technique.

Interim rehabilitation of edentulous patients is essential in implant prosthodontics. However, surgical augmentation may be required for patients with reduced bone density, which complicates the use of an interim prosthesis. This report describes the rehabilitation of an edentulous patient with an interim maxillary overdenture retained by short implants with individual attachments during the healing period of vertical and horizontal ridge augmentation. This technique increased the stability of the interim prosthesis even with shortened flanges to eliminate any pressure on the surgical site. Th

Implant-supported overdenture rehabilitation and progressive systemic sclerosis.

Progressive systemic sclerosis, or systemic scleroderma, is an autoimmune multisystemic disease associated with vascular abnormalities, connective tissue sclerosis, atrophy, and autoimmune changes. The complex oral problems that may develop in patients with the disease and the difficult oral access necessitate extreme care in preventing dental disease. The oral management of a patient with an edentulous mandible and progressive systemic sclerosis, using an overdenture supported by osseointegrated implants, is presented and discussed.
