Governing One Health under climate pressure: policy implementation in Botswana.

BACKGROUND: Climate change is tightening the links between human, animal, and environmental health, especially in settings where systems are already stretched. One Health is now widely recognised in policy circles, but in many cases, it remains more of an intention than something that is consistently practised. METHODS: This paper draws on a qualitative policy analysis of academic literature, national policy documents, and selected comparative examples from Kenya, Bangladesh, and Germany. The aim was not to be exhaustive, but to focus on how implementation unfolds, paying attention to coordin

Turkey. Health system review.

Turkey has accomplished remarkable improvements in terms of health status in the last three decades, particularly after the implementation of the Health Transformation Program (HTP (Saglikta Donus, um Programi)). Average life expectancy reached 71.8 for men and 76.8 for women in 2010. The infant mortality rate (IMR) decreased to 10.1 per 1000 live births in 2010, down from 117.5 in 1980. Despite these achievements, there are still discrepancies in terms of infant mortality between rural and urban areas and different parts of the country, although these have been diminishing over the years. The

Nutritional interventions for TB: a critical component of integrated TB care amid funding challenges.

Undernutrition remains a leading driver of TB worldwide. Modeling and epidemiologic evidence show that nutritional interventions are highly cost-effective for reducing TB incidence and mortality, with benefits that extend beyond TB. Recent funding cuts and reprioritization threaten the availability of nutritional support and may hinder TB control efforts. We outline two approaches to sustain integrated support. First, countries can expand domestic financing through innovative mechanisms, such as taxes on tobacco and calorie-dense, low-nutrition foods. Second, governments should strengthen coll

From Product Availability to Population Protection: Vaccine Deployment Readiness in Sierra Leone.

Background: The 100 Days Mission has reorganised preparedness around vaccine development speed. Modelling suggests that meeting its target during the COVID-19 pandemic could have averted more than eight million deaths, conditional on equitable distribution. Distribution depends on a national pathway from authorisation through financing, cold chain, targeting, delivery, monitoring and community trust. The Mission measures speed at only one end of this system. Objective: To develop and apply a country-anchored framework for the deployment-readiness layer of preparedness. Methods: We conducted a

Financial determinants of effective hypertension and diabetes care in rural primary health facilities in Kisumu, Kenya: a mixed-methods study.

BACKGROUND: Noncommunicable diseases (NCDs), including hypertension and diabetes, account for approximately 27% of all deaths in Kenya, with 26% of adults having elevated blood pressure. Despite devolution of health services to county governments in 2013, financing for NCD management at the primary health care (PHC) level remains weak. This study examines financial determinants shaping hypertension and diabetes care in PHC facilities within a devolved county health system in rural Kisumu County, Kenya. METHODS: We conducted a convergent parallel mixed-methods cross-sectional study in seven pub

Financial costs of assisted reproductive technology for patients in low- and middle-income countries: a systematic review.

STUDY QUESTION: What are the direct costs of assisted reproductive technology (ART), and how affordable is it for patients in low- and middle-income countries (LMICS)? SUMMARY ANSWER: Direct medical costs paid by patients for infertility treatment are significantly higher than annual average income and GDP per capita, pointing to unaffordability and the risk of catastrophic expenditure for those in need. WHAT IS KNOWN ALREADY: Infertility treatment is largely inaccessible to many people in LMICs. Our analysis shows that no study in LMICs has previously compared ART medical costs across count

From Legal Indeterminacy to Risk-Governed Continuity: Operationalizing Risk Governance for China's Family Doctor Contract Services.

China's family doctor contract services (FDCS) are an important instrument for strengthening Primary Health Care, chronic disease management and continuity of care, but their legal and institutional foundations remain uneven. This Perspective treats legal indeterminacy not simply as a drafting defect but as a governable source of risk. It uses a structured narrative and legal-policy synthesis of Chinese FDCS policy and implementation materials and peer-reviewed studies principally from 2016-2025, relevant current Chinese laws and administrative rules, and limited functional reference points fr

When formal governance persists but resilience does not: health system governance and resilience in Yemen across a decade of conflict and crisis (2014-2025).

BACKGROUND: Yemen's health system has faced prolonged shocks, including war, epidemics, and COVID-19, challenging both governance and resilience. This study examines how formal governance structures and institutional capacities shaped the system's ability to anticipate, absorb, adapt, learn, and transform across a decade of crisis (2014-2025). METHODS: We conducted a longitudinal qualitative analysis based on interviews with health system leaders and technical actors. Using a resilience-capacity framework, we analyzed governance functions across four phases: pre-conflict period (pre-2014), co

Start-stop funding, its causes and consequences: a case study of the delivery exemptions policy in Ghana.

This article looks at the issue of sustaining funding for a public programme through the case study of the delivery exemptions policy in Ghana. The Government of Ghana introduced the policy of exempting users from delivery fees in September 2003 in the four most deprived regions of the country, and in April 2005 it was extended to the remaining six regions in Ghana. The aim of the policy of free delivery care was to reduce financial barriers to using maternity services. Using materials from key informant interviews at national and local levels in 2005, the article examines how the policy has b

Determining the impacts of hospital cost-sharing on the uninsured near-poor households in Vietnam.

OBJECTIVES: The study objective was to identify the size of different hospital financing sources for different hospital services and their impact on the uninsured. METHODS: A panel dataset of 84 public general hospitals (2005-2008) with cross-section data on hospital activity and hospital revenue was created and used to calculate unit costs of different hospital services by applying multiple regression models. The resulting risk of catastrophic health expenditure (CHE) was estimated based on official income statistics. RESULTS: Average user fees (UF) for outpatient visits and inpatient bed d

Power, policy and abortion care in Uganda.

Unsafe abortion practices remain the major contributor to maternal death in Uganda, impeding the achievement of universal health coverage and quality of maternal health care. Using an ethnographic design and critical discourse analysis, we explored the operations of power in setting maternal healthcare priorities, as evident at the 2018 Reproductive, Maternal, Neonatal, Child and Adolescents Health Conference. Observational data were collected of the policy-making activities, processes and events and key informant interviews were conducted with 27 participants. We describe how neoliberal and s

Trust or money? Barriers to health and healthcare behavior during the COVID-19 pandemic.

This study aimed to examine how trust in institutions and changes in household finances were associated with healthcare utilization and preventive behaviors during and immediately after the COVID-19 pandemic. The COVID-19 pandemic worsened health disparities, ignited distrust in healthcare systems, and contributed to household economic shifts for many United States (US) residents. To examine these issues, we surveyed a nationally representative sample of US residents in July 2020 (n = 1,085) and May 2023 (n = 2,189). These repeated cross-sectional surveys enabled investigation of how trust in
