Elephant-Skin-Inspired Porous Cementitious Tiles with Programmable Crack Networks for Passive Cooling.

Passive evaporation of water reduces a building's cooling energy demand. However, water is often wasted due to rebound, uneven spreading, and rapid drainage. Here, we present an elephant-skin-inspired crack network architecture in porous diatomaceous earth (DE)-cement composites to capture, route, and store water with minimal runoff. DE's micro/nanoporosity enables ultrafast (sub-50 ms) water imbibition, while crack networks act as capillary conduits that redistribute water across and up inclined surfaces. Substrate-guided stress concentration converts drying-induced stochastic fractures into

Frozen elephant trunk technique in redo aortic arch surgery: a step-by-step approach.

Frozen elephant trunk (FET) is an established treatment modality for complex aortic arch and proximal descending aortic pathologies. While patients with limited life expectancy are more frequently considered for endovascular treatment options, the FET remains a sustainable platform, especially for younger post-dissection and heritable thoracic aortic disease (HTAD) patients. These patients frequently have undergone previous transsternal operations. A substantial portion require concomitant aortic root or other cardiac procedures. In this manuscript we describe our technique of redo valve spari

Aortic Arch Incision and Closure Technique (AICT) for Proximal Fixation of the Frozen Elephant Trunk.

Background: To describe an aortic arch incision and closure technique (AICT) for proximal fixation of a frozen elephant trunk (FET) and to report early outcomes. Methods: We retrospectively reviewed 15 consecutive patients who underwent distal arch repair with an FET using AICT (mean age 77 ± 7 years; 14 men). Indications were distal arch aneurysm (n = 12), acute Stanford type B dissection (n = 2), and distal arch enlargement after thoracic endovascular aortic repair (n = 1). Under circulatory arrest, an oblique arch aortotomy was created, the FET was deployed antegrade, trimmed, and sutured t

Frozen Elephant Trunk Technique to Minimize Circulatory Arrest and Aortic Cross-Clamp Time.

BACKGROUND: Open repair for aortic arch aneurysms is the gold standard, but prolonged circulatory arrest time and aortic cross-clamp time heighten complication risks. Our team adopted a modified frozen elephant trunk (FET) implantation technique simplifying the complex arch procedure. METHODS: The FET technique, like open and endovascular surgeries, is an evolving procedure in aortic arch repair and complex aortic pathologies. Our FET technique involves bilateral axillary artery cannulation for cardiopulmonary bypass, debranching of arch vessels with antegrade cerebral perfusion prior to circ

Extra-anatomic left subclavian artery bypass patency in frozen elephant trunk surgery.

OBJECTIVE: To explore the advantages and consequences of using an extra-anatomic Dacron bypass in frozen elephant trunk surgery for fast and secure left subclavian artery (LSA) reimplantation. METHODS: Between June 2017 and June 2023, 195 patients were treated using an LSA bypass. All postoperative imaging was reviewed to assess the patency of the bypass grafts. If the LSA bypass was not patent, symptoms of complications and their management were evaluated. Time-to-event analysis was performed to assess bypass patency and time to thrombosis. RESULTS: Out of 195 LSA bypasses, 183 remained pat

Elephant-inspired tapered cable-driven hyper-redundant manipulator: design and performance analysis.

The cable-driven hyper-redundant manipulator (CDHM), distinguished by its high flexibility and adjustable stiffness, is extensively utilized in confined and obstacle-rich environments such as aerospace and nuclear facilities. This paper introduces a novel CDHM inspired by the trunk of elephants, which changes the arm structure from cylindrical to conical. This alteration diminishes the arm's self-weight, reduces the moment arm of gravity, decreases the volume of the end joint, narrows the stroke of the driving cables, and boosts the maximum joint speed of the manipulator. Additionally, this st…

Heat Shock Related Protein Expression in Abdominal Testes of Asian Elephant (Elephas maximus).

The abdominal testes of Asian elephants show normal spermatogenesis. Heat shock in cryptorchid testes elevates heat shock factor (HSF) expression, leading to germ cell apoptosis, while increased heat shock proteins (HSPs) levels provide protection. To investigate how heat shock affects elephant spermatogenic cells, focusing on heat shock-related molecules and the cell death mechanism, immunohistochemistry and TUNEL staining were employed to assess the immunoexpression of several heat shock-related molecules and the status of apoptosis in elephant fibroblasts (EF) induced by heat shock stimulus…

Long-term outcomes of frozen elephant trunk for aortic dissection: a single-center experience.

BACKGROUND: To date a number of papers analysing outcomes of the frozen elephant trunk (FET) in acute aortic dissection has been published. However, there are limited comparative studies on long-term outcomes of FET in acute and chronic aortic dissection. The objective of the study was to analyze the long-term outcomes after FET procedure for aortic dissection (AD). METHODS: Between March 2012 and December 2022, a total of 123 FET had been performed for thoracic aortic disease. Patients with aortic dissection (n = 97) were divided into 2 groups: acute (n = 32, 33%) and chronic aortic dissecti

Breaking the fast: first report of dives and ingestion events in molting southern elephant seals.

Southern elephant seals (SES) experience a 'catastrophic molt', a costly event characterized by the renewal of both hair and epidermis that requires high peripheral vascular circulation. Molting animals are therefore constrained by high metabolic heat loss and are thought to fast and remain on land. To examine the ability of individuals to balance the energetic constraints of molting on land we investigate the stomach temperature and movement patterns of molting female SES. We find that 79% of females swam and 61% ingested water or prey items, despite the cost of cold-water exposure while molt

Addressing the Elephant in the Room: Perceptions and Treatment of Underrepresented in Medicine Physician Assistants/Associate Educators.

On Thursday, June 27, 2023, the US Supreme Court struck down race-conscious admission practices in higher education. While other demographic factors, such as the traditional Health Resources and Services Administration-disadvantaged background indicators, can be considered during the holistic admission process, explicit consideration based on race and/or ethnicity is prohibited. As a result, physician assistant/associate (PA) programs are tasked with developing novel ways to address equity, diversity, and inclusion during the admission and hiring processes. As Drumgold et al note, closing the

Hypoxia blunts angiogenic signaling and upregulates the antioxidant system in elephant seal endothelial cells.

Elephant seals experience extreme hypoxemia during diving bouts. Similar depletions in oxygen availability characterize pathologies including myocardial infarction and ischemic stroke in humans, but seals manage these repeated episodes without injury. However, the real-time assessment of the molecular changes underlying protection against hypoxic injury in seals remains restricted by their at-sea inaccessibility. Hence, we developed a proliferative arterial endothelial cell culture system to assess the molecular response to prolonged hypoxia. Seal and human cells exposed to 1% O 2 for up to 6

Intermediate outcomes following arch reconstruction with frozen elephant trunk, a single centre study.

BACKGROUND: Surgery on the aortic arch and proximal descending thoracic aorta can be lifesaving but is also associated with significant morbidity, ranging from minor infections to severe neurological impairments as well as a substantial risk of mortality. The aim of this study is to clinically assess outcomes, with special regards to neurologic injury, as well as to seek to identify predictors of in-hospital mortality in two patient groups with different underlying aortic pathology, aneurysms and dissections, undergoing arch/descending aortic repair. METHODS: 34 patients (17 aneurysms, 17 dis

Treatment For Infected Frozen Elephant Trunk Prosthesis Caused by Propionibacterium acnes: A Surgical Challenge.

In this article, we present the case of a 47-year-old man who underwent Bentall-Bono procedure and frozen elephant trunk prosthesis implantation due to severe aortic regurgitation and aortic dilatation with a second-time endovascular stent-graft repair in descending aorta. Over eight years, a subacute graft infection by Propionibacterium acnes was developed, culminating in cardiogenic shock secondary to severe aortic regurgitation due to a complete aortic root dehiscence because of multiple aortic pseudoaneurysms. The patient underwent emergency surgery in which the replacement of the graft by

Morphometric Predictors of Aortic Remodeling after Frozen Elephant Trunk Repair of Type A Dissection.

BACKGROUND: We analyzed the predictors of postoperative aortic remodeling after the frozen elephant trunk technique for type A dissection. METHODS: This retrospective study cohort comprised 20 patients who underwent the frozen elephant trunk technique for type A dissection. The incidence of aortic remodeling was evaluated at 4 levels: middle of the frozen elephant trunk, distal end of the frozen elephant trunk, 10th thoracic vertebra, and proximal to the celiac artery. Several parameters of the residual dissected aorta were analyzed at these 4 levels, and the predictors of aortic remodeling w

Kinking of frozen elephant trunk: reality versus myth-a case report and literature reported.

BACKGROUND: Kinking or iatrogenic stenosis in the frozen elephant trunk (FET) is a possible complication, however, there have been few cases reported. CASE DESCRIPTION: A 43-year-old male with acute type A aortic dissection (ATAAD) underwent total arch replacement and FET installation. After weaning from the cardiopulmonary bypass, both femoral pulses were absent. A right axillo-bifemoral bypass using 8 mm graft was performed. Postoperative aortography showed a 100 mmHg-stenosis at the FET and 28 mm stent-graft was inserted to relieve stenosis. Eleven cases of postoperative FET stenosis have

Frozen Elephant Trunk Procedure and Risk for Distal Stent-Graft-Induced New Entries.

The frozen elephant trunk (FET) procedure is known as an effective treatment option for patients with any aortic pathology involving the aortic arch. However, there is growing evidence that many patients often require secondary intended, expected, or unexpected aortic reinterventions during follow-up. In those with underlying aortic dissection pathology, a substantial risk for developing distal stent-graft-induced new entries (dSINEs) has been identified as one cause for secondary aortic reinterventions. dSINE can develop at any time after the FET procedure. Endovascular treatment is generally

Semen characteristics and second successful artificial insemination of Asian elephant (Elephas maximus) in Thailand.

BACKGROUND AND AIM: As the number of wild Asian elephants (Elephas maximus) continues to decline, maintaining healthy populations under human care is vital. Male fertility assessment is essential for understanding the reproductive status, which can help to uncover underlying problems and improve the rate of pregnancy success. The objectives of this study in Asian elephants were as follows: (1) To investigate the semen characteristics; (2) to compare the relative seminal vesicle size and semen characteristics; (3) to compare the semen characteristics between good-motile (>60% progressive motili…

Hybrid Frozen Elephant Trunk for Single-Stage Kommerell Diverticulum and Type A Dissection Repair.

Treatment of Kommerell diverticulum is challenging, often involving multistage repair to achieve successful exclusion of the aneurysmal diverticulum, division of the vascular ring to release compression, and reconstruction of the aberrant subclavian artery to maintain arm perfusion. A well-planned, elective repair remains the optimal scenario; however, emergency presentation can be extremely challenging, particularly in acute aortic dissection. We describe an innovative, single-stage approach using a novel multibranched hybrid arch frozen elephant trunk prosthesis to achieve complete repair of
