Dissociative disorders.

The dissociative disorders, including "psychogenic" or "functional" amnesia, fugue, dissociative identity disorder (DID, also known as multiple personality disorder), and depersonalization disorder, were once classified, along with conversion disorder, as forms of hysteria. The 1970s witnessed an "epidemic" of dissociative disorder, particularly DID, which may have reflected enthusiasm for the diagnosis more than its actual prevalence. Traditionally, the dissociative disorders have been attributed to trauma and other psychological stress, but the existing evidence favoring this hypothesis is p

Study of impulse-control disorders among alcohol-dependent patients.

BACKGROUND: Impulse-control disorders (ICDs) include intermittent explosive disorder, kleptomania, trichotillomania, pyromania, and pathological gambling. Several studies have suggested that the incidence of pathological gambling is substantially higher in alcoholics than in the general population. The rate of co-occurrence of other ICDs and alcohol dependence has never been systematically investigated. In our study, we assessed the frequency of all ICDs in a population of alcohol-dependent patients. We also examined the possibility that the presence of an ICD can correspond to earlier onset a

Prediction of attachment status from observation of a clinical intensive psychotherapy interview.

OBJECTIVE: The present study addresses whether it is possible to accurately determine a subject's Adult Attachment Interview (AAI) classification by observing a video-recorded clinical psychotherapy discussion that uses the principles of Intensive Short-Term Dynamic Psychotherapy (ISTDP). METHOD: A random sample of eight of the author's (Robert J. Nebrosky) private practice patients participated in an AAI administered by an experienced interviewer. The authors were blind to the results of the AAI, which were scored and classified by Erik Hesse, PHD (consultant and expert in AAI coding and cla

The role of transference work, the therapeutic alliance, and their interaction in reducing interpersonal problems among psychotherapy patients with Cluster C personality disorders.

The aim of the present study was to examine whether transference work, the therapeutic alliance, and their interaction predicted a reduction in interpersonal problems at treatment termination. Forty-nine patients with Cluster C personality disorders from a randomized controlled trial investigating the effectiveness of short-term dynamic psychotherapy and cognitive therapy were included. Transference work was measured with the Inventory of Therapeutic Strategies (Gaston & Ring, 1992), while the therapeutic alliance was measured with the Helping Alliance Questionnaire (Luborsky, Crits-Christoph,

Analysis of the BDNF Gene rs6265 Polymorphism in a Group of Women with Alcohol Use Disorder, Taking into Account Personality Traits.

It seems that BDNF has a direct influence on the brain pathways and is typically engaged during the processing of rewards. A surge in BDNF levels in the ventral tegmental area (the region from which the dopaminergic neurons of the mesocorticolimbic dopamine system originate and extend to the dorsolateral and ventromedial striatum) triggers a state of reward similar to that produced by opiates in animal studies. The aims of the study were (1) to analyze the association of the BDNF gene rs6265 polymorphism with AUD (alcohol use disorder) in women, (2) analyze personality and anxiety in alcohol-d

Unhealthy dependency in victims and perpetrators of child maltreatment: A meta-analytic review.

OBJECTIVE: Theoretical conceptualizations of interpersonal dependency and dependent personality disorder suggest that (a) high levels of dependency in parents may increase risk for perpetration of child abuse and neglect and (b) children who are victimized may show elevated levels of dependency later in life. This study used meta-analytic techniques to examine these hypothesized links. METHOD: A systematic search of psychological and medical online databases revealed 14 published studies (21 effect sizes) examining the link between parental dependency and perpetration of abuse or neglect (ove…

Emotional reliance and social loss: effects on depressive symptomatology.

A reactive form of dependence has been proposed to occur when a person is undergoing a period of substantial stress and change. The present study assessed 114 psychiatric inpatients categorized according to the presence or absence of social loss and their level of emotional reliance on others. Both emotional reliance and social loss were related to a variety of depressive symptoms. A significant interaction was observed between emotional reliance and social loss on depression severity as measured by the Beck Depression Inventory (BDI). In general, subjects high in emotional reliance but experi

Personality disorder traits associated with risk for unipolar depression during middle adulthood.

Data from the Children in the Community Study, a prospective longitudinal investigation, were used to investigate the association of personality disorder (PD) traits, evident by early adulthood, with risk for the development of unipolar depressive disorders by middle adulthood. Antisocial, borderline, dependent, depressive, histrionic, and schizotypal PD traits, identified between ages 14 and 22, were significantly associated with risk for dysthymic disorder (DD) or major depressive disorder (MDD) by a mean age of 33 after a history of unipolar depression and other psychiatric disorder was con

The female hysterical personality disorder.

It is not uncommon in family practice for the family physician to encounter the female patient with a hysterical personality disorder. A case presentation and interview are provided to help the family physician recognize the problem. A formulation of this case stresses psychosexual development, which will provide the reader with insight that can be generalized to other patients. Major emphasis is given to practical guidelines for the clinician for management of this frequently difficult problem in a medical setting.

Clinical pharmacology of phencyclidine toxicity.

Phencyclidine appears to be unique in action compared with other psychedelic drugs, and its effects are less dependent upon the individual's personality than are the effects of LSD or mescaline. The authors discuss the sensory, psychological, and behavioral symptoms of phencyclidine intoxication. Most cases are of short duration and the only treatment necessary may be observation together with minimal stimulation and diazepam. However, prolonged and severe behavioral disturbances, exaggeration of preexisting thought disorder, and serious medical complications commonly occur and must be conside

Assessment of object relations and reality testing in methadone patients.

OBJECTIVE: The authors studied methadone maintenance patients to determine the degree of their impairment in object relations and reality testing and the relationship of such impairment to comorbid axis I and axis II disorders. It was expected that deficits in object relations and, to a lesser degree, reality testing would be exhibited by the group as a whole and that they would be related to the presence of comorbid disorders. METHOD: The self-report Bell Object Relations Reality Testing Inventory was administered to 240 methadone maintenance patients. The subjects were first divided into gr

A catathymic infanticide.

A case of infanticide committed by a 37-year-old married man, the father of three sons, is reported. Clinically depressed since adolescence, and also diagnosed with obsessive-compulsive disorder and a dependent personality, the subject began to worry about killing someone a decade before the homicide. Increasingly disabled by his major depression, unable to work, and confined in his home, the idea that his only recourse was to kill one of his sons became fixed and frequent. Following his fourth psychiatric hospitalization, he took his 13-month-old son home from day care and drowned him in the

Profile clusters of the MCMI-II personality disorder scales.

An analysis was conducted to identify the major personality disorder score profiles to be found in the Millon Clinical Multiaxial Inventory II. Application of Ward's agglomerative hierarchical procedure to two subsamples of psychiatric patients (n = 83 for each) yielded four replicated subgroups. A subsequent K-means nonhierarchical approach confirmed four of the Ward clusters. Three subgroups can be characterized as (a) Antisocial, Aggressive (Sadistic), and Passive-Aggressive; (b) Avoidant, Schizoid, and Self-Defeating, and (c) Schizoid, Dependent, and Compulsive. A fourth sizable group is c

THE PROBLEM OF CO-DEPENDENCE AMONG HIV-INFECTED DRUG-ADDICTED FAMILY MEMBERS.

This paper is dedicated to the study of clinical-psychopathological manifestations of co-dependence in the families of HIV-infected injecting drug users and to finding out the structure of these manifestations and how often they happen, as well as the evaluation of the role of hereditary factors in the formation of co-dependence among those who are married to HIV-infected injecting drug users. It has been established that the specificity of genealogies of patients with co-dependence is confirmed by the following factors: high frequency of mental and behavioral disorders caused by alcohol abuse
