Water and electrolyte balance after ileoanal anastomosis.

Water and electrolyte balance was studied in 30 patients with ileoanal anastomosis and J pouch, 10 patients with conventional ileostomy, and nine nonoperated patients with quiescent ulcerative colitis. Serum electrolyte concentrations, daily urinary volume, and daily losses of sodium, potassium, and chloride were measured in all patients. Daily fecal weight and daily losses of sodium and potassium were analyzed in patients with ileoanal anastomosis or conventional ileostomy. Serum chloride in patients with ileoanal anastomosis was significantly lower (P less than 0.05) than in those with conve

Electrolyte transport across the rabbit caecum in vitro.

Electrolyte transport across rabbit caecal epithelium was investigated in vitro using conventional short-circuiting and radioisotope techniques. In standard saline the caecum exhibited a relatively high short-circuit current (Isc = 4.4 microEq.cm-2.h-1) and conductance (6.43 mS.cm-2). Both sodium and chloride were absorbed (JNa(net) = 6.40 and JCl(net) = 3.40 microEq.cm-2.h-1) and potassium was secreted (JK(net) = -0.5 microEq.cm-2.h-1). Removal of Na+ abolished Isc and JCl(net) whereas removal of Cl- reduced JNa(net) to 2.92 microEq.cm-2.h-1 but did not alter Isc. In HCO3(-)-free salines cont

Electrolyte imbalances in cardiovascular disease: the forgotten factor.

Hypomagnesemia is not an infrequent finding in diuretic-induced hypokalemia. Potassium replacement therapy in these cases may be ineffective in raising serum potassium levels unless normal magnesium concentrations are restored.

The lung and fluid-electrolyte and acid-base imbalances.

This article describes the contribution of the lungs to the regulation of acid-base balance with a focus on clinical implications. The interpretation of arterial blood gas results is described, and nursing interventions to address acid-base imbalances are discussed. A case study illustrates acid-base imbalances in chronic obstructive lung disease.

Assessment and nursing diagnosis in fluid and electrolyte disorders.

Disorders of fluid and electrolyte balance are common considerations in adult medical-surgical clients with underlying pathologic conditions. The assessment framework presented in an integrated approach is an attempt to provide the clinician with a nursing model to be used in gaining pertinent clinical information from clients with existing or suspected fluid and electrolyte disorders. The use of health patterns facilitates the collection and organization of information gained by the nurse through assessment. This framework assists the nurse in processing subjective and objective assessment da

Protective effect of steroids in electrolyte-induced demyelination.

Electrolyte-induced demyelination (EID), an experimental model for central pontine myelinolysis was produced in rats by inducing hyponatremia followed by hypernatremia. There was a marked reduction in the number and size of lesions developing in animals that were bled repeatedly by tail transection during induction of the disease. Subsequently a similar protective effect was produced in animals by injecting a single dose of dexamethasone, one hour before the induction of hypernatremia. These findings suggest that steroids may be useful in preventing central pontine myelinolysis from developing

Electrolyte composition of the amniotic fluid in Bartter syndrome.

In three patients with neonatal Bartter syndrome associated with polyhydramnios, analysis of the amniotic fluid showed normal sodium, normal-to-low potassium, but high chloride concentrations. This finding clearly suggests a renal chloride reabsorption defect as the primary cause of the neonatal form of Bartter syndrome. It is suggested that whenever polyhydramnios occurs, the electrolyte composition of the amniotic fluid should first be analysed in order to establish the diagnosis of Bartter syndrome.

Mast cells in the regulation of intestinal electrolyte transport.

Experiments were performed to determine the ionic basis and physiological messengers of transepithelial ion transport alterations (short-circuit current, Isc) measured during the induction of intestinal anaphylaxis in an Ussing chamber. Antigens derived from Trichinella spiralis, an intestinal parasite, were used to challenge jejunal tissue from guinea pigs immunized by infection with the parasite. Histamine (10(-4) M) caused an increased in Isc that was similar to that induced by antigen. Diphenhydramine (10(-5) M) inhibited the epithelial electrical responses to histamine by 100% and to anti

Disorders of the adrenal cortex: their effects on electrolyte metabolism.

The adrenal cortex is an important factor in the control of electrolyte and water balance and in blood pressure homeostasis. Not surprisingly, therefore, hyper- and hyposecretion of one or more of its products has extensive repercussions. Hypersecretion of aldosterone, as seen in primary hyperaldosteronism and related diseases, and of other mineralocorticoids such as corticosterone and/or 11-deoxycorticosterone, as seen in 17 alpha-hydroxylase deficiency or 11 beta-hydroxylase deficiency syndromes, respectively, are associated with hypertension, sodium retention, potassium wastage and a metabo

Lithium administration and urinary electrolyte excretion in the rat.

The effect of lithium administration on urine eletrolyte excretion is controversial with reports of increased, unaltered and reduced excretion rates of sodium, potassium, calcium, magnesium and inorganic phosphate. Therefore rats treated with daily intraperitoneal lithium chloride for 3 weeks were studied and were found to have marked fluctuations in electrolyte excretion when compared to untreated rats. Although lithium-treated rats were always polyuric, urinary sodium, potassium and calcium excretion was significantly elevated immediately following intraperitoneal lithium but their excretion

A physiological approach to fluid and electrolyte therapy in the horse.

In this article a physiological approach to fluid therapy is discussed, commencing with examination of fluid distribution in the normal horse. The functions of individual plasma electrolyte concentrations are considered and practical causes of acid-base disturbances discussed. When fluid administration is necessary, selection of the route for fluid administration as well as the type of fluid are important considerations and these must be adjusted to the needs of the individual case. Balanced polyionic solutions appear to be most suitable for general use because normal saline can produce hypoka

Effect of phototherapy on neonatal fluid and electrolyte status.

Three groups of "Работает' fullterm infants with hyperbilirubinaemia exposed to continuous phototherapy of different intensities, increased their oral intake of milk formula by 33%, 48% and 52% and 24, 48 and 72 h, respectively, irrespective of the intensity of exposure; this was significantly more than in the controls, who were swaddled. Serum osmolality and electrolyte values demonstrated no significant change from pre-phototherapy values, and were similar to those of the control group except for significantly raised potassium levels at 72 h in 2 of the 3 phototherapy groups. These raised val

Treatment of undernutrition and electrolyte disturbances in anorexia nervosa.

Medical care of undernutrition and hydro-electrolytic disturbances in anorexia-nervosa differs according to the stage of weight loss, as appreciates through "corpulence index". In each case, it will depend on a precise appreciation of biological data, mainly hydro-electrolytic ones. One has to particularly stress the frequence of hypochloremic-hypokalemic alkalosis which must be treated by isotonic saline perfusion till the normalisation of plasma electrolytes. Refeeding can be achieved either by oral, parenteral or nasogastric tube: this last method being, in our hand, the most successful. Th…

'Sick cell' hyponatraemia and gastrointestinal electrolyte loss.

A patient with protracted vomiting was noted to have hyponatraemia and hypokalaemia on admission. Isotope dilution studies before treatment revealed profound body depletion of sodium, potassium and water. Treatment with potassium chloride tablets produced a simultaneous rise in serum sodium and potassium concentration without change in urine volume or osmolality, or body weight. Potassium depletion appears to have contributed to the mechanism of hyponatraemia in this situation, and that replacement of potassium corrected hyponatraemia by effectively re-distributing solute and water across the
