In lung transplantation, are pulmonary grafts from donors deceased from hanging as suitable as grafts from donors deceased from other causes?

A best evidence topic was constructed according to a structured protocol. The question addressed was whether pulmonary grafts from donors deceased from hanging offer the same benefit as grafts from donors deceased from other causes in lung transplantation. Of the 17 papers found, 4 provided the best evidence to answer the question. The authors, date, journal, country of publication, study type, group studied, relevant outcomes and results of these papers are tabulated. One study reported a large cohort of donors and analysed the outcomes by cause of death, reporting no differences in survival.

Is This Organ Donor Safe?: Donor-Derived Infections in Solid Organ Transplantation.

Infection is an inevitable complication of solid organ transplantation. Unrecognized infection may be transmitted from a donor and result in disseminated disease in the immunosuppressed host. Recent outbreaks of deceased donor-derived infections resulting in high rates of mortality and severe morbidity have emphasized the need to be cautious in using donors with possible meningoencephalitis. Screening of organ donors for potential transmissible infections is paramount to improving transplantation outcomes.

Infectious Complications in Lung Transplant Recipients.

Lung transplantation is a lifesaving intervention for patients with advanced lung disease. Due to a combination of immunosuppression, continuous exposure of the lungs to the environment, and complications at the anastomotic sites, lung transplant recipients are at high risk for infectious complications. The aim of this review is to summarize recent developments in the field of infectious diseases as it pertains to lung transplant recipients.

A comparison of two behavioral influence techniques for improving blood donor recruitment.

This study was designed to test the viability of two multiple request techniques of behavioral influence for recruiting blood donors by telephone. The first technique utilizes a small antecedent request to encourage behavioral involvement and favorable disposition toward the target activity of the critical request to donate. The second approach frames the critical request as a concession following refusal of a very large request. The two techniques, dubbed the foot-in-the-door (FID) and door-in-the-face (DIF), respectively, were tested against a control condition on three donor groups: active

Sentinel trends in donor hemoglobin values and low-hemoglobin deferrals in a large US blood center.

BACKGROUND: Low hemoglobin (Hb) is the leading cause of blood donor deferral yet mean Hb values are seldom examined as sentinel indicators of changes in donor recruitment, assessment, or testing. Case in point, following unexplained improvement in these parameters, U.S. blood centers observed a sharp rise in low-Hb deferrals after the 2024 introduction of a new international calibrator for some fingerstick hemoglobinometers. METHODS: We retrospectively analyzed 15 years (2010-2024) of allogeneic donor data from a national blood center. Mean donor Hb and low-Hb deferral rates were trended over

Clinical considerations in donor sperm use.

The use of donor sperm has increased substantially over the past two decades, creating a growing need for clinicians to navigate the medical, genetic, psychosocial, and regulatory considerations associated with donor insemination. This review summarizes recommended recipient evaluation, including ovarian reserve assessment, uterine and tubal evaluation, infectious disease screening, and expanded carrier screening, as well as key principles of donor selection and genetic reconciliation. We discuss current guidance regarding directed and nondirected donors, psychosocial counseling, and practical

Effect of donor exclusion criteria on blood safety and volume of donations: a systematic review of modelling studies.

OBJECTIVE: To assess the effect of easing bans and deferral policies on blood donation from men who have sex with men on blood safety and volume from modelling studies. METHODS: We searched four databases (PubMed®, Embase®, CINAHL and Web of Science) for modelling studies on the impact of replacing bans or deferrals on blood donations from men who have sex with men with shorter deferral periods or no deferrals. We synthesized and compared findings from the different modelling studies, and assessed risk of bias and certainty of evidence. FINDINGS: Fourteen publications were included in the st

Hepatitis B Blood Donor Screening Data: An Under-Recognized Resource for Canadian Public Health Surveillance.

Hepatitis B surveillance is essential to achieving Canada's goal of eliminating hepatitis B by 2030. Hepatitis B rates, association of infection with vaccine age-eligibility, and risk factors were analyzed among 1,401,603 first-time Canadian blood donors from 2005 to 2020. Donors were classified as having likely chronic or likely resolved/occult infections based on hepatitis B surface antigen, anti-hepatitis B core antigen, and hepatitis B nucleic acid test results. Likely chronically infected and control donors (ratio 1:4) participated in risk-factor interviews. The 2019 rate of likely chroni

Re-engineering the medical assessment of blood donors in South Africa: The balance between supply and safety.

BACKGROUND: The balance between ensuring blood donor and recipient safety while maintaining a sufficient blood supply can be affected by excessive deferral of blood donors. In 2018, a biannual regulatory review of donor eligibility criteria provided the South African National Blood Service (SANBS) the opportunity to review the existing criteria. Changes to these criteria were implemented in April 2019 after an extensive review. STUDY DESIGN AND METHODS: We conducted a cross-sectional study of SANBS whole-blood donor presentations to determine the impact of the changed donor eligibility criter

Organ Donation and Primary Central Nervous System Tumors.

Primary central nervous system tumors can be the cause of brain death. Not all of them contraindicate the donation of organs and tissues for transplant. A survey of cases was carried out in our country in which it was observed that the number of brain deaths caused by primary tumors was low, of the order of 2%, with an ẋ (media) of 3 by year, which would increase the potential for donation. Medical records, an anatomopathologic study, and a detailed physical examination will be fundamental when applying the donor selection criteria. Nuclear magnetic resonance in expert hands has a sensitivity

Receiving the Unwanted Gift: Infection Transmission through Organ Transplantation.

BACKGROUND: Infections are common in the general U.S. population, so it is inevitable that some persons with a potentially transmissible disease will become organ donors. There are numerous reports of viral, parasitic, fungal, and bacterial transmission through transplantation. At the same time, immunosuppression increases the risk of infection in organ recipients, so attribution of infectious diseases to the transplanted organ is often difficult. METHOD: Review of the English-language literature. RESULTS: The Organ Procurement and Transplantation Network states that all potential deceased o

Pushing the frontiers of living donor right hepatectomy.

Living donor right hepatectomy (LDRH) is currently the most common donor surgery in adult-to-adult living donor liver transplantation although the morbidity and mortality reported in living donors still contradicts the Hippocratic tenet of "do no harm". Achieving low complication rates in LDRH remains a matter of major concern. Living donor surgery is performed worldwide as an established solution to the donor shortage. The aim of this study was to assess the current status of LDRH and comment on the future of the procedure; assessment was made from the standpoint of optimizing the donor selec

[How can the number of liver transplantations be increased?].

The number of patients suitable for liver transplantation is progressively increasing due to the excellent results achieved with this procedure, giving rise to a growing imbalance in the number of candidates on the waiting list and the number of donors. This situation has prompted transplant teams to search for alternatives to increase the number of liver grafts. On the one hand, the criteria for donation have been broadened to include donors with advanced age, liver steatosis, hepatitis B and C viruses, neoplasms, and benign underlying diseases. On the other hand, new transplant techniques ha

Current status and potential of living-donor lobar lung transplantation.

Mosquitoes are vectors for a number of disease-causing pathogens such as the yellow fever virus, malaria parasites and filarial worms. Laboratories are investigating anti-pathogen components of the innate immune system in disease vector species in the hopes of generating transgenic mosquitoes that are refractory to such pathogens(1, 2). The innate immune system of mosquitoes consists of several lines of defense (3). Pathogens that manage to escape the barrier imposed by the epithelium-lined mosquito midgut (4) enter the hemolymph and encounter circulating hemocytes, important cellular componen

The United States' potential blood donor pool: estimating the prevalence of donor-exclusion factors on the pool of potential donors.

BACKGROUND: Efforts to ensure donor and recipient safety have reduced the population of eligible voluntary blood donors. The current method for determining eligible blood donors in a population using only age as the criterion for excluding donors poorly reflects the large constellation of factors known to cause donor deferrals. An epidemiologic model has been developed to determine the prevalence of donor exclusions and thus improve the estimate of total eligible blood donors in the nation. STUDY DESIGN AND METHODS: Epidemiologic databases were selected to enumerate the population prevalence

Donor exclusion in the National Blood Service Tissue Services living bone donor programme.

National Blood Service (NBS) Tissue Services (TS) operates living donor and deceased donor tissue banking programmes. The living bone donor programme operates in collaboration with 91 orthopaedic departments across the country and collects bone donations, in the form of surgically removed femoral heads (FHs), from over 5,000 patients per annum undergoing total hip replacement. Bone donated via the living programme constitutes approximately 55% of the total bone donated to NBS. Non-NBS tissue banks, primarily in hospital orthopaedic departments, also bank donated bone for the UK. A survey of in

Assessment of the willingness of potential live related kidney donors.

Even in the case of live related kidney donation, coercion within families is applied on the most vulnerable amongst them. This vulnerability could be financial, social or psychological In this paper we discuss how to assess the willingness or unwillingness of a kidney donor and discover if any coercion has been applied on him/her be it direct or indirect.
