Treatment strategies for intramedullary nailing of femoral shaft fractures.

Intramedullary nailing has become the gold standard to treat femoral shaft fractures. It is unknown which nailing technique orthopedic surgeons prefer. The goal of this study was to determine current techniques and perioperative complications of intramedullary nailing of diaphyseal femoral fractures. Fifty-one institutions in 26 countries participated in an international survey to assess detailed descriptions of preferred operative strategies and perioperative complications. Altogether, 517 cases of diaphyseal femoral fractures were collected. The Internet-based survey incorporated information…

Treatment of subtrochanteric femoral fractures using Selfdynamisable internal fixator.

BACKGROUND: Surgical treatment is the preferred method for treating subtrochanteric femoral fractures and the variety of extramedullary and intramedullary implants continues to evolve. The purpose of our study was to retrospectively evaluate the clinical and radiological results of subtrochanteric fractures that are treated with the Selfdynamisable internal fixator. METHODS: From January 2000 to January 2004, we treated 49 consecutive patients who had subtrochanteric fractures. According to the AO classification, 8 (16.3%) fractures were type 32-A, 16 (32.7%) were type 32-B and 25 (51%) fract…

Management, complications and clinical results of femoral head fractures.

A systematic review of the literature was conducted to investigate data regarding femoral head fractures, particularly focusing on their management, complications and clinical results. Twenty-nine eligible articles, meeting prespecified inclusion criteria, reported on 453 femoral head fractures in 450 patients (mean age of 38.9 years with a mean follow-up of 55.6 months). 84.3% of patients had been victims of an automobile accident. The most widespread classification scheme used was that of Pipkin (65.4% of cases) whereas clinical results were evaluated mainly according to Thompson-Epstein cri

Prevention of complications after treatment of proximal femoral fractures.

Two factors are primarily responsible for complications after treatment of proximal femoral fractures. First, the strong deforming forces across the hip joint and proximal femur can make fracture reduction difficult. Second, the placement of the implant affects fracture healing and outcome more dramatically than in other areas of the body. In subtrochanteric fractures, the use of appropriate reduction and stabilization techniques can prevent varus malreduction and subsequent failure of the fixation device. In intertrochanteric fractures, lag screw cutout can be prevented by correct implant pos…

Salvage procedures and implant selection for periprosthetic femoral fractures.

Periprosthetic femoral fractures with severe bone loss are challenging to treat. There are various treatment options, depending on the severity of the bone loss, age and activity of the patient and experience of the surgeon. This review focuses on the treatment of these Vancouver type B3 fractures with long-stem implants, proximal femoral replacements, allograft-prosthesis composites and cortical strut grafting.

[New minimally invasive methods of stabilizing distal femoral fractures].

New, minimally invasive methods of stabilizing distal femur fractures offer the advantage of superior bone healing rates and lower infection rates despite of a decreased necessity of bone grafts. In comparison to conventional plating, the employed indirect reduction method does not lead to an increase in postoperative malalignments. The Less Invasive Stabilizing System (LISS) and the Distal Femur Nail (DFN) are introduced as examples of these new methods of stabilization. Although bone biology is widely respected by these new implants and operation techniques, distal femur fractures remain a s

Second-generation intramedullary supracondylar nail for distal femoral fractures.

The objective of this study was to review the use of intramedullary supracondylar (IMSC) nails for distal femoral fractures. We reviewed 24 fractures treated with second-generation IMSC nails. The fractures consisted of 18 type A1, one type A2, two type C1, one type C2, and two type C3 fractures. The relationships between clinical results and fracture type, approaches, and patient age were retrospectively reviewed. All fractures healed clinically and radiographically. Twenty-one patients maintained gait performance equivalent to that before injury. Average operating time was 108 min +/- 43 min…

Outcomes of external fixation of pediatric femoral shaft fractures.

Thirty-seven femoral shaft fractures, in 33 patients, were treated with unilateral external fixation after reduction from 1992 through 1998. Ten girls and 23 boys ranged in age from 4 to 14 years. Thirteen children had multiple injuries, whereas 20 children had isolated fractures. Average follow-up was 3 years, 9 months, with only five children lost to follow-up. The average duration in fixator was 107 days. Thirty-six of 37 fractures healed, and there was one delayed union. There was minimal angulation, and limb-length inequality was generally <1 cm; 72.7% had pin-tract infections. Eight (21.

[Classification, therapy and complications of pediatric femoral neck fractures].

Femoral neck fractures in children are rare injuries (less than 1 percent). The fracture pattern consists of transepiphyseal, transcervical, cervicotrochanteric and intertrochanteric fractures. Main problems with this type of injury are avascular necrosis, varus deformity, nonunion and premature epiphyseal closure. Varus and nonunion are affected by the treatment, vascular compromise with necrosis or growth problems are not related to therapy and hardly to control. Primary dislocated fractures are emergencies and need immediate operative management. Stabilization is achieved with K-wires or ca

Femoral neck fractures. 121 cases treated by Knowles pinning.

The femoral neck fracture remains one of the unsolved fractures. It is a fracture with a high incidence of nonunion and avascular necrosis. One hundred twenty-one mostly young adult patients with femoral neck fractures were treated by Knowles pins internal fixation. Patients were observed for an average of 32 months following surgery. Union occurred in all Garden Stage I and II undisplaced fractures, and the incidence of nonunion was 14% in Garden Stage III and IV displaced fractures. The incidence of avascular necrosis was 5.9% in undisplaced fractures and 34.5% in displaced fractures. There

[Epidemiology of proximal femoral fractures in La Plata, Argentina].

The epidemiology of proximal femur fractures (FXf) was studied in the city of La Plata, Argentina. At the time of the study the total population was 288,000 inhabitants and 73,900 (30,700 men and 43,200 women) were 50 years of age and over (Table 1). From May 1, 1989 to April 30, 1990 all the patients admitted in local hospitals with FXf were personally interviewed by one of the authors. Patients living outside the city of La Plata or fractures due to metastatic or Paget disease were excluded. During the period of the study 195 patients with FXf (164 women and 31 men) were registered. The medi

External fixation of femoral fractures: experience with 15 cases.

We report our experience with external fixation in 15 femoral fractures in 14 patients. Follow-up times ranged from 4 to 56 months. Thirteen patients had multiple injuries. All but two fractures healed. In one patient with a supracondylar fracture, a knee arthrodesis had to be performed. One septic osteomyelitis of the femur led to amputation. The range of active knee joint motion was greater than or equal to 90 degree in 13 knees. Only six knees, however, regained a flexion of greater than or equal to 120 degrees. Pin tract infection occurred in one case and resolved uneventfully after revisi…

Intramedullary nailing of open fractures of the femoral shaft.

The cases of eighty-six patients in whom eighty-nine open fractures of the femoral shaft had been treated by intramedullary nailing with reaming were retrospectively reviewed. Twenty-seven fractures were classified as grade-I open fractures; sixteen, as grade-II open fractures; and forty-six, as grade-III open fractures. Immediate intramedullary nailing was done for fifty-six fractures, and delayed stabilization (five to seven days after delayed closure of the wound) was done for thirty-three fractures. A prerequisite for immediate intramedullary nailing was that irrigation and debridement of

Flexible intramedullary nails for ipsilateral femoral and tibial fractures.

Between June 1981 and August 1983, six patients were treated with flexible intramedullary nails for both femoral and tibial fractures in six floating knees. All six patients were males averaging 26 years of age and all six were involved in motor vehicle accidents. Concomitant associated injuries were common. Two femoral and five tibial fractures were open. All fractures were stabilized within 24 hours of injury by closed intramedullary nailing with Ender nails. At final followup, there was one femoral and one tibial nonunion in the same patient. For the remaining patients, femoral union averag

Prediction of overgrowth in femoral shaft fractures in children.

A prospective study of femoral shaft fractures in children under the age of 14 years has been carried out in 71 patients who were reviewed both clinically and radiographically at yearly intervals. The processing of data has been carried out by univariate and multivariate analysis. Femoral overgrowth (F.O.) averaged 8.63 mm (range 1-21 mm), and was found to be influenced by age (greater overgrowth in children between 3 and 9 years), the initial displacement of the fracture (greater overgrowth in severely displaced fractures), and overriding of the fragments at the time of healing (the greater t

Management of femoral fractures and pseudofractures in adult hypophosphatasia.

Ten pseudofractures and six complete fractures occurred in five patients with adult hypophosphatasia. The four pseudofractures that were treated non-operatively progressed to complete fracture. The six pseudofractures and six complete fractures that were treated by internal fixation united. New fractures occurred in four patients after the removal of the internal fixation. We recommend that complete fractures, symptomatic pseudofractures, and progressive asymptomatic pseudofractures in patients with adult hypophosphatasia be treated with load-sharing internal-fixation devices, such as rigid in
