Managing technological and environmental risks in Japan.

This paper is primarily concerned with environmental risks arising from utilizing modern technology. Our attention focuses on the interface between policy decisions and regulatory measures. The interface involves not only science and technology but also politics and social values, and perceptions which are deeply rooted in the industrial countries. A case of setting air quality standards and regulatory practices in Japan is studied on the basis of scientific and technological knowledge, economic cost, and subjective judgment of what constitutes an acceptable level of risk or health hazard.

Quantitative risk assessment of environmental hazards.

Quantitative risk assessment may never become a rigorous scientific discipline because of the inherent difficulties in working with highly uncertain and often controversial data and methods, and because the predictions of risk assessment may not be subject to validation in their most important areas of application. However, the potential benefits of having quantitative estimates of risk may make quantitative risk assessment a valuable adjunct to traditional methods for making individual and social decisions about health hazards in the home, workplace, and general environment. Risk assessment,

Manganese: a review of occupational and environmental toxicology.

The following topics are dealt with in this brief review concerning the hygienic and toxicologic aspects of manganese: Occurrence, production, uses; experimental pathology; clinical toxicology; occupational and environmental hygiene aspects of human exposure. The preventive measures outlined: reduction of dust concentration to comply with the established MAC value, in extreme situation even with the use of personal protection; preventive pre-employment examinations to exclude from work all individuals with liver and kidney lesions, blood dyscrasia, organic CNS lesions and progressive lung dise

Environmental validation of the homocystine theory of arteriosclerosis.

It has been proposed that elevated concentrations of homocystine in vascular tissue could cause arterial damage leading to arteriosclerosis. This theory is indirectly supported by research in the area of environmental toxicology, which has revealed that carbon monoxide and carbon disulfide, agents whose prolonged exposure is known to result in the development of arteriosclerotic changes, induced vitamin B6 deficiency states which predictably lead to a homocystinuria-like state. Such information provides strong indirect support of the controversial homocystine theory of arteriosclerosis.

Cancer of the larynx: occupational and environmental associations.

This paper briefly reviews the available evidence for the influence of environmental and occupational factors in the development of cancer of the larynx. Although several occupational exposures have been suggested to play a role in the etiology of this disease, few have been convincingly confirmed. The strong association between smoking and cancer of the larynx, the weaker but still important association between alcohol consumption and cancer of the larynx, and the strong synergism between the two is emphasized. Other accepted or suspected environmental and occupational factors are also discus

Environmentally induced cholinergic urticaria and anaphylaxis.

Although urticaria is generally an uncomfortable condition, it is usually considered to be relatively benign. Recent evidence indicates that numerous environmental stimuli can initiate cholinergic urticaria, and severe systemic manifestations may be associated with the onset of the urticaria. Exercise-induced anaphylaxis is a specific life-threatening reaction that has been documented to occur very unpredictably in susceptible individuals with cholinergic urticaria. The occurrence of severe hypotension, syncope, or laryngeal edema poses specific limitations to optimum performance should it occ

Fetal malformations and environmental influences. A perspective.

In western society, congenital malformations and genetic disorders have assumed much greater importance as causes of early morbidity and mortality, with the waning of infectious diseases. The personal and societal burden arising as a consequence is an important public health problem. Approximately 25-30% of admissions to major childrens' hospitals in the western hemisphere are for children with one of these disorders. About 3-4% of all births are associated with a major congenital malformation, genetic disorder, or mental retardation. For congenital malformations and mental retardation in part

Review of environmental factors affecting hearing.

The major nongenetic causes of sensorineural hearing loss are exposure to noise, aging, ototoxic drugs, viral and bacterial infections, and interactions between these factors. Regarding exposure to continuous noise, the data base from laboratory and field studies indicates that a risk of hearing loss is present when noise levels exceed 75-80 dBA. As noise level, duration and number of exposures increase so does risk. The data base for other forms of noise (intermittent, impact) is not as established. Risk of hearing loss due to impulse noise increases as the peak SPL exceeds 145-155 dB and as

Environmental factors in the etiology of rhabdomyosarcoma in childhood.

In a case-control study of childhood rhabdomyosarcoma (RMS), families of 33 cases and 99 controls were interviewed. A relative risk (RR) of 3.9 was found associated with fathers' (but not mothers') cigarette smoking (P = 0.003). Cases had had fewer immunizations than controls, particularly smallpox vaccination (RR = 0.2; P = 0.001), and conversely had more preventable infections. An RR of 3.2 (P = 0.03) was found associated with exposure to chemicals and and RR of 3.7 (P = 0.004) was found associated with diets that included organ meats. Mothers of cases were more likely to be over age 30 year

Environmental factors in allergic bronchopulmonary aspergillosis.

Many workers have suggested that exposure to an environment rich in Aspergillus spores is aetiologically significant in allergic bronchopulmonary aspergillosis. This hypothesis has been investigated by means of an environmental survey in which eight urban dwelling patients with this disease and five atopic control patients took part. Enquiries were made about contact with known sources of Aspergillus fumigatus and air spora studies were conducted in the home. Findings were similar for both groups. No high spore concentrations were recorded in the homes of either group. Patients with allergic b

Endocrinological analysis of environmental menstrual disorders.

In 139 new student nurses, a change of menstrual cycles after school entry was studied, recording their basal body temperature over more than 4 months. In some of these students, serum FSH, LH, prolactin, estradiol, and progesterone were assayed every day or every other day throughout the second cycle after entry. Forty-six out of 72 students with previously normal menstrual cycles changed the length of their cycles. In most of the cases with a change in menstrual cycle, LH seemed not to be released in adequate amounts judging by the peak of estrogen and ovulation did not occur; the lack of su

Congenital nystagmus--genetic and environmental causes.

A survey of 40 individuals registered with the Canadian National Institute for the Blind (CNIB) as blind from congenital nystagmus revealed that an abnormal single gene was responsible for the disorder in 33 patients. Fifteen of these were due to autosomal recessive conditions while X-linked disorders accounted for another 15 patients. In 3 cases the pedigrees were consistent with both autosomal recessive or X-linked inheritance. A clearly defined environmental origin was present in 1 case while specific genetic or environmental factors were not detected in the remaining six patients. The albi

Environmentally triggered small vessel vasculitis.

Ten randomly selected patients with recurrent non-specific small vessel vasculitis (edema, petechiae, spontaneous bruising, peripheral cyanosis) were studied under rigid environmental control. All cleared their symptoms without medications. All patients had their vasculitis reproduced after direct challenge with numerous individual foods and chemicals. Multiple incitants were found in each patient.

Some current aspects of environmental fluoride.

This review presents a Total Environment evaluation of current inorganic fluoride intake by human populations. Inorganic fluoride is a persistant bioaccumulator, and the ever-increasing use (and release) of fluoride compounds in the environment should be of long-term concern in population sub-groups who are most susceptible, and therefore, most "at risk". One of these sub-groups consists of people with impaired kidney function, including subjects with nephorphatic diabetes. The diabetes factor is of particular relevance, not only because the incidence of diabetes has increased by 6%/yr during
