Pretibial myxedema or localized myxedema or thyroid dermopathy is an autoimmune manifestation of Graves' disease. It also occasionally occurs in Hashimoto's thyroiditis. Lesions of thyroid dermopathy are usually asymptomatic and have only cosmetic importance. Advanced forms of dermopathy are associated with elephantiasis or thyroid acropachy. Almost all cases of thyroid dermopathy are associated with relatively severe ophthalmopathy. Usually ophthalmopathy appears first and dermopathy much later. All patients with localized myxedema have high serum concentrations of thyroid-stimulating hormone…

Lymphatic filariasis is recognized as one of the world's most disabling diseases. Adult lymphatic filarial nematodes live for several years; they infest the lymphatic and blood systems, and are transmitted via mosquito vectors. Acute and chronic disease can develop, manifesting as acute inflammation, hydrocoele, lymphoedema and elephantiasis. The development of a global programme for the elimination of lymphatic filariasis (focusing on disease caused by Wuchereria bancrofti) followed a resolution by the World Health Assembly in 1997 that endemic countries should develop programmes to eliminate…

A markedly obese, 41-year-old Japanese man who had suffered from psoriasis vulgaris for several years visited us with elephantiasis-like swelling of his lower legs of three months' duration. His right lower leg showed marked papillomatosis with thick scales, and the left lower leg was eroded and papillomatous. Although direct lymphography of his lower extremities showed no abnormality, indirect lymphography revealed local lymphatic damage in the involved skin. Histological examination showed hyperkeratosis, marked papillomatosis, proliferation of capillaries in the upper dermis, and lymphectas…

A house-to-house screening exercise was carried out in Shao, Kwara State, Nigeria, to determine the status of onchocerciasis in the town. Th … tations of onchocerciasis, including blindness, hernias, hanging groins, 'leopard skin' and scrotal elephantiasis, strongly suggest that the community had a history of long-standing onchocerciasis. The desirability for the implementation of a nation-wide control programme in Nigeria to complement the ongoing control efforts in the Volta River Basin Areas, is hi …

We evaluated the cellular immune competence of 101 subjects living in an area of South Kalimantan (Borneo) where Malayan filariasis is endemic. All patients with elephantiasis but none with other clinical stages of filariasis reacted with adult worm antigens. The majority of subjects without clinical or parasitological evidence of filariasis and approximately one-half of those with amicrofilaremic filariasis reacted with microfilarial antigens. In contrast, most patients with patent microfilaremia did not respond to microfilarial antigens. The in vitro reactivity of all patient categories to n…

A survey in 5 study areas along the Liberian coast was conducted to gain information on the prevalence and importance of bancroftian filaria … th Wuchereria bancrofti. An estimated 2500 to 40000 men suffering from hydroceles and 3700 to 40000 elephantiasis patients demonstrate that bancroftian filariasis has to be regarded a health problem in rural coastal Liberia.

Filarial parasites are able to survive for many years in their host, with suppression of the host's immune response being one major survival strategy of the parasite. However, knowledge on molecules that induce these pathways is limited. Additionally, molecules that induce inflammation, thereby leading to severe pathology, such as elephantiasis, are also not fully identified. This article assesses the findings of a recently published analysis of stage-specific excretory-secretory proteins by sodium dodecylsulfate-polyacrylamide gel electrophoresis in combination with liquid chromatography-tand…

The aim of this study, carried out between August 1991 and August 1992, was to understand community perception regarding chronic filarial swellings in Lutsangani village, Kwale district, Kenya. Interviews were conducted with 65 patients and informal discussions held with family and community members. The results indicate that bodily change is the concern of most patients who felt that their physical appearance influenced community reactions towards them. These reactions varied from sympathy to neglect. The community attributed elephantiasis to causes other than mosquitoes and these included wi…

We conducted an implementation research study to integrate a holistic package of physical health, mental health and psychosocial care for podoconiosis, lymphatic filariasis and leprosy into routine healthcare in Gusha cluster, Guagusa Shikudad district, northwest Ethiopia. The healthcare package included training patients in lower limb hygiene and skin care and provision of shoes, hygiene supplies and medication. The implementation activities included training events, workshops, awareness raising, self-help groups, supportive supervision, staff secondments and advisory board meetings. The cost

Hereditary gingival enlargement also known as gingivitis or familial elephantiasis is a rare type of gingival enlargement. It appears as an isolated autosomal dominant disorder or maybe associated with other conditions. Oral manifestations may vary from minimal involvement of only tuberosity area and the buccal gingiva around the lower molars to a generalized enlargement inhibiting eruption of the teeth. This paper discusses the case of a 13-year-old female patient with distinctive facial characteristics who presented to the department with a chief complaint of swollen gums since 1 year. She h…

Only 1 individual converted to having asymptomatic microfilariae. No case of elephantiasis was observed. Results indicate that most females remained normal and males faced considerable risk of developing hydrocoele in these Wuchereria bancrofti-endemic areas.

Lymphatic filarial nematodes are infected with endosymbiotic Wolbachia bacteria. Lipopolysaccharide from these bacteria is the major activat … to opportunistic infections, which cause acute dermatolymphangitis associated with lymphoedema and elephantiasis. The recognition of the contribution of endosymbiotic bacteria to filarial disease could be exploited for clinical intervention by the targeting of bacteria with antibiotics in an attempt to reduce the development of filarial pathology.
