Necrotizing fasciitis: a common problem in Darwin.

Necrotizing fasciitis is considered a rare condition, with an incidence rate of 1 in 100,000 persons on average in the worldwide arena. However, at the Royal Darwin Hospital, an average of 10 cases per year with this condition are managed, which appears disproportionately high considering that the population of Darwin is 89,000. A case is presented in this report of a sports tourist to Darwin to indicate the importance of accurate diagnosis and treatment, which are essential to reduce the high mortality (approximately 40%) associated with necrotizing fasciitis. However, early detection relies

Caloric requirements in patients with necrotizing fasciitis.

Patients with necrotizing fasciitis (NF) and other soft tissue infections are often treated in burn centers due to the extent of wound care and surgical intervention needed. Sepsis and surgery increase metabolic needs and may limit oral intake and necessitate enteral (TEN) or parenteral (TPN) nutrition. We reviewed the records of patients admitted with necrotizing fasciitis or surgical soft tissue infections from January 1993 to June 1998 who had indirect calorimetry (IC) measurements performed. Records were also reviewed for surgical/medical management and nutritional intervention. Twenty-six

The hospital costs of treating necrotizing fasciitis.

BACKGROUND: Management of necrotizing fasciitis places significant demands upon hospital and medical resources. A successful management usually requires extensive surgical intervention and an adjunct hyperbaric oxygen treatment. The cost impact on the health care system has not been well characterized. We have, therefore, analysed the cost of treating this disease at an Australian tertiary referral hospital with extensive case experience and well-developed financial costing systems and have compared this with the current casemix-based government funding arrangements applying in Victoria, Austr

Hypocalcemia: a sign of severity of necrotizing fasciitis.

The growth disturbance of the superior end of the femur which is related to a superior lateral epiphysiodesis of the femoral neck is known as caput valgum (C.V.). Most often, it appears after the treatment of a congenital dislocation of the hip (C.D.H.) which can also produce many other growth disorders of the hip. Fifteen children (seventeen hips) presenting similar evolution and morphological abnormalities have been reviewed. We performed nine surgical procedures most of the time for painful hips because of excentration of the femoral head. When the bone maturity was acquired all hips except

[Varicella associated necrotizing fasciitis in a child].

We present a rare case of necrotizing fasciitis in an 5-year-old infant with preexisting varicella infection. Initially, the patient showed acute pain and swelling of the left thorax with high inflammatory parameters and varicella-specific skin affections. MRT presented diffuse soft tissue swelling of the left chest consistent with necrotizing fasciitis. After aggressive surgical debridement of necrotic tissue and extirpation of axillary lymph nodes, the clinical course improved under appropriate parenteral antimicrobial therapy using penicillin and clindamycin. In conclusion, necrotizing fasc

Management of a patient with postoperative necrotizing fasciitis.

This case study highlights the care of a diabetic woman who had previously undergone elective surgery for a hernia repair, and who later presented with necrotizing fasciitis. The need for a flexible approach to dressing choice is emphasized in terms of patient comfort and ease of management in the community.

Necrotizing fasciitis of abdominal wall in AIDS.

Necrotizing fasciitis (NF) is an uncommon but potentially lethal soft-tissue infection. Mortality rate is high and has not changed since it was first described by Meleny. Although immunodeficiency is a risk factor for NF, there is only one reported case of NF in AIDS involving the cervical region. We report the first case of necrotizing fasciitis of the abdominal wall in an AIDS patient.

[Surgical concepts and results in necrotizing fasciitis].

INTRODUCTION: Necrotizing fasciitis (NF) is a rapidly progressive soft tissue infection involving primarily the superficial fascia and subcutaneous tissue. The disease is caused by Streptococcus pyogenes or synergistic infection of anaerobic and facultative anaerobic bacteria. Further characteristics are severe, intolerable pain and a mortality of 30-50%. PATIENTS AND METHODS: From January 1996 to January 2000 12 patients underwent treatment for NF. The patients' charts were investigated retrospectively. RESULTS: In 7 patients the bacterial cultures showed a mixed, polymicrobial infection an

Necrotizing fasciitis of the pharynx following adenotonsillectomy.

Necrotizing fasciitis is a rare clinical entity in the head and neck region. We report a case of necrotizing fasciitis following adenotonsillectomy in a previously healthy 2-year-old girl. The child presented in a septic state with impending airway compromise. Computed tomography (CT) showed massive soft tissue widening with air in the retropharyngeal, parapharyngeal and retromandibular spaces. Intraoperative exploration showed necrosis of the posterior pharyngeal wall from the skull base to the cricoid, with extension into the parapharyngeal and retropharyngeal spaces. Cultures from the debri

Streptococcal necrotizing fasciitis of the eyelids and orbit.

Streptococcal necrotizing fasciitis in previously healthy persons can develop quickly and may be difficult to diagnose. In a number of cases, this potentially fatal condition has been seen originally in the eyelid and orbit. Intensive antibiotic therapy and prompt debridement are the usual course of treatment.

Clinical and microbiological features of necrotizing fasciitis.

The microbiological and clinical characteristics of 83 patients with necrotizing fasciitis (NF) treated over a period of 17 years are presented. Bacterial growth was noted in 81 of 83 (98%) of specimens from patients with NF. Aerobic or facultative bacteria only were recovered in 8 (10%) specimens, anaerobic bacteria only were recovered in 18 (22%) specimens, and mixed-aerobic-anaerobic floras were recovered in 55 (68%) specimens. In total, there were 375 isolates, 105 aerobic or facultative bacteria and 270 anaerobic bacteria, for an average of 4.6 isolates per specimen. The recovery of certa

Implications of risk factors in necrotizing fasciitis.

The following risk factors, previously associated with necrotizing fasciitis, were identified in 25 consecutive patients: diabetes mellitus, intravenous drug abuse, age greater than 50, hypertension, and malnutrition/obesity. Additional data recorded included the duration of illness to the time of the first operative procedure, the type of procedure performed, the anatomic location of the infection, the etiology, culture reports, and leukocyte counts. The goal of this study was to determine whether the number of risk factors present in a patient was predictive of mortality. Six patients (24%)

[Necrotizing fasciitis caused by group A streptococci].

The number of serious infections from group A streptococci has increased, and there have simultaneously been several cases of necrotizing fasciitis. This is an uncommon, but well-defined entity characterized by extensive necrosis of subcutaneous tissue and fascia, combined with signs of septicaemia. It is often accompanied by multi-organ failure. The mainstay of management is early diagnosis and prompt surgical intervention. It is mandatory that a surgeon, an infectious disease specialist and an anaesthesiologist collaborate on the therapy as a whole.

Necrotizing fasciitis in a renal transplant patient.

We have described a 28-year-old diabetic woman who had necrotizing fasciitis of the perineum three years after receiving a living related renal transplant. The diagnosis of necrotizing fasciitis was made early and she was referred to a tertiary care center where she received radical perineal debridement and aggressive medical and surgical follow-up. Necrotizing fasciitis in a transplant patient is rare; review of the literature shows few cases and no survivors. Our patient has returned to a normal life despite continuation of all immunosuppressive therapy throughout the entire hospital course.
