[Association of coronary-pulmonary fistula and intercoronary fistula. Apropos of a case].

The authors present the case of a 30-year-old woman admitted to hospital for investigation of a systolodiastolic murmur. Assessment revealed a fistula between the trunk of the left coronary artery and the pulmonary artery, with an aneurysm on the left anterior descending artery (LAD), associated with an intercoronary anastomosis between the left conus artery and the LAD. Thallium myocardial scintigraphy demonstrated reversible decreased uptake in the anterolateroapical region. Simple coronaro-pulmonary fistulas are a rare disease. The positive diagnosis is suggested by echocardiography and con…

[Esophago-tracheal fistula and ano-cutaneous fistula in a newborn (author's transl)].

Esophago-tracheal fistulas are rare malformations in which the frequency of associated defects is not yet well known. This is the reason for the description of a case of oesophago-tracheal fistula and ano-cutaneous fistula in a female newborn. Special attention is also paid to the problem of diagnostic difficulties, which were here resolved by oesophagoscopy accompanied by positive pressure ventilation through a tracheal tube.

Rectocutaneous fistulas.

This article reviews the etiology, diagnosis, and treatment of rectal perforation and rectocutaneous fistula in small animals. In addition, the literature regarding use of enterostomy in small animals is reviewed and comparative aspects of this procedure in humans and horses are presented. A case report of rectal perforation leading to rectocutaneous fistula formation and the use of colostomy in the management of the case is presented.

[Vesicovaginal fistulas].

Vesicovaginal fistulas are often the result of obstetric trauma in third world countries or gynaecologic surgery in developed countries. The incidence of obstetric trauma is approximately 3-4/1000 births in West Africa. The incidence of fistulas as a result of surgery has remained relatively unchanged for years; 75% occur during gynaecologic procedures. The main clinical symptom of a vesicovaginal fistula is urine loss. Different surgical techniques with similar repair results are available: transvaginal approach, transvesical approach and transperitoneal approach. Irrespective of the approach…

[Aortobronchial fistulas].

INTRODUCTION: Aortobronchial fistulas are uncommon but generally fatal if not treated surgically. Haemoptysis is the main symptom of this pathology. STATES OF ART AND PERSPECTIVES: Aortobronchial fistulas occur most commonly in patients with thoracic aneurysms (atherosclerosis, mycotic, aortic surgery's complication...). Main investigation is CT angiography with 2 D and 3 D reconstructions. CONCLUSION: Endovascular exclusion can be efficient treatment option.

Difficult fistulae.

Fistula in ano is a common disorder. The goals of treatment are to cure the fistula with minimal loss of sphincter function and with minimal healing time. Fortunately about 90% of fistulae are simple and obey Goodsall's rule. These fistulae are easily treated by the "lay-open" technique. Treatment can however become much more difficult with increasing complexity of fistula tracks, higher internal opening with major sphincter involvement, atypical and secondary tracks or at recurrence. Understanding of the anatomy and the pathogenesis of fistulae is mandatory to identify the fistula tracks and …

Rectovaginal fistulas.

The treatment of rectovaginal fistulas is controversial. The choice of the technique used for repair depends on many factors. Therefore the classification, etiology and treatment are discussed, in order to help decision making in the management of this troublesome disease.

Ureterovesicovaginal fistulas.

Our experience with urogenital fistulas are reviewed and three instances of complex ureterovesicovaginal fistulas, which can be mistaken for pure vesicovaginal fistulas because of diagnostic difficulties, are presented. If the ureterovaginal component of these fistulas is overlooked intraoperatively, urinary leakage will persist despite otherwise successful closure of the vesicovaginal component of the fistula. Because of the involvement of the terminal ureter in the fistulous system, operative therapy must combine the closure of the vesicovaginal fistula with reimplantation of the ureter into…

Choledochoduodenal fistulas.

In 2,012 ERCP studies reviewed in the period between 1976 and 1989, we observed 14 choledochoduodenal fistulas found in the upper portion of the papilla of Vater. Two fistulas occurred spontaneously due to necrosis of the duodenal wall caused by stones located in the distal common bile duct above the sphincter of the papilla. In these two cases the stones were removed endoscopically. The twelve remaining cases were iatrogenic, caused during surgical papillotomy which resulted in a false tract. All patients showed raised AP and gamma GT. Eight had acute cholangitis, 7 were jaundiced, there was …

[Cholangiothoracic fistulae].

Eight cholangio-thoracic fistula patients were seen from 1981 to 1987 in our hospital. The clinical features of the cases were classified into four types, i.e. bronchobiliary fistula (BBF, 3 cases), bronchopleurobiliary fistula (BPBF, 2 cases), pleurobiliary fistula (PBF, 2 cases) and pericardiobiliary fistula (PCBF, 1 case). In addition to a history of cholangitis or cholelithiasis and continuous or intermittent secondary biliary tract infection, the principle manifestations of BBF included: acute, subacute or chronic biliary empyema; biliary pneumonia or biliary lung abscess; and biliptysis.…

Colovesical fistulas.

Colovesical fistulas were identified in 42 patients. Diverticulitis was the underlying cause in 40 per cent of the patients. Carcinoma of the colon or cervix caused 33 per cent of the fistulas. Cystoscopy was the most effective procedure in diagnosing the fistulas. Surgical therapy had to be individualized to the patient's condition. Fistulas secondary to radiation were associated with a high complication rate.
