Adjusted Internal Joint Stabilizer Implants as a Supplementary Management of Chronic Elbow Injury: A Preliminary Case Series.

Chronic elbow injuries often cause instability and stiffness. The Internal Joint Stabilizer of Elbow (IJS-E), introduced in 2014, stabilizes the elbow and enables early mobilization. In resource-limited settings where the IJS-E is unavailable, an adapted radius T-plate technique has been developed. This study evaluated patients (n=11) with chronic elbow injuries, showing significant improvements in Mayo Elbow Performance Scores (MEPS) and range of motion (ROM). Despite challenges like implant rigidity and precise screw placement, the T-plate effectively maintained stability and allowed early R…

Median Nerve Injury and Repair After Pediatric Elbow Dislocation Without an Associated Fracture: A Case Report.

CASE: A 9-year-old boy presented with a severe median nerve palsy 6 months after a simple elbow dislocation. Neurotmesis was confirmed on surgical exploration, and it was repaired primarily in elbow flexion with the arm casted postoperatively. At 1-year follow-up from surgery, the patient reported near normal sensory improvement with motor deficits that did not affect his full return to activities. CONCLUSION: The median nerve neurotmesis after elbow dislocation without an associated fracture is rare. This case highlights the importance of careful neurologic examination in children sustaining

Diagnosis and treatment of lateral to medial diagonal injury of the elbow in children: concomitant lateral condyle and olecranon fractures.

Lateral condylar (LC) and olecranon (OC) fractures occurring concurrently on one side are rare phenomena with limited scientific literature. Our study aims to evaluate the radiologic, clinical, and functional consequences of lateral to medial injury of the elbow (LAMEINE) in pediatric patients, simultaneously comparing with lateral to medial diagonal injury of the elbow (MELAINE). Eighteen males and 10 females were diagnosed with LAMEINE. The average age at the time of injury was 3.8 ± 2.3 years (range, 1-9). Out of the 28 fractures, 19 (67.9%) occurred on the left side and nine (32.1%) on the

Three-dimensional quantitative study and functional outcome analysis of coronoid fracture in different elbow injury patterns.

BACKGROUND: Coronoid fractures are often part of a complex fracture-dislocation of the elbow. For an optimum prognosis, it is important to understand the characteristics of coronoid fractures in different elbow injury patterns. Significant differences in these characteristics have been observed in various studies, but these previous studies have suffered from limitations, such as insufficient quantification and unknown prognostic differences among different injury patterns. Therefore, we aimed to quantitatively analyze coronoid fracture characteristics and functional outcomes in different elbo

Effects of Contralateral Trunk Tilt on Shoulder and Elbow Injury Risk and Pitching Biomechanics in Professional Baseball Pitchers.

BACKGROUND: Baseball pitchers employ various contralateral trunk tilt (CTT) positions when pitching depending on if they have an overhand, three-quarter, or sidearm delivery. There are no known studies that have examined how pitching biomechanics are significantly different in professional pitchers with varying amounts of CTT, which may provide insight into shoulder and elbow injury risk among pitchers with different CTT. PURPOSE: To assess differences in shoulder and elbow forces and torques and baseball pitching biomechanics in professional pitchers with maximum 30° to 40° CTT (MaxCTT), mod

Characteristics of upper limb mass, muscle CSA and stiffness in adolescent baseball players with and without elbow injury.

Since elbow injuries are common in adolescent baseball players, this study aimed to determine the relationship between musculoskeletal status and elbow injuries in 47 baseball players aged 12-14 years. Participants answered a questionnaire and had their general body measurements taken. Magnetic resonance imaging was used to evaluate muscle cross sectional areas (CSA) of the upper arm flexor and extensor muscles, and x-ray absorptiometry was used to determine the mass of the upper arm and the total mass of the forearm and hand. Shear wave elastography was used to determine muscle stiffness of t…

Ultrasonographic Measurement of Elbow Varus Laxity With a Sequential Injury Model of the Lateral Collateral Ligament-Capsular Complex.

BACKGROUND: There is no consensus how to determine the varus laxity due to the LCL injury using the ultrasonography. There is a risk of lateral collateral ligament injury during or after arthroscopic extensor carpi radialis brevis release for tennis elbow. The equator of the radial head has been suggested as a landmark for the safe zone to not increase this risk; however, the safe zone from the intra-articular space has not been established. HYPOTHESIS: Increased elbow varus laxity due to lateral collateral ligament-capsular complex (LCL-cc) injury could be assessed reliably via ultrasound.

Can a Partially Injured Donor Nerve Restore Elbow Flexion in an Acute Brachial Plexus Injury in Rats?

BACKGROUND: Loss of elbow flexion commonly occurs following acute brachial plexus injury. The double fascicular transfer is often used in acute C5-C6 and C5-C7 root injuries, but is rarely applied in cases involving concomitant C8 or T1 root injury. The authors designed a rat model using varying severities of lower trunk injury to determine whether partial injury to the lower trunk affects nerve transfers for elbow flexion. METHODS: There were four different rat groups in which 0, 25, 75, or 100 percent of the donor lower trunk remained intact. One-fourth of the cross-sectional area of the ul

Use of the fix and flap approach to complex open elbow injury: the role of the free anterolateral thigh flap.

BACKGROUND: Complex elbow injuries with associated nerve, muscle, or joint injury commonly develop post-inury stiffness. In order to preserve function, joint congruency, elbow stability and durable wound coverage must be achieved in a timely manner. METHODS: A retrospective review of patients who underwent orthopaedic fixation followed by free anterolateral thigh (ALT) flap soft tissue coverage was performed. Five patients were identified and included in this study. RESULTS: We present a series of 5 cases managed with this principle. Soft tissue defects ranged in size from 4×9 cm (36 cm(2)) …

Restoration of elbow flexion after brachial plexus injury by free functional rectus femoris muscle transfer.

A 22-year-old patient sustained a complex injury of the left brachial plexus. Primary brachial plexus reconstruction did not lead to any functional recovery. Twenty-six months later, the patient showed an upper brachial plexus palsy, which precluded the possibility for a latissimus dorsi muscle transfer. To re-establish elbow flexion, a free myocutaneous neurovascular rectus femoris flap, harvested from the left thigh, was neurotised to the accessory nerve using a sural nerve graft. Ten months after the free functional rectus femoris transfer, early electromyographic results were detected, and

Utilizing remaining voluntary muscle synergies to control FES elbow extension after spinal cord injury.

Individuals with a C5/C6 spinal cord injury (SCI) have paralyzed elbow extensors, yet retain weak to strong voluntary control of elbow flexion and some shoulder movements. They lack elbow extension, which is critical during activities of daily living. This research focuses on development of a synergistic controller employing remaining voluntary elbow flexor and shoulder electromyography (EMG) to control elbow extension with functional electrical stimulation. We hypothesized that remaining voluntarily controlled upper extremity muscles could be used to train an artificial neural network (ANN) t

To Explore or Not? Conservatively Managed Brachial Artery Injury with Good Collateral Flow After Elbow Reduction: A Case Report.

INTRODUCTION: Elbow dislocation, the second most common dislocation involving a large joint, typically results from a fall on an outstretched hand and most frequently presents as a posterior displacement. Neurovascular compromise may involve the ulnar, median, or radial nerves, with injury to the brachial artery being of particular clinical significance. Due to its anterior location within the cubital fossa and relative fixation, the brachial artery is especially susceptible to stretching or rupture during dislocation. In addition, a well-developed collateral vascular network may preserve dist
