Commentary: considerations for using the 'Trials within Cohorts' design in a clinical trial of an investigational medicinal product.

BACKGROUND: The 'trials within cohorts' (TwiC) design is a pragmatic approach to randomised trials in which trial participants are randomly selected from an existing cohort. The design has multiple potential benefits, including the option of conducting multiple trials within the same cohort. MAIN TEXT: To date, the TwiC design methodology been used in numerous clinical settings but has never been applied to a clinical trial of an investigational medicinal product (CTIMP). We have recently secured the necessary approvals to undertake the first CTIMP using the TwiC design. In this paper, we des

Maternal health and human rights impacts of Russian drug policy.

This article describes how Russian drug policy defies international ethical standards in patient care and violates the human rights of pregnant people who use drugs. While the CEDAW Committee previously found Russia to be in violation of the Convention on the Elimination of All Forms of Discrimination against Women (CEDAW) by failing to ensure that pregnant people have access to gender-sensitive drug dependence treatment, to date the Committee has refused to address the role of drug criminalization in enabling this human rights violation. This article outlines the gendered impacts of Russia's

Why do so many in academic health care tend to assume that subspecialty surgical training begets higher quality health care than general pediatric surgical training?

BACKGROUND: The author spent all his attending level practice in general hospitals, resulting in early exposure to developing technologies, allowing early adaptation. ILLUSTRATIVE EXAMPLES: The adaptation of balloon dilation of arterial stenoses to treat esophageal strictures in children, and the use of MRI in anorectal malformations. RESULTS: Balloon dilatation of esopageal strictures and MRI assessment of anorectal anomalies became routine care. CHANGES IN ETHICAL LANDSCAPE: Both of these innovations were discussed with the Ethics Committee. Neither were subjected to further ethical overs

Prospective multicentre study evaluating ctDNA as a biomarker of residual disease after chemoradiotherapy for locally advanced head and neck squamous cell carcinoma: NeckTAR-IN protocol.

INTRODUCTION: The first therapeutic assessment of locally advanced (LA) head and neck squamous cell carcinomas (HNSCCs) is often performed 10-12 weeks after the end of chemoradiotherapy as a result of the delayed action of radiotherapy. Diagnostic uncertainty between persistent disease and treatment-related changes (oedema, necrosis) can delay confirmation of residual cancer. According to data in the literature, a correlation exists between the detection of circulating tumour DNA (ctDNA) at the end of chemoradiotherapy treatment and residual disease. However, additional data are required befor

Health-resilient frameworks and their impact on routine immunisation and Maternal, Neonatal and Child Health services during pandemics in sub-Saharan Africa: a scoping review protocol.

INTRODUCTION: Despite significant global advancements, the past decade has seen stagnation in Maternal, Neonatal and Child Health (MNCH) service coverage and a concerning high under-five and maternal mortality rates, which have been worsened by COVID-19 pandemic-related disruptions, particularly in sub-Saharan Africa. This scoping review protocol will support the comprehensive mapping, evaluation and assessment of the application, impact, effectiveness and adaptability of health-resilient frameworks in maintaining these services during pandemics, while also identifying gaps in the literature a

Safety of different concentrations of glycerine enema for meconium evacuation in preterm infants: study protocol for a randomised controlled trial.

INTRODUCTION: Various approaches are employed to expedite the passage of meconium in preterm infants within the neonatal intensive care unit (NICU), with glycerine enemas being the most frequently used. Due to the potential risk of high osmolality-induced harm to the intestinal mucosa, diluted glycerine enema solutions are commonly used in clinical practice. The challenge lies in the current lack of knowledge regarding the safest and most effective concentration of glycerine enema. This research aims to ascertain the safety of different concentrations of glycerine enema solution in preterm inf

The relationship between different bispectral index and the occurrence of dreams in elective surgery under general anesthesia: protocol for a randomized controlled clinical trial.

INTRODUCTION: Dreaming reported after anesthesia remains a poorly understood phenomenon. At present, there is a hypothesis that dreaming occurs intraoperatively and is related to light or inadequate anesthesia; thus, in order to further verify the hypothesis, we choose elective surgery under general anesthesia to observe whether the generation of dreams is related to the dose of general anesthetics maintenance. METHODS AND ANALYSIS: This randomized, double-blind controlled trial to observe whether the generation of dreams is related to the dose of general anesthetics maintenance in the electi

Informed consent and biobanks: a population-based study of attitudes towards tissue donation for genetic research.

AIMS: The procurement and usage of tissue samples has begun receiving increasing legal and ethical attention. The authors' aim was to develop an empirically based understanding of public attitudes to the use of tissue for research. METHOD: A questionnaire was distributed to a randomized sample (n=1,000) of the population in the age group 18-85 in Västerbotten County, Sweden. RESULTS: The response rate was approximately 60%. There was a general acceptance of genetic research based on biobank material (71%) but, though it is often a prerequisite for tissue-based research, a majority (62%) woul

Vaccine Preventable Disease Seroprevalence in a Nationwide Assessment of Timor-Leste (VASINA-TL): study protocol for a population-representative cross-sectional serosurvey.

INTRODUCTION: Historic disruption in health infrastructure combined with data from a recent vaccine coverage survey suggests there are likely significant immunity gaps to vaccine preventable diseases and high risk of outbreaks in Timor-Leste. Community-based serological surveillance is an important tool to augment understanding of population-level immunity achieved through vaccine coverage and/or derived from prior infection. METHODS AND ANALYSIS: This national population-representative serosurvey will take a three-stage cluster sample and aims to include 5600 individuals above 1 year of age.

Protocol for process evaluation of CIVIC randomised controlled trial: Community-based InterVentions to prevent serIous Complications following spinal cord injury in Bangladesh.

INTRODUCTION: People with spinal cord injuries in low-income and middle-income countries are highly vulnerable to life-threatening complications in the period immediately after discharge from hospital. We are conducting a randomised controlled trial in Bangladesh to determine whether all-cause mortality at 2 years can be reduced if health professionals regularly ring and visit participants in their homes following discharge. We will conduct a process evaluation alongside the trial to explain the trial results and determine the feasibility of scaling this intervention up in low-income and middl
