BACKGROUND: Heart failure (HF) is a chronic condition characterized by significant functional limitations, with exercise intolerance as a major determinant of reduced quality of life. Supervised aerobic exercise is a core intervention in cardiac rehabilitation, yet adherence and tolerance may be hindered by physical and motivational factors. Immersive technologies such as virtual reality (VR) have shown potential to increase engagement and positive perceptions during exercise. This study aims to evaluate the influence of VR on tolerance to cycle-ergometer aerobic exercise in hospitalized patie

BACKGROUND: Lung clearance index (LCI2.5 ) is a marker of overall lung ventilation inhomogeneity and has proven to be able to detect early peripheral damage in subjects with cystic fibrosis (CF), with greater sensitivity than conventional spirometry. Combining its sensitivity with the output of an incremental exercise testing, we hypothesized that any sign of ventilation inhomogeneity in subjects without severe airflow obstruction and with a normal exercise tolerance could be relevant for the CF team, tracking early lung disease and potential exercise limiting factors.
METHODS: Patients with

Multiple compensatory mechanisms operate to preserve exercise tolerance in patients with left ventricular failure. Exercise capacity of most patients with chronic heart failure is limited by dyspnea or fatigue, or both. Maximal stress testing with direct assessment of peak O2 uptake is an essential measurement in planning exercise conditioning programs, which are now attracting patients with chronic heart failure. The biochemical and histologic patterns of skeletal muscle changes seen in chronic heart failure patients are consistent with the effects of long-term exercise deconditioning in norm

Exercise testing is not commonly used in canine medicine because of several limitations. The aim of this study was to investigate the suitability of a treadmill test to measure the exercise capacity of untrained canine cardiac patients and to measure some biological parameters that might reflect the tolerance of dogs with heart failure to submaximal exercise. The exercise capacity of seven dogs with naturally occurring heart failure was evaluated before the institution of cardiac medication and 7 days after the beginning of the study. An additional re-examination was requested after 28 days. T

PURPOSE: Expiratory flow limitation (EFL) and dynamic hyperinflation (DH) may significantly impact exercise capacity in patients with heart disease. Although commonly linked to lung diseases, recent evidence highlights their role in cardiovascular disease (CVD), contributing to exercise intolerance. This systematic review examines studies from the past decade on pulmonary function during exercise, measured using inspiratory capacity (IC) maneuvers, in patients with CVD, emphasizing prevalence and clinical significance.
REVIEW METHODS: A systematic literature search in PubMed, Scopus, and Coch

Evidence of the effect of exercise therapy in patients who have undergone total thoracoscopic ablation is lacking. This study aimed to evaluate the effects of eight weeks exercise-based cardiac rehabilitation on cardiopulmonary fitness and adherence to exercise in patients who underwent total thoracoscopic ablation and followed a regimen of exercise therapy. Twenty-four patients were involved in the study and were divided into two groups. The exercise group underwent exercise therapy, which included aerobic and resistance exercises, twice a week as part of an eight weeks hospital-based outpati

Cardiopulmonary exercise testing (CPET) has long been used as diagnostic tool for cardiac diseases. During recent years CPET has been proven to be additionally useful for 1) distinguishing between normal and abnormal responses to exercise; 2) determining peak oxygen uptake and level of disability; 3) identifying factors contributing to dyspnoea and exercise limitation; 4) differentiating between ventilatory (respiratory mechanics and pulmonary gas exchange), cardiovascular, metabolic and peripheral muscle causes of exercise intolerance; 5) identifying anomalies of ventilatory (respiratory mech

There have been few studies examining the response of persons with neuromuscular disease and postpolio syndrome to cardiopulmonary testing and aerobic exercise training. In persons with neuromuscular disease that directly involves the cardiac and respiratory systems, deficits in performance may be primarily due to these limitations, along with loss of functional muscle tissue from the disease process. In the more slowly progressive disorders, deconditioning may play an important role in limiting aerobic exercise performance and may be amenable to training. Recommendations are provided for futu

In chronic heart failure, minute ventilation (V'E) for a given carbon dioxide production (V'CO2 ) might be abnormally high during exercise due to increased dead space ventilation, lung stiffness, chemo- and metaboreflex sensitivity, early metabolic acidosis and abnormal pulmonary haemodynamics. The V'E versus V'CO2 relationship, analysed either as ratio or as slope, enables us to evaluate the causes and entity of the V'E/perfusion mismatch. Moreover, the V'E axis intercept, i.e. when V'CO2 is extrapolated to 0, embeds information on exercise-induced dead space changes, while the analysis of en

We studied the physiologic adaptation of patients with mitochondrial myopathies to aerobic training. Ten patients underwent individually supervised, moderate-intensity aerobic training on a treadmill for 8 weeks. Biochemical and functional measures improved with training. Estimated aerobic capacity increased by 30%. Blood lactate concentrations at rest and after exercise decreased by 30%. Muscle phosphorus magnetic resonance spectroscopy measurements of adenosine diphosphate recovery after exercise improved by more than 60%. Fatigue and tolerance to daily activities also improved. Although the

BACKGROUND: Patients with COPD experience air flow limitation, reduced expiratory capacity, and dynamic hyperinflation (DH), which causes dyspnea and decreased exercise tolerance. Expiratory positive airway pressure (EPAP) is a potential intervention that can reduce DH and improve respiratory muscle efficiency, but its impact on exercise tolerance remains unclear. The aim was to evaluate the acute effects of EPAP during exercise on the respiratory pattern, chest wall volumes, thoracoabdominal asynchrony (TAA), and DH in individuals with COPD using optoelectronic plethysmography.

The effect of different exercise intensities on the magnitude of post-exercise hypotension has not been rigorously clarified with respect to the metabolic thresholds that partition discrete exercise intensity domains (i.e., critical power and the gas exchange threshold (GET)). We hypothesized that the magnitude of post-exercise hypotension would be greater following isocaloric exercise performed above versus below critical power. Twelve non-hypertensive men completed a ramp incremental exercise test to determine maximal oxygen uptake and the GET, followed by five exhaustive constant load trial

Topical timolol given for the treatment or chronic simple glaucoma may cause unrecognised bronchospasm among elderly people. We recruited 80 patients aged over 60 years, who were without a history of airways disease and already used timolol, into a randomised crossover study comparing the effects on spirometry and exercise tolerance of changing to betaxolol or dipivefrine therapy. Results showed an increase of 13% and 8% in mean peak flow rate and forced expiratory volume in 1 s (FEV1), respectively, when using betaxolol; and of 14% and 11% when using dipivefrine. There was also improved exerc

Sex differences in the metabolic and anti-inflammatory effects of exercise have been reported, but whether males and females exhibit a differential response to exercise in a setting of cardiometabolic disease is unknown. The objective of this study was to investigate the glucose handling, adipose and cardiac effects of voluntary exercise in male and female mice in a cardiometabolic disease setting induced by a high-fat diet (HFD). The extent of exercise tolerance improvement was similar between HFD male and HFD female mice with running wheel access, despite greater daily running distances in f
