Diaphragm electromyography results at different high flow nasal cannula flow rates.

Heated, humidified, high-flow nasal cannula (HHHFNC) is increasingly being used, but there is a paucity of evidence as to the optimum flow rates in prematurely born infants. We have determined the impact of three flow rates on the work of breathing (WOB) assessed by transcutaneous diaphragm electromyography (EMG) amplitude in infants with respiratory distress or bronchopulmonary dysplasia (BPD). Flow rates of 4, 6 and 8 L/min were delivered in random order. The mean amplitude of the EMG trace and mean area under the EMG curve (AEMGC) were calculated and the occurrence of bradycardias and desat…

Vocal fold motion outcome based on excellent prognosis with laryngeal electromyography.

OBJECTIVES/HYPOTHESIS: As laryngeal electromyography (LEMG) becomes more refined, accurate predictions of vocal fold motion recovery are possible. Focus has been on outcomes for patients with poor prognosis for vocal fold motion recovery. Limited information is available regarding the expected rate of purposeful vocal fold motion recovery when there is good to normal motor recruitment, no signs of denervation, and no signs of synkinetic activity with LEMG, termed excellent prognosis. The objective of this study is to determine the rate of vocal fold motion recovery with excellent prognosis fin…

Remodeling of motor units after nerve regeneration studied by quantitative electromyography.

OBJECTIVE: Peripheral nerve has the capacity to regenerate after nerve lesions; during reinnervation of muscle motor units are gradually reestablished. The aim of this study was to follow the time course of reestablishing and remodeling of motor units in relation to recovery of force after different types of nerve repair. METHODS: Reinnervation of muscle was compared clinically and electrophysiologically in complete median or ulnar nerve lesions with short gap lengths in the distal forearm repaired with a collagen nerve conduit (11 nerves) or nerve suture (10 nerves). Reestablishment of motor

Cervical radiculopathy: discrepancy or concordance between electromyography and magnetic resonance imaging?

In the screening of patients with cervical radiculopathy, a relatively common finding is the discrepancy between normal needle electromyography (EMG) and abnormal MRI, or vice versa. We carried out a retrospective study to assess the relationship between needle EMG and MRI findings in the preoperative evaluation of patients with cervical radiculopathy. The records of 147 patients were reviewed; 58 patients (M = 32, F = 26; age 53 ± 9) were included in the study. Needle EMG examination was abnormal in 28 patients. MRI abnormalities were found in all 58 patients, showing degenerative changes not…

Classification of Temporomandibular disorder from electromyography signals via Directed Transfer Function.

A new approach for the detection of Temporomandibular joint disorders (TMDs) from the recorded electromyography (EMG) signals from the muscles around the temporomandibular joint (TMJ) has been presented in this paper. Multivariate Autoregressive (MVAR) modelling has been applied to a six-channel set of EMG signals from the muscles of both sides of the jaw during mouth opening and closing. The MVAR coefficients are then used to define the Directed Transfer Function (DTF), which estimates the strength of the direction of the signals flow between the channels. The DTF energy parameters were chose…

Improving functional magnetic resonance imaging motor studies through simultaneous electromyography recordings.

Specially designed optoelectronic and data postprocessing methods are described that permit electromyography (EMG) of muscle activity simultaneous with functional MRI (fMRI). Hardware characterization and validation included simultaneous EMG and event-related fMRI in 17 healthy participants during either ankle (n = 12), index finger (n = 3), or wrist (n = 2) contractions cued by visual stimuli. Principal component analysis (PCA) and independent component analysis (ICA) were evaluated for their ability to remove residual fMRI gradient-induced signal contamination in EMG data. Contractions of an…

Standardization of anal sphincter electromyography: quantification of continuous activity during relaxation.

AIM: Sphincter motor units (MUs) are continuously firing during relaxation. The quantification of this activity is a potentially useful electromyographic (EMG) parameter related to the number of MUs innervating the muscle and the level of motor neuron excitation. The aim of the present study was to develop a reliable, quantitative measure of continuous sphincter muscle activity during relaxation. METHODS: EMG activity was analyzed during relaxation, 1 minute after insertion of the concentric needle electrode into four sites in the external anal sphincter (EAS) muscle. In 10 control subjects,

Standardization of anal sphincter electromyography: utility of motor unit potential parameters.

Advanced electromyography systems offer quantitative analysis of a number of motor unit potential (MUP) parameters. However, only limited data are available on the diagnostic usefulness of these parameters. In the present study, we compared the sensitivities of MUP parameters in revealing "neuropathic" changes in the external anal sphincter (EAS) muscles in 56 patients examined 5-240 months after damage to the cauda equina or conus medullaris. Using multi-MUP analysis, 20 MUPs were obtained from patients' EAS muscles. Their MUP parameters were compared with normative data from 64 controls. The…

Continuous electromyography monitoring of motor cranial nerves during cerebellopontine angle surgery.

OBJECT: Electromyography (EMG) monitoring is expected to reduce the incidence of motor cranial nerve deficits in cerebellopontine angle surgery. The aim of this study was to provide a detailed analysis of intraoperative EMG phenomena with respect to their surgical significance. METHODS: Using a system that continuously records facial and lower cranial nerve EMG signals during the entire operative procedure, the authors examined 30 patients undergoing surgery on acoustic neuroma (24 patients) or meningioma (six patients). Free-running EMG signals were recorded from muscles targeted by the faci…

Single fibre electromyography in multifocal motor neuropathy with persistent conduction blocks.

OBJECTIVE: To study the process of denervation-reinnervation in multifocal motor neuropathy with persistent conduction blocks in clinically affected and unaffected muscles. METHOD: Volitional single fibre electromyography (SFEMG) was performed in the extensor digitorum communis (EDC) of seven patients. The jitter, the fibre density, and the mean interpotential interval were determined. The results before and after treatment with intravenous immunoglobulin (IVIg) between the unaffected EDC and affected EDC examined during the same SFEMG session were also compared. In addition the values of jit…

Experience with two types of electromyography monitoring electrodes during thyroid surgery.

PURPOSE: To prevent damage to the recurrent laryngeal nerve during thyroid or parathyroid surgery, spontaneous and evoked electromyography (EMG) can be used to locate the recurrent laryngeal nerve or determine when the nerve is being disturbed. Two commercial electrodes have been introduced to receive laryngeal EMG. This study compares the clinical performance of the two electrodes. PATIENTS AND METHODS: The Xomed-Treace endotracheal tube electrode (Xomed-Treace, Jacksonville, FL) and the RLN postcricoid laryngeal electrode (RLN Systems, Jefferson City, MO) were placed concomitantly in 11 pat…

Use of spontaneous electromyography during revision and complex total hip arthroplasty.

Intraoperative peripheral nerve injury is a serious potential complication of orthopaedic surgery and various intraoperative neurophysiologic monitoring techniques have been used to avoid this complication. Although somatosensory evoked potentials have been used effectively in spinal surgery, the efficacy of this technique has not been demonstrated in total hip arthroplasty. Spontaneous electromyography is a promising, alternative nerve monitoring technique. This technique was used in 44 consecutive revision and complex hip arthroplasty procedures. Five cases demonstrated sustained electromyog…

Single fiber electromyography in normal subjects: reproducibility, variability, and technical considerations.

We performed 59 single fiber electromyography (SFEMG) examinations on 33 patients with spasmodic torticollis, but no other medical or neurological disorder. 14 patients were studied serially (3 examinations over 12 weeks). There was excessive jitter (greater than 55 microseconds) in 14 of 1180 fiber pairs (1.1%). 7 of the 14 abnormal fiber pairs were found at single sites (1 in 20 sites studied). Abnormal jitter occurred at more than 2 of 20 sites in only two patients, both of whom head increased fiber density and were more than 70 years of age. Of the 14 abnormal fibers, subtle technical prob…

Temporal patterns of bruxism behaviors: multiple day recording with portable electromyography.

To assess inter-day variabilities and define temporal patterns of awake and sleep muscular activity during 72 h of continuous electromyographic (EMG) recording. 20 subjects used a portable EMG device for 72 h. Additional to the variables supplied by the instrument's algorithm, the number of grinding and of clenching episodes, per each hour of wakefulness and each hour of sleep, was counted. Number of grinding and clenching of episodes, per hour of sleep and wakefulness, were used in a mixed model analysis. For sleep grinding episodes, a linear temporal model showed a day-to-day increase indica
