Case report of Modified Viabahn Open Revascularization TEChnique (VORTEC) as a rescue strategy for hepatic artery dissection after initial endovascular treatment of postpancreaticoduodenectomy hemorrhage.

RATIONALE: Currently endovascular treatments are commonly utilized to treat postpancreaticoduodenectomy hemorrhage. However, when endovascular procedure went wrong, open surgery with ligation of the culprit vessels would be the most common salvage method. With Modified Viabahn Open Revascularization TEChnique (VORTEC), we can try to rescue the vessel without sacrificing it by introduction of another endovascular stent under direct method. PATIENT CONCERNS: A 76-year-old man with stage IIIA ampulla vater adenocarcinoma underwent pancreaticoduodenectomy and experience pancreatic leak complicate

[Branched endovascular aortic repair of a contained rupture in chronic aortic dissection].

A late complication of aortic dissection is the dilatation of the weakened aortic wall. The only urgent endovascular alternative to high-risk open surgery in the treatment of postdissection aneurysms is branched endovascular aortic repair (BEVAR), which is extremely difficult due to the intimal membrane in the aorta lumen and the compressed true lumen. In this case report, we present the first application of BEVAR for aortic dissection in Hungary. Our 76-year-old male patient underwent type B aortic dissection 13 years before, for which supraaortic debranching was followed by thoracic stent gr

Computational Investigation and Histopathological Validation of Interaction Between Stent Graft and Aorta in Retrograde Type A Dissection After TEVAR in Canine Models.

PURPOSE: Retrograde type A dissection (RTAD) after thoracic endovascular aortic repair (TEVAR) has been a major drawback of endovascular treatment. To our knowledge, no studies have simulated and validated aortic injuries caused by stent grafts (SGs) in animal models. Therefore, the aim of this study was to evaluate and quantify the SG-aorta interaction through computational simulations and to investigate the underlying mechanism through histopathological examinations. METHODS: Two custom-made Fabulous® (DiNovA Meditech, Hang Zhou, China) SGs were implanted in 2 canine aortas with a 5-mm diff

Aortic Arch Anatomy in Candidates for Aortic Arch Repair.

Detailed knowledge of aortic anatomy is necessary before new prostheses can be developed. Our aim was to provide a thorough analysis of aortic arch anatomy in patients who are potential candidates for arch repair. Patients' charts were screened between 2001 and 2019 for all those with a dissection or aneurysm involving aortic arch. Aortic diameters, segmental lengths, aortic arch type, tortuosity, diameters and length of supraaortic vessels were analyzed via computed tomography angiography. We included 558 patients who underwent thoracic aortic treatment for type A, B, non-A non-B dissection,

Successful surgical repair of traumatic aortic dissection utilizing the Najuta stent graft in a patient with pre-existing situs inversus and anomalous neck vessels.

We report the case of a 39-year-old man who underwent surgical repair for traumatic Stanford type B acute aortic dissection. He was involved in a traffic accident, resulting in multiple rib fractures and a sternum fracture. On investigation, he was also found to have anatomical anomalies, namely, situs inversus and abnormality in the aortic arch and neck vessels. Thoracic endovascular aortic repair was performed as it is a less invasive procedure, thus recommended in such cases. A fenestrated thoracic endovascular aortic repair using the Najuta stent graft was found to be effective in this cas

Takayasu's arteritis and acute type B aortic dissection treated with a stent graft.

OBJECTIVES: Takayasu's arteritis is a large vessel vasculitis which usually involves the aorta and its first generation branches. Aortic dissection, however, is a rare manifestation of Takayasu's arteritis and for this reason optimum management in both short and long term is unknown. METHODS: This is a case of a 31-year-old female presenting with acute type B AD in association with underlying TA which failed to respond to conservative management and underwent thoracic stent grafting (TEVAR). RESULTS: Despite successful coverage of the intimal breach in the thoracic aorta by TEVAR there were

Comparative study of the frozen elephant trunk and classical elephant trunk techniques to supplement total arch replacement for acute type A aortic dissection†.

OBJECTIVES: In patients with acute type A aortic dissection, the use of the frozen elephant trunk (FET) procedure with total arch replacement (TAR) has been indicated for emergency operations to obtain thrombosis of the distal false lumen (FL). However, data comparing the FET and the classical elephant trunk (CET) procedures, including the incidences of mortality, morbidity, spinal cord injury and aortic remodelling, have not yet been reported. The goal of this study was to compare the early outcomes of TAR with the FET and the CET procedures. METHODS: The past 7 years of medical records of 3

Precise plan of hybrid treatment for thoracoabdominal aortic aneurysm: Hemodynamics of retrograde reconstruction visceral arteries from the iliac artery.

Hybrid visceral-renal debranching procedures with endovascular repair have been proposed as a less invasive alternative to conventional thoracoabdominal aortic aneurysm or dissection (TAAA or TAAD) surgery. Up to now, there has been no information about the hemodynamic effects of retrograde visceral reconstruction (RVR) for Crawford type II/III/IV TAAA patients undergoing hybrid treatment. The aim is to provide insights in the abnormal hemodynamics of RVR from unilateral or bilateral common iliac arteries (CIAs). Idealized three-dimensional AAs with RVR from unilateral CIA or bilateral CIAs we

Primary aortoenteric fistula with a chronic isolated abdominal aortic dissection: report of a case.

A 47-year-old woman on long-term hemodialysis due to a chronic isolated abdominal aortic dissection was admitted to our department with severe abdominal pain. She had not suffered any hematemesis or melena. An emergency laparotomy revealed an abdominal aortic aneurysm with a diameter of 60mm, densely adhered to the ileum. An aortoenteric fistula manifesting as intramural rupture into the ileum was found after infrarenal abdominal aortic and bilateral common iliac cross-clamping. The fistula on the ileac side was nontransmural, but that on the aortic side communicated with the pseudolumen of th

Surgical Techniques for a Parasitic Myoma Fed by the Mesentery: A Case Report and Technical Discussion.

Parasitic myoma (PM) is a rare form of leiomyoma that detaches from the uterus and acquires a blood supply from other organs. While often iatrogenic following power morcellation, primary spontaneous PM can occur. We present a surgical case of a PM fed by the mesentery, likely resulting from inflammatory adhesions following a previous small bowel ileus. A 33-year-old nulliparous woman with no history of prior surgeries underwent laparoscopic myomectomy for a suspected subserosal myoma. Intraoperatively, we encountered a PM with a significant vascular supply extending from the mesentery to the u

Vertebral artery dissection: natural history, clinical features and therapeutic considerations.

When a tear occurs in one of the major cervicocerebral arteries and allows blood to enter the wall of the artery and split its layers, the result is either stenosis or aneurysmal dilatation of the vessel. Vertebral artery dissection (VAD) is an infrequent occurrence but is a leading cause of stroke in young and otherwise healthy patients. This article discusses recent developments in understanding of the epidemiology and pathogenesis of VAD and the various clinical manifestations, methods of diagnosis, and approaches to treatment.

Pathogenesis of vasculitis syndromes.

The pathogenesis of vasculitis is complex and involves a variety of mechanisms acting in concert to bring about necrotizing inflammation of blood vessel walls. In recent years, there has been considerable progress in dissecting the immunologic abnormalities present in specific vasculitis syndromes. The primary immunopathogenic events that initiate the process of vascular inflammation and blood vessel damage, however, are still largely unknown. Although the cause of most vasculitis syndromes remains a mystery, advances in molecular and cellular immunology have defined many of the effector mecha
