During aging, secretion and motility of the upper GI tract slow down. The reduction of these functions, however, does not create complaints. In the higher age groups, a number of symptoms from age-dependent diseases occur more frequently, e.g., dysphagia in response to cerebral ischemia, or disturbed gastric emptying caused by diabetic visceral neuropathy. Moreover, certain GI diseases occur more often in the elderly, e.g., chronic atrophic gastritis, NSAR-induced gastric ulcers, malignancies, and others. In contrast, almost nothing is known about diseases or symptoms of the GI tract that migh

Marked advances in the treatment of neurological disorders which affect the elderly have been established in recent years. Cerebrovascular disorders including stroke and vascular dementia are still among the most frequent diseases in the Japanese elderly. For treatment of hypertensive patients with or without a history of stroke, slight decrease of blood pressure (BP) is recommended since recent PET studies have revealed that an excessive drop of BP markedly decreases cerebral blood flow. Furthermore, 24-hour-monitoring of BP revealed that physiological fluctuation of BP consisting of high day

Norwegian scabies is highly contagious and presents with a psoriasiform dermatosis. It afflicts particularly the elderly and patients with immunosuppression. Two weeks after the admission of an index case of Norwegian scabies to a geriatric ward, 13 and 25 patients and 6 of 18 ward nurses developed scabies. Despite comprehensive treatment, the ward epidemic recurred 6 weeks later probably as a result of inadequate treatment of the index case. This diagnosis should be considered when patients from high-risk groups present with an undiagnosed rash.

The elderly patient, who is phobic or exceedingly anxious established behavior patterns years ago when dentistry was not as advanced as today and when dental experiences were unpleasant and uncomfortable. The elderly phobic patient may view his role as a passive participant as he was in his youth; that is, he fears that the dentist will treat him as he was treated by an unsympathetic dentist years ago. In order to create acceptable, positive experiences for the phobic or anxious patient, the first appointment should give the patient an opportunity to know the dentist and office staff. The dent

A number of scientifically substantiated indications of the application of placental suspension in man of higher age, the positive results of the studies of the effectiveness of placental therapy on metabolic processes and physiological functions of the aging organism, and the experience won in the application of placental suspension in the complex therapy of some diseases frequently occurring at old age allow us to consider this kind of treatment an effective therapy and thus to recommend tissue therapy with a placental suspension for the introduction into geriatric practice.

The authors administered two scales for the evaluation of depression, the Zung SDS and the DFS (Depression Factor Score), derived from the SCL-90, to two groups of medical inpatients: One composed of adult subjects (N = 201) and the other of geriatric subjects (N = 178). The results confirm a high presence of depressive symptoms among patients with medical problems, particularly among geriatric subjects; the prevalence of depression was 20.39% for the adults and 42.13% for the elderly patients. High indices of correlation between the two scales were found for both populations. A multiple regre

In 1980, a geriatric consultation team was formed at Massachusetts General Hospital to meet the complex medical, psychological, rehabilitative, nutritional, and social needs of geriatric patients. This team strives to provide elderly patients with the comprehensive care necessary to potentiate return to maximum independent functioning and to change attitudes of primary caretakers toward care of geriatric patients through increased recognition of the elderly as a heterogeneous group. The team consults on patients with changes in mental status, need for physical rehabilitation, "failure to thriv

A brief review is given of the incidence and nature of psychiatric symptoms, particularly depression, that are associated with chronic physical diseases common in the elderly. A discussion of medical illnesses that mimic depression, and depression presenting as somatic disease, illustrates the challenge of differential diagnosis. Guidelines are presented for the treatment of depression in patients with central nervous system, cardiovascular, hepatic and renal, and gastrointestinal disorders. Pharmacokinetic factors relevant to the use of cyclic and monoamine oxidase inhibitor antidepressants i

This study examines questionnaire data obtained from a group of older persons with sexual dysfunction mistakenly referred through a newspaper column. These 35 respondents (28 men, seven women) were characterized by age (43-82 years), symptomatic sexual problems, and a willingness to write about them. They were chiefly men had varying degrees of erectile failure and were negatively affected by the development of their problems. Two thirds had consulted a physician. The older subjects were more likely to be abstinent, to have total erectile failure (if men), to have known their physicians only a

Dogs and cats that have been perfectly acceptable pets for many years may develop behavioral problems with old age. Most often these problems develop in animals that have never been very well behaved but have been tolerated by the owners until the problem has somehow exacerbated secondary to some other geriatric problem. The behavioral problems of old dogs most likely to be encountered by the practitioner are those of deaf dogs, dogs that house-soil because of renal, cardiovascular or gastrointestinal problems, and dogs that are aggressive because of a change in the number of animals or humans

Regardless of the physical and mental health of any elderly person, verbal communications tend to deteriorate with advancing years. A significant loss of hearing occurs in 30-50 percent of persons over age 65, and voice changes are virtually inevitable after the age of 60. These alterations in the organs of communication increase markedly in incidence and degree after age 70. Deafness is a symptom and not a disease. Presbycusis is the result of degenerative changes affecting the organ of Corti and its central connections. The effects of aging on the expressive system of human communications ma

The death rate of 6662 major gynaecological operations on women of 60 years or more in 17 West-Berlin Gynaecological Departments (1960--1969) was 7,7% (512 cases). Of these, 89 women (16,8%) died either during operation or within 3 days after the operation. In all cases there was an adequate indications for surgical intervention. With few exceptions the procedures of the operations could not be criticized. Only a very small part of the intra- and postoperative mortality could be attributed to the anesthesia. In some cases medication was insulfficient (e.g. lack of preoperative treatment with d
