Significance of a frozen section analysis of the ureteral margin in bladder cancer patients treated with radical cystectomy and neoadjuvant chemotherapy.

Although the clinical utility of a frozen section analysis (FSA) at the time of radical cystectomy (RC) has already been established, its significance and utility in bladder cancer patients receiving neoadjuvant chemotherapy (NAC) have not yet been fully evaluated. We identified 458 patients (937 ureters) who underwent open RC for bladder cancer at our 7 Japanese institutions between 2004 and 2015. Among these patients, 139 (284 ureters) received NAC before RC (NAC group), while 319 (653 ureters) underwent RC alone (non-NAC group). FSA was performed on 356 out of 937 (38.0%) ureters and 179 ou

T2b Gallbladder Adenocarcinoma Diagnosed by Frozen Section With High-Grade Dysplasia at the Cystic Duct Margin: Diagnostic and Prognostic Considerations.

Gallbladder adenocarcinoma is an uncommon gastrointestinal malignancy that is frequently discovered incidentally during cholecystectomy. Intraoperative frozen section is often used to guide surgical decision-making but has inherent limitations, particularly in assessing depth of invasion. We report the case of a 78-year-old female who underwent an elective laparoscopic cholecystectomy for a gallbladder mass. Intraoperative frozen section demonstrated a tubulovillous adenoma with at least carcinoma in situ. Subsequent permanent histologic evaluation revealed a moderately differentiated adenocar

Evaluation of Diagnostic Value of Frozen Section for Ovarian Masses in a Referral Hospital in Central Part of Iran, with Focus on Borderline Cases.

BACKGROUND: Differentiating between benign ovarian masses and malignant ones is done using a variety of pathological examination methods. The two most prominent are frozen section diagnosis and permanent pathology diagnosis, the gold standard. We conducted this study with the aim of assessing the accuracy of frozen section diagnosis in ovarian masses. MATERIALS AND METHODS: We studied different cases of ovarian masses with pathologic studies done using frozen section diagnosis and then compared the results with the final diagnosis using permanent studies. Sensitivity, specificity, positive pr

Omission of Routine Frozen Section for Clinically Node-Negative Patients Undergoing Upfront Mastectomy Avoids Unnecessary Axillary Treatment.

BACKGROUND: Randomized trials established the safety of postmastectomy radiation/regional nodal irradiation (PMRT) as an alternative to axillary lymph node dissection (ALND) for cN0 patients undergoing upfront mastectomy with one or two positive sentinel lymph nodes (+SLNs). In January 2022, the authors adopted a policy omitting routine frozen section for cT1-3N0 patients undergoing upfront mastectomy, and this study sought to examine the impact of this policy on axillary treatment. METHODS: Consecutive patients with cT1-3N0 breast cancer who underwent upfront mastectomy from January 2022 to

[Germ Cell Tumours of the Testis: Classification, Frozen Section Diagnostics, Sentinel Lymph Node Biopsy, and Special Biological Variants].

Germ cell tumours (GCTs) are the most common testicular neoplasms and are classified into type I, type II, and type III tumours depending on age of onset, molecular biology, and histomorphology. There are six histological subtypes, whose distribution varies among the three groups: seminomas, embryonal carcinomas, choriocarcinomas, yolk-sac tumours, teratomas, and spermatocytic tumours. The most common are malignant type II GCTs, typically treated with radical orchiectomy. In cases of small tumours where tumour dignity is uncertain - such as when serum markers are negative - initial enucleation

Development and validation of a predictive model for assessing the risk of follicular carcinoma in thyroid nodules identified as suspicious by intraoperative frozen section.

INTRODUCTION: Follicular thyroid carcinoma (FTC) is the second most common thyroid malignancy and is characterized by a higher risk of distant metastasis compared to papillary thyroid cancer. Intraoperative frozen section (IOFS) diagnosis of FTC is challenging due to its limited sensitivity and accuracy, leading to uncertainty in intraoperative surgical decision-making. In response, we developed a predictive model to assess the risk of follicular carcinoma in thyroid nodules identified as suspicious for follicular neoplasm by IOFS. METHODS: This model was derived from preoperative clinical an

Intraoperative pathologic diagnosis of central nervous system lymphomas: A comparison of frozen and permanent section diagnoses, and the significance of preoperative imaging.

BACKGROUND: Central nervous system (CNS) lymphomas, either primary or secondary in origin, are rare malignant tumors affecting the brain, spinal cord, or leptomeninges. Diagnosis of CNS lymphomas is complicated by their diverse clinical presentations, radiological features, and histopathological characteristics. Although frozen section (FS) analysis is commonly employed for various CNS tumors, its role and accuracy in CNS lymphoma diagnosis are less explored. In this study, we conducted a comparative analysis to assess the impact of knowledge of preoperative imaging on enhancing the accuracy o

Cellular Leiomyoma versus Endometrial Stromal Sarcoma: A Report of a Rare Case Presenting a Diagnostic Challenge on Intraoperative Frozen Section.

Endometrial stromal sarcomas (ESSs) account for approximately 0.2% of all uterine malignancies. Cellular leiomyoma (CL) often simulates low-grade ESS due to similar cytology. We report the case of a 34-year-old female with a mass per abdomen. Frozen sections showed a tumor with many thin- and thick-walled vessels along with hyaline material. A differential diagnosis of CL and endometrial stromal tumor was suggested. The index case was diagnostically challenging to pathologists. Paraffin sections supplemented by immunohistochemistry (smooth muscle actin, CD10, and beta-catenin) favored CL. Froz…

Preoperative axillary ultrasound in breast cancer: safely avoiding frozen section of sentinel lymph nodes in breast-conserving surgery.

BACKGROUND: The American College of Surgeons Oncology Group Z0011 trial results provided convincing evidence that completion axillary lymph node dissection (CALND) was unnecessary in selected patients with 1 to 2 positive sentinel lymph nodes (SLNs). We hypothesized that preoperative axillary ultrasound (AUS) with fine-needle aspiration is sufficiently sensitive to detect worrisome macrometastasis to preclude the need for frozen-section pathology of SLNs. STUDY DESIGN: We conducted a retrospective single-institution study at a tertiary academic referral center. A total of 1,140 T1 to 2 breast

Reliability of frozen section in detecting tumor-free margins in oral squamous cell Carcinoma: a systematic review & meta-analysis.

BACKGROUND: The frozen section technique is a vital intraoperative diagnostic tool that enables rapid microscopic assessment of tissues, providing immediate information to guide surgical decision-making, including evaluation of tumor margins, disease extent, and metastatic involvement. Given the high risk of positive surgical margins in oral squamous cell carcinoma (OSCC), achieving margin-negative resection is critical to minimizing recurrence. This systematic review and meta-analysis evaluates the reliability of frozen section analysis in detecting tumor-free margins in OSCC. METHODS: A com
