Endolymphatic Duct Blockage as a Surgical Treatment Option for Ménière's Disease.

Endolymphatic duct blockage is a relatively new treatment option for Ménière's disease, aiming to reduce vertigo attacks while sparing hearing and equilibrium. After a regular mastoidectomy, the posterior semicircular canal is identified, and Donaldson's line is determined. This is a line through the horizontal semicircular canal, crossing the posterior semicircular canal. The endolymphatic sac is usually found at this site under the posterior semicircular canal. The bone of the endolymphatic sac and the dura are thinned until the sac is skeletonized, after which the endolymphatic duct is iden…

Endolymphatic hydrops induced by noise exposure.

In animal with cochlear deafness induced by firecracker explosion, fibrous degeneration of the endolymphatic sac was observed 4 months after the disappearance of Preyer's reflex. Endolymphatic hydrops induced by noise exposure may develop as the results of degeneration of the endolymphatic sac due to cochlear deafness by acoustic trauma after the lapse of a long period of time.

A Revised Surgical Approach to Induce Endolymphatic Hydrops in the Guinea Pig.

Endolymphatic hydrops is an enlargement of scala media that is most often associated with Meniere's disease, though the pathophysiologic mec … proach to induce experimental endolymphatic hydrops at 30 days post-operatively by obliterating the endolymphatic sac and injuring the endolymphatic duct with a fine pick. The sample size consisted of seven guinea pigs. Functional measurements of hearing were made and temporal bones were subsequently harvested for histologic analysis. The approach had a success rat …

Pathogenesis of experimental endolymphatic hydrops.

The protein content and the d.c. potential of the endolymph differs in the various parts of the endolymphatic space (cochlea, utricle, semicircular canals and endolymphatic sac) as also does the ion composition (chloride, potassium, sodium). 12 months after obliteration of the endolymphatic sac and duct in guinea pigs the d.c. potential falls, whereas, the sodium activity increases. The endolymphatic hydrops is not caused by an increased colloid osmotic pressure. The increased water-binding capacity of the cochlear endolymph is correlated with the increased Na+ activity.

Ménière disease caused by an anomalous vein of the vestibular aqueduct.

The precise cause of Ménière disease remains unclear. Multiple causes have been proposed with most experimental evidence pointing to impaired fluid resorption by the endolymphatic duct and sac as the final common pathway in development of hydrops. We report a unique case of Ménière disease secondary to compression of the endolymphatic duct and sac by an anomalous vein of the vestibular aqueduct. The resulting mechanical obstruction led to excessive accumulation of endolymph with clinical signs of Ménière disease. We review the literature and discuss proposed pathogenesis of disease. Evolution of understanding and clinical practice of Ménière's disease.

Since Prosper Ménière's original description more than 160 years ago, concepts of Ménière's disease (MD) pathophysiology and treatment have … lassic hydrodynamic era to a modern framework that conceptualizes MD as a disorder characterized by endolymphatic sac (ELS) failure, leading to impaired inner-ear homeostasis. Classification of Menière's disease.

To create a rational treatment system for Meniere's disease, it is helpful to divide Meniere's disease into five stages according to the signs, symptoms, pathology, and natural history by recalling what is known for certain and what is probably true about Meniere's disease. It is known that Meniere's disease is attributable to endolymphatic hydrops, caused by a small, malfunctioning endolymphatic sac, abnormally placed since birth, and Meniere's disease is therefore a congenital disorder of the endolymphatic sac. It is probably true that there is often an immune, viral, metabolic, or other ins…

Sensory cell damage in two-phase endolymphatic hydrops: a morphologic evaluation of a new experimental model by low-voltage scanning techniques.

HYPOTHESIS: The aim of this study was to create a more dynamic animal model of Ménière's disease combining multiple causes, such as the role of endocrine factors and endolymphatic sac dysfunction, that may mimic the fluctuant characteristics of Ménière's disease. BACKGROUND: Endolymphatic hydrops remains to be considered a pathologic substrate in the etiology of Ménière's disease. The classic guinea pig model of inducing hydrops by total destruction of the endolymphatic sac is a nonphysiologic rigid model of Ménière's disease. METHODS: The authors developed the two-phase endolymphatic hydrop…

Regulation of endolymphatic fluid volume.

Direct measurements of the dispersal of markers in endolymph have failed to support previously established hypotheses of endolymph homeostas … he cochlea, contributing to the addition of electrolytes and volume to the endolymphatic space. The endolymphatic sac responds to endolymph volume disturbance, showing op posite responses to volume increases and decreases. Although evidence is still limited, the endolymphatic sac appears to act as a "bidirectional overflow" system. While volume disturbances origina …

Histopathology of Endolymphatic subarachnoid shunt surgery for Meniere's disease.

The histopathological findings in the temporal bones of three patients who underwent endolymphatic subarachnoid shunt operation for Meniere's disease are reported. The postoperative follow-up period varied from three to five years. Two patients had a successful operation; the third was unsuccessful. The continuity between th mesothelial lining of the endolymphatic sac and the subarachnoid space was demonstrated in one case; no definite conclusions could be made for the other two cases. Although failure of shunt surgery was presumably due to fibrosis in the shunt area in the reported case, the …

[Surgical treatment of Meniere's disease and its pathophysiological validation].

The authors analyse their own and literature data on surgical treatment of Meniere's disease (MD) as intervention on the endolymphatic sac with assessment of various modifications by long-term results, on sympathetic intraaural and extraaural lesions. Surgical interventions on the endolymphatic sac are pathophysiologically validated. Insufficient knowledge on hydrodynamics in the internal ear labyrinth and the role of endolymphatic hydrops in MD pathogenesis hampers assessment and further development of surgical treatment of the disease. Future lines of the research in Meniere's disease and it…

Pathogenesis of Menière's disease.

While endolymphatic hydrops is a characteristic pathologic feature of Menière's disease, there are exceptions to this rule. There is evidence that hydrops develops as a result of malabsorption of endolymph. This implies dysfunction of the endolymphatic sac and duct, which normally absorb endolymph. In approximately 20% of cases of Menière's disease, a specific pathologic condition in the temporal bone can be associated as a cause of endolymphatic hydrops. Syphilis, fractures of the temporal bone, otosclerosis, and preceding chronic otitis media are some of the more commonly encountered patholo…

[MR imaging of the inner ear; findings of Ménière's syndrome].

Symptoms in Ménière's disease are explained by hydrops of endolymphatic system with recurrent ruptures of the membranous labyrinth. The primary cause of the increased endolymphatic volume appears to be an imbalance between secretion and resorption of endolymph which may be due to an obstruction of the endolymphatic duct and sac, located in the vestibular aqueduct. Non-visualization or narrowing of the vestibular aqueduct of the paper have been demonstrated by conventional tomography and high resolution computed tomography (HR-CT). But the endolymphatic duct and sac can not be obtained by HR-CT

Temporal bone findings in a case of bilateral Menière's disease treated by parenteral streptomycin and endolymphatic shunt.

A patient with bilateral Menière's disease who had progressive hearing loss and intractable vertigo was treated at ages 60 and 62 with parenteral streptomycin to ablate vestibular function, and at age 74 by a left endolymphatic shunt procedure. He was confined to a wheelchair because of ataxia from age 75 until the time of his death at age 81. Both temporal bones show congenitally hypoplastic endolymphatic sacs and severe endolymphatic hydrops consistent with Meniere's disease. There was atrophy of the striae vasculares and loss of cochlear neurons consistent with presbycusis. A loss of hair c…

Surgical treatment of Menière's disease--two techniques compared.

Two techniques for surgical treatment of Menière's disease, endolymphatic sac decompression according to Shambough and decompression on the stapes footplate, i. e., platino decompression according to Martin, were compared. A series of 81 patients were hospitalized for treatment, and 20 surgical procedures were performed: platino decompression in eight cases, and endolymphatic sac decompression in 12. None of the eight patients had relapse of the disease after platino decompression, whereas the endolymphatic sac operation was followed by relapse in 6 of the 12 patients. Considering the postoper…
