A retrospective study was carried on 66 patients surgically treated for perforated diverticular colonic disease: 22 had acute phlegmon or pericolic abscess and underwent primary resection and anastomosis. Of the remaining 44 patients, who had multiple pericolic and/or pelvic abscesses, or generalized peritonitis, in 30 cases the Hartmann procedure was used, 6 underwent Mikulicz operation, while drainage with proximal colostomy was performed in 5 cases, and simple suture and drainage in the last 3 cases. Diverticulosis of the vermiform appendix, either single or multiple, congenital or acquired, is rather infrequent and usually asymptomatic. However, it may be complicated by flogosis configuring an acute abdomen hardly recognizable from an acute appendicitis not related to the diverticular disease. The Authors report a case of acute appendicular diverticulitis surgically treated. A brief review of the literature is also reported.

The authors consider some nosographic aspects of diverticular disease, and refer the personal experience on the treatment of 36 patients with diverticulitis. Beside surgery, that is indicated when complications occur (perforation, abscess, fistulae, occlusion), medical therapy (diet + drugs) could reduce the symptomatology and the risk or progression of complications. The authors present data on the clinical use of rifaximin in the treatment of acute diverticulitis.

The role of a contrast enema and computed tomography (CT) was evaluated in 228 patients with a discharge diagnosis of acute diverticulitis. Forty-nine percent of these patients eventually required surgery. In the surgical group, 90% of contrast enemas and 69% of CT examinations reported either diverticulitis or findings compatible with diverticulitis. To assess the value of sonography as the initial imaging procedure in patients suspected of having acute diverticulitis of the colon, we evaluated the sonograms of 71 patients who had clinical signs and symptoms of the disease. Fifty-seven patients who were treated conservatively for colonic diverticulitis between 1977 and 1979 were followed for at least 10 years. 17 patients with colonic diverticulitis of various degree were examined by conventional abdominal ultrasound in comparison to either colonoscopy or operation. The sonography of the inflammated region showed an echo-poor enlargement of the colonic wall (normal less than 3 mm) ranging from 5 to 15 mm (mean 7.8 mm) over a short distance. We report a case of inflammation of the spermatic cord and testicle resulting from a perforated diverticulum of the sigmoid colon. Management included sigmoid resection with diversion, right orchiectomy and débridement of the right groin. To our knowledge this is the first reported case of retroperitoneal necrotizing fasciitis presenting initially as an acute scrotum.

A case of vesicosigmoidal fistula secondary to the diverticulitis of colon is reported. A 63-year-old man was admitted to our clinic with the chief complaint of pneumaturia. Cystoscopy revealed an edematous, papillomatous lesion at the left posterior bladder wall. Colovesical fistula, a not infrequent complication of diverticulitis, can be difficult to demonstrate by commonly used diagnostic imaging modalities. We present four cases in which computed tomography was used to detect the presence of such a fistula. Computed tomography readily detected air within the bladder earlier and without equivocation when compared with other imaging techniques. It was useful also in the assessment of the extent and the degree of pericolonic inflammation, thus playing an important role in preoperative surgical planning and postoperative follow-up.

Small bowel diverticulitis is an unusual condition but the development of a secondary enterovesical fistula is extremely rare. This report describes the second documented case of ileovesical fistula secondary to ileal diverticulitis. Definitive diagnosis was made by double-contrast enema and small bowel fluoroscopic examinations. Management should consist of dissection of the fistula with either resection or primary repair.

A retrospective review of 1353 cases of acute perforated sigmoid diverticulitis treated surgically demonstrates that those operations that resect or exteriorize the perforated segment at the first operation are associated with a lower operative mortality rate than procedures that fail to remove the perforated segment at the initial operation. These results are true for both diffuse peritonitis and localized abscess.

Diverticulitis is a complex disease and demands careful cooperation between physician and surgeons, because although it is a benign disease, the presence of complications makes it potentially lethal.
