We present the case of a child impaled in the face by a meat thermometer who subsequently suffered a significant complication due to the administration of hydrogen peroxide to the wound. The soft tissues of the face rapidly expanded and blanched, the child experienced mental status changes, and imaging revealed massive subcutaneous emphysema, pneumomediastinum, and pneumo-orbit. Herein we review the literature on this rare complication and provide photodocumentation in the hopes that other practitioners, patients, and parents avoid administering hydrogen peroxide into or near any penetrating i

BACKGROUND: Ballistic facial injuries are rare, with most trauma centers reporting 1-20 cases annually. These patients present significant management challenges to reconstructive surgeons, not only due to their rarity but also due to the complex decision-making process that is involved. The aim of this study was to review our experience with the application of craniofacial microsurgery in management of facial gunshot wounds.
METHODS: A retrospective review of a single-surgeon experience at a level I trauma center from 2011 to 2020 for patients sustaining self-inflicted gunshot wounds to the f…

The objective of this study was to determine the prevalence of temporary and permanent facial nerve injury in total temporomandibular joint (TMJ) replacement surgery and to identify potential predictive risk factors. We made a retrospective review of case notes and opportunistic review of patients treated in a single tertiary referral unit by a single surgeon. For each patient a number of potential risk factors were determined, and the presence or absence of nerve injury recorded at 2 week follow up and a minimum of 12 months follow up. At 2-week review, facial nerve weakness was noted in 38/1

BACKGROUND: Local anesthetic toxicity has been well-documented to cause neuronal injury, death, and dysfunction, particularly in a susceptible nerve.
OBJECTIVE: To determine whether select local anesthetics affect neuron survival and/or functional recovery of an injured nerve.
METHODS: This report describes 6 separate experiments that test immediate or delayed application of local anesthetics in 3 nerve injury models. Adult C57/black6 male mice underwent a facial nerve sham, transection, or crush injury. Local anesthetic or saline was applied to the facial nerve at the time of injury (immedi

Facial nerve injury following endovascular treatment of cavernous dural arteriovenous fistulae may be caused by ischemia from inadvertent embolisation of vasa nervorum, shearing or compressive forces. We report a case of unilateral facial nerve palsy as a complication of cavernous dural arteriovenous fistula embolisation.

OBJECTIVE: To study the effect of electrical stimulation on accelerating facial nerve functional recovery from a crush injury in the rat model.
STUDY DESIGN: Experimental.
METHOD: The main trunk of the right facial nerve was crushed just distal to the stylomastoid foramen, causing right-sided facial paralysis in 17 Sprague-Dawley rats. An electrode apparatus was implanted in all rats. Nine rats underwent electrical stimulation and eight were sham stimulated until complete facial nerve recovery. Facial nerve function was assessed daily by grading eyeblink reflex, vibrissae orientation, and vi

OBJECTIVES/HYPOTHESIS: To assess whether the use of continuous intraoperative facial nerve monitoring correlates to postoperative facial nerve injury during parotidectomy.
STUDY DESIGN: A retrospective analysis.
METHODS: Forty-five consecutive parotidectomies were performed using an electromyograph (EMG)-based intraoperative facial nerve monitor. Of those, 37 had complete data for analysis. Intraoperative findings and final interpretation of the EMGs were analyzed by a senior neurologist and neurophysiologist. All patients were analyzed, including those with preoperative weakness and facial

Unilateral chronic constriction injury to the infraorbital nerve (IoN-CCI) induces an increase in face-grooming behavior that is not part of normal body grooming (i.e., "isolated face grooming"). Despite the validity of isolated face grooming as a measure of spontaneous neuropathic pain, variation between rats in postoperative face grooming has limited its usefulness. We examined whether performing bilateral rather than unilateral IoN surgery could induce a more stable face-grooming behavior. It was found that bilaterally ligated rats performed a significantly greater amount of isolated face g

BACKGROUND: Cutaneous surgery in the temporal region of the forehead can lead to injury to the superficial temporal branch of the facial nerve. A flattened forehead and with ipsilateral forehead paralysis can occur with damage to this nerve.
METHODS: A case is presented of transient forehead paralysis resulting from Mohs' micrographic surgery with reconstruction of the defect. The paralysis resolved over a period of fifteen months.
RESULTS: The anatomy of the nerve makes it susceptible to injury during cutaneous surgery. The area of danger is the area superior to the zygomatic arch and later

The determination of long-term prognosis is one of the most difficult tasks in the management of persons with brain injury (BI). Initial Glasgow Score, duration of coma, brain stem reflexes, and pupillary responses are all clinical findings that have been related to outcome in BI. We describe another easily observable clinical finding, persistent facial myoclonus (PFM), which may provide additional information on prognosis after severe BI. We retrospectively reviewed 295 consecutive admissions to an inpatient brain injury rehabilitation program. Data extracted included mechanism of injury, pat

It is a common experience that the facial nerve is affected in closed head injury. 781 patients with head injury were treated at Kobe Central Municipal Hospital over a period of 5 years from January 1977 to April 1982, and facial palsy occurred in 25 cases (3.2%). The male to female ratio was 22: 3 and age ranged from 9 to 78 (average 36). The patients were studied, using radiological, topognostic and electrodiagnostic methods. Facial nerve decompression was done in 5 cases and 20 cases were treated conservatively. Results were as follows. The overall rate of complete recovery was 64% (16 of 2

BACKGROUND: Posttraumatic facial paralysis is a disabling condition. Current surgical management by faciofacial nerve suture provides limited recovery. To improve the outcome, the authors evaluated an add-on strategy based on a syngeneic transplantation of nasal olfactory stem cells in a rat model of facial nerve injury. The main readouts of the study were the recording of whisking function and buccal synkinesis.
METHODS: Sixty rats were allocated to three groups. Animals with a 2-mm facial nerve loss were repaired with a femoral vein, filled or not with olfactory stem cells. These two groups

Muscles innervated by the facial nerve show differential sensitivities to muscle relaxants than muscles innervated by somatic nerves. The evoked electromyography (EEMG) response is also proportionally reduced after facial nerve injury. This forms the theoretical basis for proper utilization of muscle relaxants to balance EEMG monitoring and immobility under general anesthesia. Tracheoesophageal fistula (TEF) is a rare but devastating complication of mechanical ventilation and tracheostomy, classically described as a late sequela of prolonged airway instrumentation. Patients with severe burn injury and inhalation injury represent a uniquely vulnerable population due to airway mucosal ischemia, thermal injury, and impaired tissue healing. We report a case of fulminant posterior tracheal wall failure with early TEF formation occurring within days of tracheostomy in a 75-year-old patient with extensive full-thickness facial burns following thermal inhalation injury from
