Whose knowledge counts? Equity, epistemic justice, and reforming infectious disease research culture.

Infectious disease epidemiology is shaped by engrained research cultures that privilege biomedical and quantitative knowledge systems, systematically marginalizing qualitative, contextual, and locally informed approaches. These hierarchies reflect deeper inequities in who leads, who participates, and whose knowledge counts-disparities often patterned along geography, gender, language, and disciplinary background. This perspectives paper examines how funding priorities, academic training, and publishing norms sustain epistemic and structural exclusion, particularly for researchers based in the

A global analysis of adaptation to societal aging across low-, middle- and high-income countries using the Global Aging Society Index.

We have previously presented a multidimensional Aging Society Index, a weighted summation of five domains central to successful adaptation to societal aging: well-being, productivity and engagement, equity, cohesion and security, as a tool to assess countries' adaptation to demographic transformation. As the index was based on data from developed countries and some of the individual metrics or weightings may not be well suited for application to low- and middle-income countries, we here present the scores on a modified index (Global Aging Society Index) on 143 countries distributed across the

An intersectionality-based policy analysis of COVID-19 measures and women informal food vendors in Kisumu, Kenya.

INTRODUCTION: Women working as informal food vendors in Kisumu faced precarious work conditions that worsened during the COVID-19 pandemic. Despite playing a vital role in urban household food security, they were excluded from policy decisions, which were largely confined to technical experts and national government officials. METHODS: Using the Intersectionality-Based Policy Analysis (IBPA) framework, this study examined the engagement of women informal food vendors and the disparities in COVID-19 control policies. Between July 10, 2024, and October 15, 2024, we conducted a qualitative study

Workforce diversity and earnings differentials in Canadian nursing.

BACKGROUND: Studies from different countries document persistent female/male wage gaps in the health workforce, including in female-dominated professions such as nursing. Evidence is less clear on inequalities by gender, sexual, and racial-ethnic minority status. OBJECTIVE: To analyze whether nurses of diverse identities are receiving comparable compensation for comparable work. METHODS: This national observational study leverages microdata from the 2021 Canadian Population Census, the first large-scale source collecting information on gender diverse persons, along with ethnic origins and la

Assessing feasibility of an adolescent relationship abuse prevention program for girls.

INTRODUCTION: Sexual violence and relationship abuse are prevalent among adolescents and programs promoting gender equity, reproductive justice, and healthy relationships are key strategies for prevention. While such "gender transformative" approaches appear promising for boys, they have not been evaluated among girls. This study assessed the feasibility of this community-based program, called Sisterhood 2.0, among girls in socially disadvantaged urban neighborhoods in Pittsburgh, Pennsylvania. METHODS: This quasi-experimental trial examined feasibility of Sisterhood 2.0 (n = 246), delivered …

Physical activity for public health in the 21st century.

With over 5 million attributed deaths per year, physical inactivity is a major global public health issue. Although the importance of physical activity is well recognized within the scope of obesity and cardiometabolic disease prevention and control, its broader benefits for the health of individuals and societies are yet to be fully harnessed. Furthermore, the role of active leisure, active transport and active labor-primary domains of physical activity-in supporting or hindering social and health equity has been largely overlooked. Here we (1) used a health equity lens to describe global dom

Silences in the System: A Scoping Review Examining How Medical Education Literature Shapes Neurodivergent Learners' Pathways.

Neurodivergent learners represent an estimated 15-20% of the population, yet medical education has been slow to create environments where learners feel safe disclosing neurodivergence. The literature on neurodiversity in medical education remains fragmented, with limited attention to how institutional structures and dominant assumptions shape which learners are rendered visible or invisible in research and practice. To critically examine how the medical education literature describes neurodivergent learners and their pathways to professional legitimacy, and to identify patterns of presence, ab

Adoption and utilization of India's eSanjeevani national telemedicine service.

Telemedicine is increasingly used to expand access to primary healthcare, especially in low- and middle-income countries with workforce and infrastructure constraints. India's eSanjeevani platform, launched in 2019, is the world's largest government-led telemedicine initiative. The objective of this study was to analyse the adoption and utilization patterns of eSanjeevani, India's national telemedicine service, from its national rollout in November 2019 through September 2023. We analysed aggregated programmatic data from the Centre for Development of Advanced Computing and official government

Intersectoral interventions: integration for impact on preterm birth.

PROGRESS: The last two decades have seen a growing focus on intersectoral interventions to improve maternal and newborn health and well-being outcomes, as reflected in efforts to achieve the Millennium Development Goals (MDGs) and advance the Sustainable Development Goals (SDGs). Preterm births are linked to cross-sectoral determinants that affect health outcomes and human capital across the life-course, necessitating an intersectoral approach that addresses these multifaceted challenges. PROGRAMMATIC PRIORITIES: Recognizing that social, biological and economic determinants significantly infl

Having a voice and saving lives: a qualitative survey on employment impacts of people with lived experience of drug use working in harm reduction.

BACKGROUND: Ongoing legal and social discrimination, and stigmatization of people with lived experience of drug use (PWLE) continues to contribute to overdose-related deaths in Canada. The involvement of PWLE working in harm reduction services has proven effective in decreasing drug-related harms among PWLE; however, there exist unintended negative impacts. PWLE working in harm reduction services risk overextending themselves beyond employment parameters (e.g., emotional labor) with few systems in place (e.g., employment advocacy) for support. While meaningful participation of PWLE in harm red

Elizabeth Blackwell (1821-1910): A Pioneering Physician and Trailblazer for Women in Medicine.

Elizabeth Blackwell transformed the medical field in the United States by breaking barriers as the first female physician in the United States. Her determination and resilience challenged the restrictive social norms that confined women to roles considered acceptable at the time. By defying these gender expectations, she became a guiding force for other women seeking to enter medicine. The obstacles she faced fueled her drive to create educational and professional opportunities for women in healthcare. Her work went beyond clinical practice, aiming to empower women and expand their access to t

Women's empowerment and fertility decline among the Pare of Kilimanjaro region, Northern Tanzania.

This research was designed to explore the connection between the empowerment of women and fertility outcomes, through an ethnographic study, a community-based survey and in-depth interviews. The purpose of the work is to test the relationship between a fertility decline and the status of women in a rural area of Kilimanjaro Region, Tanzania. Our major hypothesis was that the decline in fertility in the Kilimanjaro Region-given that the preconditions proposed by Caldwell, Orubuloye, and Caldwell (1992) have been satisfied-is due to the empowerment of women, particularly to gender equity within …

The Scylla State. A gendered understanding of the experiences of marginalised women in the United Kingdom.

Since the formation of the 2012 Coalition government, the UK has been subject to 12 years of neoliberal policy enacted with ferocity and vigour. This has comprised austerity measures including the retrenchment of welfare via the reshaping of the welfare state and public services according to business practices, ideals of individual responsibilisation and overwhelmingly, the notion of reducing the state's ideological and fiscal responsibility for equity and social welfare. The neoliberal state has been conceptualised by Loic Wacquant as a Centaur, boasting a liberal head, yet one atop an author
