Charcot neuroarthropathy is a significant limb-threatening complication that develops in some patients with long-term diabetes mellitus. Early diagnosis is vital to secondary prevention of the destructive process and avoidance of consequent deformity and, ultimately, amputation. The purpose of this study was to determine which historical and physical findings would be more accurate risk factor indicators in those diabetics with and without Charcot foot deformity. A controlled series of tests, historical findings, and physical examinations were performed on 41 patients with diabetes who were Ch

BACKGROUND: Wolf-Hirschhorn syndrome is caused by distal deletion of the short arm of chromosome 4 (4p-). We report a case in which intrauterine growth restriction, hypospadias and foot deformity were detected by prenatal ultrasound examination at 29 weeks of gestation. CASE PRESENTATION: A 31-year-old gravida 2 partus 1 woman was referred at 29 weeks' gestation with suspicion of intrauterine growth restriction. Sonographic examination revealed deformity of the right lower limb and undescended testes with an irregular distal penis. A cordocentesis was performed and chromosome analysis revealed

The factors associated with failed operative intervention in the treatment of equinovarus foot deformity in children with cerebral palsy (CP) were evaluated after long-term follow-up. One hundred eight children with CP who had surgery on the posterior tibialis tendon (split tendon transfer, intramuscular lengthening, or Z-lengthening) on 140 feet were reviewed at a mean age of 16.8 years with 7.3 years of follow-up. The surgery was considered a failure when a 10 degrees or greater varus or valgus hindfoot deformity was present or if an additional operative intervention was required or planned.

We reviewed 24 feet in 15 patients who had undergone talectomy for recurrent equinovarus deformity; 21 were associated with arthrogryposis multiplex congenita, two with myelomeningocele and one with idiopathic congenital talipes equinovarus. The mean follow-up was 20 years. Good results were achieved in eight feet (33%) in which further surgery was not needed and walking was painless; a fair result was obtained in ten feet (42%) in which further surgery for recurrence of a hindfoot deformity had been necessary but walking was painless; the remaining six feet (25%) were poor, with pain on walki

The purpose of this study was to establish and characterize the relationship between foot pressure analysis and radiographic measurements in children that underwent a lateral column lengthening procedure for pes planovalgus (PPV). Eight children (13 feet) with PPV that had failed non-operative treatment underwent a lateral column lengthening procedure. Pre-operative and post-operative standing AP and lateral radiographs and foot pressure data were obtained. The relationships between the radiographic and foot pressure measurements were investigated. The findings from this study demonstrated str

We operated on 111 patients with 159 congenital club feet with the aim of correcting the deformity and achieving dynamic muscle balance. Clinical and biomechanical assessment was undertaken at least six years after operation when the patient was more than 13 years of age. The mean follow-up was for 11 years 10 months (6 to 36 years). Good and excellent results were obtained in 91.8%. Patients with normal function of the calf had a better outcome than those with weak calf muscles. The radiological changes were assessed in relation to the clinical outcome. The distribution of pressure under the

The authors explain the mechanism of pes equino-planus-valgus formation in patients with infantile cerebral paralysis (ICP) and describe in detail the clinico-radiological characteristics of such patients. The authors possess experience in the treatment of 31 patients who were operated on by a method suggested by A. M. Zhuravlev--osteomyoplasty of the root of the foot. The method removes all the components of the deformity and creates conditions preventing its recurrence. Study of the late-term results in postoperative follow-up periods of 6 months to 2 years shows the method to be much less i

In spite of the fact that discomfort from the subtalar area is common after varus-deformed tibial shaft fractures no plausible mechanism is to be found in the literature. A mechanical analysis of the problems shows that a varus deformity is compensated as pronation of the foot. A limited pronation capacity could thus be the cause of the pain. Pronation capacity was accurately measured in ten osteoligamentous preparations. The average pronation capacity was found to be 9.5 degrees +/- 7.0 degrees. There was a marked interindividual variation. In two of the specimens the pronation capacity was l

The indications, contraindications, technique, and end results of thirty-four tarsometatarsal truncated-wedge arthrodeses performed for cavus and equinovarus deformity of the fore part of the foot were reviewed. The etiology of deformity included idiopathic pes cavus, equinus deformity of the fore part of the foot, residual club foot, poliomyelitis, and compartment syndromes. With th truncated-wedge arthrodesis the subtalar joint is not violated and selective correction of any combination of depression of the metatarsal head is readily accomplished. By strict adherence to the indications and e

Forefoot disorders are prevalent in the general population, with an incidence between 2 and 20%. Among them, lesser toe deformities (hammer, claw, and mallet toes) are frequent disorders, and their conservative management is often not adequately considered but usually attempted before surgical indication. Among conservative treatments, shoe modifications and the application of orthoses may, in most cases, alleviate symptoms. From a careful analysis of the available literature, it emerged that treatment approaches should be individualized, and patient education has to be a central aspect of the

BACKGROUND: Hereditary motor and sensory neuropathies (HMSN), also known as Charcot-Marie-Tooth disease, are characterized by affected peripheral nerves. This often results in foot deformities that can be classified into four categories: (1) plantar flexed first metatarsal, neutral hindfoot, (2) plantar flexed first metatarsal, correctable hindfoot varus, (3) plantar flexed first metatarsal, uncorrectable hindfoot varus, and (4) hindfoot valgus. To improve management and for the evaluation of surgical interventions, a quantitative evaluation of foot function is required. The first aim of this

BACKGROUND: The use of conventional techniques in treating cleft foot deformities with two central ray deficiencies often yields unsatisfactory outcomes. This study describes the flap-bag technique, a novel technique using both a dorsal rectangular flap and a plantar triangular flap that was designed to yield more favorable outcomes in the treatment of this condition, and the outcomes obtained for three patients surgically treated with this technique.
METHODS: After the proper width of the forefoot was measured by manually holding the toes to maintain a transverse arch around the metatarsopha

Fifty-one children with cerebral palsy who were candidates for surgery for equinovarus deformities of the foot had their gait evaluated by means of foot-switches and electromyography to determine the contribution of the anterior tibialis and posterior tibialis muscles to the deformity. Seven EMG patterns and five foot-switch patterns were identified, but there was no correlation between the EMG and foot-switch patterns. Foot-switch data alone could not identify the deforming muscles or their patterns, so are not adequate for planning surgical correction of varus feet of children with cerebral

BACKGROUND: Lateral column lengthening (LCL) is reported as an effective operative treatment for flat foot deformity and can alter hindfoot alignment (HA). However, there are few reports studying changes in HA after LCL compared to other radiographic parameters.
PURPOSE: To investigate changes in HA and other radiographic parameters following isolated LCL in pediatric flatfoot deformity.
STUDY DESIGN: Retrospective case series.
METHODS: A total of 46 surgical corrections in 28 patients who underwent isolated LCL between 2008 and 2013 were reviewed. The mean patient age was 9.7 years (range,

BACKGROUND: Peritalar subluxation (PTS) contributes to progressive collapsing foot deformity (PCFD), yet its pathophysiology remains unclear. This study analyzed subtalar joint (SJ) dynamics throughout deformity progression.
METHODS: A retrospective review of 153 WBCT scans classified PCFD into four stages: (1) no PTS, (2) PTS without impingement, (3) PTS with sinus tarsi impingement (STI), and (4) PTS with subfibular impingement (SFI). Three-dimensional measurements of middle facet subluxation and incongruence, foot-ankle offset, SJ offset, and lateral process offset were compared between gr

Charcot-Marie-Tooth (CMT) disease is the most prevalent inherited peripheral neuropathy, characterized by progressive motor and sensory dysfunction resulting from a wide spectrum of genetic abnormalities. More than 70% of affected individuals develop cavovarus foot deformities, which impair gait mechanics, reduce functional mobility, and frequently necessitate orthotic support. The diagnosis of CMT-related foot deformity requires a comprehensive clinical evaluation, with particular emphasis on identifying muscle imbalance and assessing the function of individual musculotendinous units contribu

The foot deformation approach (FDA) estimates the ground reaction force (GRF) and moment (GRM) from kinematic data with practical accuracy, low computational cost, and no requirement for training data. Our previous study demonstrated practical estimation accuracy of the FDA under barefoot conditions. However, since the FDA estimates GRFs and GRMs based on foot deformation under body weight, there are concerns about its applicability to footwear conditions, where the foot deformation characteristics differ from those of bare feet. Following the issue, this study conducted a walking experiment a
