[Peculiarities of surgical technique for the treatment of rhinogenic abscesses of the frontal cerebral lobe].

Peculiar features of the clinical picture of rhinogenic abscesses of the frontal cerebral lobe (RAFCL) were studied for the purpose of optimization of medical and diagnostic measures intended for the early detection of this severe disease. Causes and pathways of infection penetrating in the cranial cavity and possibilities of RAFCL diagnosis were thoroughly analysed in 4 patients with the help of computerized tomography (CT), in 1 patients by brain ultrasound study (BUS) during osteoplastic trepanation (OPT), and in 9 patients using neurologic diagnostic tools. A total of 14 patients were give

Brain structural differences in temporal lobe and frontal lobe epilepsy patients: A voxel-based morphometry and vertex-based surface analysis.

PURPOSE: Exploration of the effect of chronic recurrent seizures in focal epilepsy on brain volumes has produced many conflicting reports. To determine differences in brain structure in temporal lobe epilepsy (TLE) and extratemporal epilepsy (using frontal lobe epilepsy (FLE) a surrogate) further, we performed a retrospective analysis of a large cohort of patients with seizure-onset zone proven by intracranial monitoring. METHODS: A total of 120 TLE patients, 86 FLE patients, and 54 healthy controls were enrolled in this study. An analysis of variance of voxel-based morphometry (VBM) was used…

A case of probable korsakoff's syndrome: a syndrome of frontal lobe and diencephalic structural pathogenesis and a comparison with medial temporal lobe dementias.

Korsakoff's Syndrome is an amnestic disorder that involves both anterograde and retrograde amnesia. Traditionally associated with longstanding alcohol misuse, thiamine deficiency has been long posited in its pathogenesis, as has dienecephalic lesions. Yet, through this case report, we highlight similarities (and differences) with medial temporal lobe memory disorders and the role of frontal lobe dysfunction in its retrograde amnesia.

Long-term seizure outcome with the surgically remediable syndrome of frontal lobe epilepsy associated with superior frontal sulcus-related dysplasia.

OBJECTIVE: To assess the long-term outcome of the surgically remediable syndrome of frontal lobe epilepsy (FLE) associated with superior frontal sulcus (SFS)-related dysplasia. METHODS: We retrospectively reviewed the medical charts and surgical features of 31 patients with drug-resistant frontal lobe epilepsy in our centers between 2016 and 2018. All patients underwent surgical resection. According to the epileptogenic zone (EZ), localization and resection extent were classified as (1) pure SFS group (PS group), (2) associated SFS group (AS group), and (3) no SFS group (NS group). The genera…

Association between lack of functional connectivity of the frontal brain region and poor response inhibition in children with frontal lobe epilepsy.

PURPOSE: We investigated the relationship between electroencephalographic (EEG) functional connectivity and executive function in children with frontal lobe epilepsy (FLE). METHODS: We enrolled 24 children with FLE (mean age, 11.0 years; 13 boys) and 22 sex-, age-, and intelligence-matched typically developing children (TDC) to undergo 19-channel EEG during light sleep. We estimated functional connectivity using the phase lag index (PLI) that captures the synchronization of EEG. We also performed continuous performance tests (CPTs) on the children and obtained questionnaire responses on atten…

Fetal frontal lobe development in gestational diabetes mellitus: a cross-sectional neurosonographic study.

OBJECTIVES: To evaluate the impact of gestational diabetes mellitus (GDM) on fetal frontal lobe development, as assessed by the anteroposterior frontal diameter (APFD) and its relationships with standard cranial biometric parameters. METHODS: This case-control study included 257 singleton pregnancies between 20 and 38 + 6 week's gestation. Participants were divided into two groups: uncomplicated pregnancies (n=152) and pregnancies complicated by GDM (n=105). Ultrasound images were retrospectively analyzed to obtain APFD, occipitofrontal diameter (OFD), head circumference (HC), and transverse …

Ictal emotional features in pediatric and young adult patients with frontal lobe epilepsy.

OBJECTIVE: To assess the localization and lateralization values of ictal emotional features and ictal defensive behaviors in pediatric and young adult patients with frontal lobe epilepsy (FLE). METHODS: Children and young adults with FLE were identified from a single institution's epilepsy surgery database. Two epileptologists determined by consensus the presence of ictal emotional features (positive- smile, laughter, humming, singing; negative- fear, crying, anger, disgust, contempt, sadness) and ictal defensive behaviors (freezing with a fearful facial expression, shielding the face with ha…

EEG-Based Seizure Onset Detection of Frontal and Temporal Lobe Epilepsies Using 1DCNN.

OBJECTIVE: The manual interpretation of electroencephalogram (EEG) signals for detecting epileptic seizures is time-consuming and labor-intensive, highlighting the critical importance of exploring automated seizure detection methods. Given this, this work concentrates on seizure detection using scalp EEG signals collected from people with frontal lobe epilepsy (FLE) and temporal lobe epilepsy (TLE). METHOD: 20 FLE patients and 20 TLE patients are utilized in our work, and a parallel one-dimensional convolutional neural network (1DCNN) model is built for classification. Our work explores two s…

Disorganization of language and working memory systems in frontal versus temporal lobe epilepsy.

Cognitive impairment is a common comorbidity of epilepsy and adversely impacts people with both frontal lobe (FLE) and temporal lobe (TLE) epilepsy. While its neural substrates have been investigated extensively in TLE, functional imaging studies in FLE are scarce. In this study, we profiled the neural processes underlying cognitive impairment in FLE and directly compared FLE and TLE to establish commonalities and differences. We investigated 172 adult participants (56 with FLE, 64 with TLE and 52 controls) using neuropsychological tests and four functional MRI tasks probing expressive languag…

Protein expression of P-glycoprotein in neocortex from patients with frontal lobe epilepsy.

Frontal lobe epilepsy (FLE) is the second most frequent type of epilepsy and the surgical outcome depends on the etiology. For instance, patients with posttraumatic FLE (PTE) have a worse surgical outcome compared to patients with FLE related to a tumoral lesion (TL). The present study focuses to determine if the FLE etiology is associated with the P-glycoprotein (P-gp) expression, a condition associated with drug resistance. P-gp expression and cellular localization were determined by Western Blot and immunohistochemical experiments in cortical brain samples obtained from patients with PTE (n…

Activation of the basal ganglia and indirect pathway neurons during frontal lobe seizures.

There are no detailed descriptions of neuronal circuit active during frontal lobe motor seizures. Using activity reporter mice, local field potential recordings, tissue clearing, viral tracing, and super-resolution microscopy, we found neuronal activation after focal motor to bilateral tonic-clonic seizures in the striatum, globus pallidus externus, subthalamic nucleus, substantia nigra pars reticulata and neurons of the indirect pathway. Seizures preferentially activated dopamine D2 receptor-expressing neurons over D1 in the striatum, which have different projections. Furthermore, the D2 rece…

Arterial spin labeling for presurgical localization of refractory frontal lobe epilepsy in children.

BACKGROUND: Epilepsy is one of the most common chronic neurological diseases. Despite the great variety and prevalence of antiepileptic drug treatments, one-third of epilepsies remain drug resistant. The frontal lobe is extensive, and frontal lobe seizures are difficult to locate, which increases the difficulty of the preoperative localization of the epileptogenic zone. CASE PRESENTATION: Two previously healthy girls with refractory frontal lobe epilepsy showed significant perfusion abnormalities in the right frontal lobe using the cerebral blood perfusion (CBF) quantitative analysis system. …

Frontal lobe epilepsy with focal neuronal lipofuscinosis - Case report of a rare entity.

Focal neuronal lipofuscinosis is a unique neuronal pathology, characterised by accumulation of lipofuscin within dysmorphic neurons. We report a case of 12-year-old female with drug resistant epilepsy since one and a half years of age. MRI brain showed right frontal dysplasia, and PET showed right frontal hypometabolism. She underwent electrocorticography-guided resection of the lesion. Histopathology revealed cortical dyslamination with several hypertrophic dysmorphic neurons showing intracytoplasmic granular accumulation of lipofuscin which was positive for Periodic acid-Schiff, Luxol fast b
