Association Between Head Impact Biomechanics and Physical Load in College Football.

Head impacts and physical exertion are ubiquitous in American football, but the relationship between these factors is poorly understood across a competitive season or even within an individual session. Gameplay characteristics, including player position and session type, may contribute to these relationships but have not been prospectively examined. The current study aimed to determine if an association exists between head impact biomechanics and physical load metrics. We prospectively studied college football players during the 2017-2021 football seasons across representative playing position

Video Analysis of Pectoralis Major Injuries in Professional Australian Football Players.

BACKGROUND: There is little evidence regarding the mechanisms of pectoralis major (PM) injury and player outcomes in Australian Football League (AFL) players. PURPOSES/HYPOTHESIS: The study aims were to investigate (1) the mechanisms of PM muscle injury in elite AFL players via video analysis and (2) the player profile, method of management, and clinical outcomes of the PM injuries sustained. We hypothesized that the majority of PM tears would occur in outer-range PM positions (hyperextension of the glenohumeral joint). STUDY DESIGN: Case series; Level of evidence, 4. METHODS: We analyzed v

Impaired cerebral blood flow regulation and cognition in male football players.

Football players are at increased risk of neurodegeneration, the likely consequence of repetitive mechanical trauma caused by heading the ball. However, to what extent a history of heading the ball affects cerebral blood flow (CBF) regulation and its potential relationship to cognitive impairment is unknown. To address this, we recruited 16 concussion-free male amateur football players (age: 25 ± 6 y) with a history of heading the ball (18 ± 6 y) and 18 sex, age, education, and activity-matched controls with no prior history of contact sport participation or concussion. Cerebral perfusion was

Physical Performance Measures Correlate with Head Impact Exposure in Youth Football.

PURPOSE: Head impact exposure (HIE) (i.e., magnitude and frequency of impacts) can vary considerably among individuals within a single football team. To better understand individual-specific factors that may explain variation in head impact biomechanics, this study aimed to evaluate the relationship between physical performance measures and HIE metrics in youth football players. METHODS: Head impact data were collected from youth football players using the Head Impact Telemetry System. Head impact exposure was quantified in terms of impact frequency, linear and rotational head acceleration, a

Finite Element Model of a Deformable American Football Helmet Under Impact.

Despite the use of helmets in American football, brain injuries are still prevalent. To reduce the burden of these injuries, novel impact mitigation systems are needed. The Vicis Zero1 (VZ1) American football helmet is unique in its use of multi-directional buckling structures sandwiched between a deformable outer shell and a stiff inner shell. The objective of this study was to develop a model of the VZ1 and to assess this unique characteristic for its role in mitigating head kinematics. The VZ1 model was developed using a bottom-up framework that emphasized material testing, constitutive mod

Estimating drivers of concussions from punt returns in professional American football.

OBJECTIVES: To estimate drivers of concussions during punt kick returns in football and consider rule change incentives to potentially reduce concussions. STUDY DESIGN: The design of the study was to analyze concussions during punt returns. METHODS: National Football League data of all punts from the 2016 and 2017 seasons are examined using propensity score matching (PSM) to estimate the causal effect of punt returns on the likelihood of a player concussion. RESULTS: The PSM logistic regression estimates that the odds of a concussion increase 8.31, or are 731% higher, when a punt is returne

The Pediatrician's Moral Obligation to Counsel Directively Against Youth Tackle Football.

In this issue of The Journal of Clinical Ethics, Professor Ruth Tallman argues that pediatricians ought to support adolescent football players in their athletic goals. She does not deny that doing so means "helping children hurt themselves"; rather she argues that this would be consistent with a shared decision-making model in which both the physician and the patient seek to promote the patient's well-being in light of the patient's own goals. I argue that this ignores the role of the parents, meaning that Tallman is suggesting "helping parents allow their children to hurt themselves." As a ge

Arthroscopic Anterior Ankle Decompression Is Successful in National Football League Players.

Anterior ankle impingement is a frequent cause of pain and disability in athletes with impingement of soft-tissue or osseous structures along the anterior margin of the tibiotalar joint during dorsiflexion. In this study, we hypothesized that arthroscopic decompression of anterior ankle impingement would result in significant, reliable, and durable improvement in pain and range of motion (ROM), and would allow National Football League (NFL) players to return to their preoperative level of play. We reviewed 29 arthroscopic ankle débridements performed by a single surgeon. Each NFL player underw

Transosseous Physeal-Sparing Rotator Cuff Repair in an Adolescent Football Player.

Rotator cuff injuries in the pediatric and adolescent population are rare. These injuries are normally caused by an acute event or by repeated microtrauma as a result of poor mechanics during overhead sporting activities. Overall, these injuries account for fewer than 1% of all rotator cuff tears. Physeal injuries, tuberosity avulsions ("rotator cuff equivalents"), and proximal humeral fractures commonly occur in the pediatric population. Traumatic full-thickness rotator cuff ruptures that occur in this age group during contact sports are an extremely uncommon cause of shoulder pain. The rarit

Possible disease transmission by contaminated mouthguards in two young football players.

Previous studies have demonstrated that athletic mouthguards worn by ice hockey and football players harbor large numbers of bacteria, yeasts, and molds, some of which are either opportunistic or frank pathogens. This article details the clinical history of two junior high school football players. The first player had cellulitis of the leg after a non-break injury. The same unusual bacterium was isolated from both the athletic mouthguard and abscess cultures from the wound. The second patient suffered an attack of exercise-induced asthma so severe that his inhaler could not control the symptom

Chronic traumatic encephalopathy in a national football league player: part II.

OBJECTIVE: We present the second reported case of autopsy-confirmed chronic traumatic encephalopathy in a retired professional football player, with neuropathological features that differ from those of the first reported case. These differing pathological features underscore the need for further empirical elucidation of the pathoetiology and pathological cascades of long-term neurodegenerative sequelae of professional football. METHODS: A psychological autopsy was performed with the next-of-kin and wife. Medical and hospital records were reviewed. A complete autopsy was accompanied by a compr
