We report onychomycosis caused by Candida parapsilosis in a 35-day-old premature infant. To our knowledge, this represents the youngest instance of an isolated lesion of the nail plate without involvement of glabrous skin, caused by C. parapsilosis in this age group to be reported in the literature.

Contact dermatitis of the nail unit is not rare. The most common allergens are found in nail cosmetics such as enamel, sculptured nails, and preformed plastic tips. Toluene sulfonamide formaldehyde resin, acrylates, and ethylcyanoacrylate are the most common allergens. Along with a proper clinical history focusing on nail practices, a thorough understanding of the common allergens and patch testing are necessary to diagnose contact dermatitis. A discussion of the common allergens and irritants relevant to the nail unit will be presented.

A 38-year-old female presented with a history of recurrent multiple deep dermal nodules located on the dorsa of the feet, over the ankle joints and lower pretibial area. A 46-year-old patient with 'locked-in syndrome' due to brainstem infarction developed symmetrically distributed, grouped erythematous cornified hemispherical papules on the fingertips and toes in 2 years. Histologically they showed psoriasiform changes. There was no evidence of internal malignancy nor a history of special drug intake. To our knowledge, such an eruption has not been previously reported.

Although penicillin still remains highly effective for syphilis, concerns have been raised that current regimens may be inadequate for the treatment of early syphilis. We report the failure of benzathine penicillin in the treatment of secondary syphilis. Extensive tracing of sexual contact history failed to identify sources of possible reinfection; therefore, we believe that this case represents a treatment failure and an ensuing relapse of secondary syphilis.

We describe here nine women who had similar distinctive clinical features that consisted of painful red-to-purple macules, papules, and plaques on the digits, predominantly on the toes. All patients reported that exposure to cold had preceded their symptoms. Histologic study demonstrated a lymphocytic vasculitis, characterized by lymphocytes infiltrating the walls of blood vessels in eight (88%) of nine lesions. Glucose tolerance was investigated in 41 patients with pustulosis palmaris et plantaris. Twenty-eight (68%) of the patients showed abnormal results. These findings suggest that pustulosis palmaris et plantaris is associated with impaired carbohydrate metabolism.

In previous study we found that clavicular lesions in chronic recurrent multifocal osteomyelitis (CRMO) in young patients was associated with palmoplantar pustulosis (PPP). In this work the relationship between sternoclavicular lesions and PPP was studied in a group of adult patients.
