BACKGROUND: The study was done in three villages in Nueva Ecija, Philippines. It surveys farmers' belief system and pesticide practices relative to health and safety.
METHODS: Initially it used a simulated market study on willingness to pay for personal protective equipment in the form of gloves and masks. Then a combination of semi-structured, formal, informal, and key-informant interviews, as well as focus groups, and field observations was done intermittently in a span of approximately 12 years.

Hairy cell leukaemia (HCL) is a rare non-Hodgkin lymphoma of unknown aetiology most frequently diagnosed in males. In earlier epidemiological studies of HCL, exposures to benzene and chemicals appear to be risk factors. Since 1984, 48 patients (36 men and 12 women) with HCL has been treated in Uppsala. A cross sectional survey on the prevalence of acute pyrethroid poisoning in cotton farmers was conducted in 1987 and 1988. A total of 3113 pyrethroid spraymen (2230 men (71.6%) and 883 women (28.4%] were interviewed after spraying and followed up for 72 hours. To understand better the effects of noise levels associated with dairy farming, we randomly selected 49 full-time dairy farmers from an established cohort. Medical and occupational histories were taken and standard audiometric testing was done. Two cases of hypersensitivity pneumonitis in grain workers are described. Both cases had evidence of sensitization to the gram-negative bacteria Erwinia herbicola as judged by skin tests, detection of serum precipitins, and inhalation challenge. Tinea barbae is a rare zoophilic dermatophyte infection of the beard area, commonly mimicking other dermatological or infectious conditions. We report a young man with progressive erythematous, nodular neck and wrist lesions, unresponsive to antimicrobials. Extensive investigation, including fungal PCR, identified Trichophyton species, leading to successful treatment with systemic antifungals. This case highlights the importance of considering atypical tinea barbae in patients with occupational animal exposure and persistent cutaneous lesions unresponsive to standard antibacterial treatments.

Hypersensitivity pneumonitis is an immunologic reaction to an inhaled antigen, with a wide spectrum of clinical presentations. The most common manifestations are fever, cough, and dyspnea. We describe a case of hypersensitivity pneumonitis with marked alveolar lymphocytosis; the patient presented with respiratory failure and shock requiring mechanical ventilation and vasopressive agents. We hypothesized that immune mediators implicated in the pathogenesis of hypersensitivity pneumonitis were responsible for the transient shock observed in this patient.

An exploratory strategy was used to investigate why 55% of patients with farmer's lung (FL) disease quit farming. Three groups were recruited: 47 patients with FL disease who quit farming because of the disease (FLq), 76 patients with FL disease who continued farming (FLc), and 123 control farmers without a history of FL disease.
