Post-Traumatic stress disorder and post-traumatic growth in firefighters: examining the moderating effects of resilience on occupational safety behaviors.

BACKGROUND: Posttraumatic stress disorder (PTSD) and posttraumatic growth (PTG) represent divergent outcomes following trauma exposure in high-risk professions. This investigation examined the complex interplay between PTSD, PTG, and resilience in relation to occupational safety behaviors among Turkish firefighters. METHODS: A multi-site cross-sectional study was conducted across five metropolitan fire departments in Turkey (N = 122). Participants completed validated psychometric instruments including the PTSD Checklist for DSM-5 (PCL-5), Posttraumatic Growth Inventory-Expanded Form (PTGI-X),…

Evaluating Exposure to VOCs and Naphthalene for Firefighters Wearing Different PPE Configurations through Measures in Air, Exhaled Breath, and Urine.

Firefighters are at an increased risk of cancer due to their occupational exposure to combustion byproducts, especially when those compounds penetrate the firefighter personal protective equipment (PPE) ensemble. This has led to questions about the impact of base layers (i.e., shorts vs. pants) under PPE ensembles. This study asked 23 firefighters to perform firefighting activities while wearing one of three different PPE ensembles with varying degrees of protection. Additionally, half of the firefighters unzipped their jackets after the scenario while the other half kept their jackets zipped …

Biomechanics Analysis of the Firefighters' Thorax Movement on Personal Protective Equipment during Lifting Task Using Inertial Measurement Unit Motion Capture.

Back injury is a common musculoskeletal injury reported among firefighters (FFs) due to their nature of work and personal protective equipment (PPE). The nature of the work associated with heavy lifting tasks increases FFs' risk of back injury. This study aimed to assess the biomechanics movement of FFs on personal protective equipment during a lifting task. A set of questionnaires was used to identify the prevalence of musculoskeletal pain experienced by FFs. Inertial measurement unit (IMU) motion capture was used in this study to record the body angle deviation and angular acceleration of FF…

Hippocampal cerebral blood flow increased following low-pressure hyperbaric oxygenation in firefighters with mild traumatic brain injury and emotional distress.

BACKGROUND: Recent evidence suggests that hyperbaric oxygenation (HBO), which has been used as an effective treatment for certain types of tissue injury, may change neural activities in the human brain and subsequently improve symptoms of psychiatric disorders. To scrutinize the neural mechanism of HBO in the human brain, we investigated whether 20 sessions of HBO changed regional cerebral blood flow (rCBF) of the limbic system in firefighters with mild traumatic brain injury (mTBI) and subjective emotional distress. METHODS: Twenty firefighters with mTBI and mild emotional distress were trea…

Exercise-Based Performance Enhancement and Injury Prevention for Firefighters: Contrasting the Fitness- and Movement-Related Adaptations to Two Training Methodologies.

Using exercise to enhance physical fitness may have little impact on performers' movement patterns beyond the gym environment. This study examined the fitness and movement adaptations exhibited by firefighters in response to 2 training methodologies. Fifty-two firefighters were assigned to a movement-guided fitness (MOV), conventional fitness (FIT), or control (CON) group. Before and after 12 weeks of training, participants performed a fitness evaluation and laboratory-based test. Three-dimensional lumbar spine and frontal plane knee kinematics were quantified. Five whole-body tasks not includ…

Secondary traumatic stress and its impact on work efficiency and performance among firefighters in Saudi Arabia: A mixed-methods study.

BackgroundSecondary traumatic stress (STS) is a significant concern for firefighters due to their repeated exposure to traumatic events. In Saudi Arabia, cultural and organizational dynamics may amplify the risk of STS among firefighters' work performance. This heightened risk stems from culturally rooted stigma surrounding mental health, a lack of structured psychological support systems, and rigid hierarchical work environments that often discourage emotional expression or help-seeking behavior.ObjectiveThis study aims to investigate the prevalence of STS among Saudi firefighters, its impact…

Non-cancer mortality among firefighters: a meta-analytic review of heart disease, stroke, respiratory disease, liver disease, accidents, and suicide.

INTRODUCTION: Firefighting is a hazardous occupation linked to elevated cancer risk. However, occupational exposures unique to this profession may also contribute to non-cancer morbidity and mortality. To better understand firefighters' health risks, it is essential to examine causes of death beyond cancer. METHODS: Following PRISMA guidelines, we conducted a systematic review and meta-analysis of population-based studies published between 1978 and 2025. Studies reporting standardized mortality ratios (SMRs) for non-cancer causes of death were identified through database searches. We estimate…

Predictive Biomarkers of Gastroesophageal Reflux Disease and Barrett's Esophagus in World Trade Center Exposed Firefighters: a 15 Year Longitudinal Study.

Gastroesophageal reflux disease (GERD) and Barrett's Esophagus (BE), which are prevalent in the World Trade Center (WTC) exposed and general populations, negatively impact quality of life and cost of healthcare. GERD, a risk factor of BE, is linked to obstructive airways disease (OAD). We aim to identify serum biomarkers of GERD/BE, and assess the respiratory and clinical phenotype of a longitudinal cohort of never-smoking, male, WTC-exposed rescue workers presenting with pulmonary symptoms. Biomarkers collected soon after WTC-exposure were evaluated in optimized predictive models of GERD/BE.
