[Systemic granulomatous reaction in hip prosthesis. Apropos of 2 anatomoclinical cases].

PURPOSE OF THE STUDY: The production of particulate wear debris is a recognised complication of joint arthroplasty. Focus was made on local tissue reactions. Systemic distribution of wear debris are less know. We report two new cases of distant granulomatous reaction. MATERIAL: The first case concerned a 71-year-old man with lymph node histiocytosis incidentally discovered during the staging of a prostatic carcinoma. The second case concerned a 61-year-old man with a visceral granulomatosis reaction (liver, spleen and lymph node) associated to hepatic ans splenic enlargement. METHODS: We rea

Periocular migration of an intraocular lens.

A woman presented with a painful eye 6 weeks after cataract extraction and intraocular lens implantation. In the past she had had a sector iridectomy for iris bombé caused by chronic anterior uveitis. On examination the three central corneal sutures were absent, whilst the medial and lateral sutures had broken and were protruding from the section. The eye was quiet and the section intact. Combined clinical and ultrasound examination failed to locate the intraocular lens. Four months postoperatively, while being fitted for contact lenses for the correction of aphakia, the intraocular lens appea

Modified method for prophylaxis against unishunt system complications with presentation of total intraventricular migration of unisystem ventriculoperitoneal shunt.

This report is a case of shunted hydrocephalus in which the Unishunt system was placed without the clips (lock and slip). Upward migration of the whole system into the lateral ventricle was discovered 1 month after surgery. The shunt was reinserted, placing the proximal "lock-clip" under the cranial flap and the distal "slip-clip" subcutaneously at the abdominal area. The skin over the slip-clip ulcerated, so it was repositioned and fixed deep in the abdominal muscle, extraperitoneally. Different factors caused the migration: negative sucking intraventricular pressure; pushing, positive intra-

Intrapelvic migration of Knowles pin through external iliac vein.

Intrapelvic migration of a Knowles pin is rare. A case is reported of Knowles pin migration into the pelvis and through the external iliac vein. This was caused by a combination of excessive motion of the femoral neck nonunion and osteoporosis of the femoral head which allowed forward migration of the pin.

Twiddler's syndrome: a new twist.

Twiddler's syndrome, characterized by dislodgment of pacemaker leads due to twisting of pulse generators within the subcutaneous pocket with subsequent retraction of leads and loss of pacing function, has been well described in patients with permanent pacemakers implanted for bradyarrhythmias. The case presented here is the first report of a patient with an internal automatic cardioverter-defibrillator with lead dislodgment due to pulse generator rotation. This case exemplifies a new subset of patients prone to the Twiddler's syndrome.

Renal actinomycosis associated with a duodenorenal fistula caused by foreign body.

We are reporting a case of a rare renal actinomycosis in a 12-year-old mentally-retarded girl. Proteinuria and hemopyuria were pointed out one year before the operation by an annual medical check-up and IVP subsequently performed showed foreign bodies at the upper pole of the right kidney. The patient continued to have pyuria and right nephrectomy was performed. There was a fistula between the duodenum and the upper portion of the right kidney. Foreign bodies (two bobby pins) were found in the kidney. Subsequent pathologic examination of the resected kidney revealed an actinomycotic lesion.

Aspiration following a cave-in.

Reported is a case of massive aspiration of particulate matter after a cave-in. A 14-year-old boy was buried beneath 18 inches of dirt while playing at a construction site. After extrication he was breathing spontaneously and was transported to the emergency department on supplemental oxygen. Physical examination revealed cough, tachypnea, and diminished breath sounds of the right lung field. Chest radiographs showed multiple radiopaque densities filling the right mainstem bronchus and a left-to-right shift of the mediastinum. The patient was treated with bronchodilators followed by postural d

Septic pericarditis caused by a migrating grass awn in a cat.

Septic pericarditis and cardiac tamponade associated with migrating grass awn foreign bodies is reported rarely in companion animals. We report such a case in a previously healthy, 9-year-old, neutered female, indoor-outdoor, domestic long-hair cat who presented for acute tachypnea. Large volume pericardial effusion and pericardial tamponade was identified by thoracic-focused assessment with sonography. Following removal of 108 mL of purulent pericardial effusion by pericardiocentesis, the cat improved. Cytologic examination of pericardial fluid demonstrated septic, suppurative inflammation, P

A unique challenge overcome: Clay foreign body extracted from the bladder endoscopically; A case report.

INTRODUCTION: Urethral foreign bodies are uncommon clinical entities typically associated with psychiatric disorders or self-erotic stimulation. They present unique challenges, especially when involving malleable materials. CASE PRESENTATION: A 26-year-old male presented with urinary retention and brown urethral discharge after inserting an 8 cm clay-like foreign body. Cystoscopy revealed migration to the bladder. Initial bedside flexible cystoscopy under local anesthesia removed 40 % of the material using a 4.5 mm basket. Complete extraction was achieved in the operating room using a 17 Fr r

A Plastic Torpedo-Shaped Firecracker That Caused Injury to the Parotid Gland: A Case Report.

Penetrating injuries affecting the parotid gland are relatively rare compared to injuries affecting other head and neck structures. Likewise, foreign bodies impacting the parotid gland are extremely rare. These foreign bodies can be secondary to penetrating injury or may reach the parotid gland by retrograde migration through Stensen's duct. Management of parotid gland foreign bodies is a challenging clinical situation for otolaryngologists due to the course of the facial nerve through the gland. In this article, a patient with penetrating parotid injury as a result of the explosion of a torpe

Endovascular Retrieval of a Detached and Dislocated Venous Port Catheter in the Right Heart Chamber Using a Triple-Loop Snare Device.

The authors present a case of a successful percutaneous retrieval of a detached port-a-catheter device that had migrated to the right cardiac chambers in a patient with inoperable pancreatic cancer and hepatic metastases. The patient was admitted to the vascular clinic department on an urgent basis due to an accidental detachment of the catheter during removal at another hospital. The catheter had migrated from the initial placement site in the right subclavian vein to the superior vena cava and right heart chambers. Under local anesthesia, the right femoral vein was accessed using the Selding

ENDOSCOPIC MANAGEMENT OF MESH MIGRATION FOLLOWING HIATAL HERNIA REPAIR.

BACKGROUND: The use of mesh in the repair of large hiatal hernias is still controversial. One of the most feared adverse events related to the use of mesh is erosion into the esophageal and gastric walls. AIMS: To record the endoscopic treatment of mesh that has migrated into the gastric lumen after surgical treatment of hiatal hernia. METHODS: The technical option was to wait for the progressive migration of the mesh into the gastric lumen, monitoring with upper digestive endoscopy, with removal by traction at the best time, with the aid of foreign body forceps. RESULTS: The mesh was compl

The Cochlear Implant "Seatbelt" Technique for Receiver/Stimulator Fixation.

Receiver/stimulator migration and extrusion are among the most commonly reported complications of cochlear implantation. Current techniques implement either a tight subperiosteal pocket or postauricular bone well. Here we describe a safe and effective technique to secure a cochlear implant receiver/stimulator with a Mersilene cervical cerclage tape "seatbelt." Laryngoscope, 134:3374-3376, 2024.

A rare case of CardioMEMS™ sensor migration.

The CardioMEMS™ system remotely monitors changes in pulmonary artery pressures, which allows for early detection of heart failure worsening. It is a safe and reliable invasive monitoring system. We report a case in which there was a late migration of the device at 6 months of follow-up to the contralateral pulmonary artery. The mechanisms, consequences, and management of device migration are discussed. To our knowledge, there are very few published data on late sensor migration.
