Retrograde cerebral venous thrombosis (CVT) is a rare complication following internal jugular vein (IJV) ligation. The patient described in this report is a male in his 30s with locally advanced carcinoma tongue. He underwent near-total glossectomy and bilateral neck dissection. Due to heavy nodal burden, his right IJV had to be sacrificed. The patient presented with features of raised intracranial pressure (ICP) postoperatively. Magnetic resonance venogram of the brain revealed CVT involving sigmoid and transverse sinus. This case report describes a perplexing case of right-sided IJV ligation…

Post-resection wound healing, oral microbiota composition and saliva lysozyme activity were assessed in 36 patients with tongue carcinoma before and after surgery with the local application of 2% solution of acid-diluted chitosan in the postoperative period. The microbiocenosis of the oral cavity before treatment in cancer patients was represented by conditionally pathogenic and pathogenic bacteria, whose number is more than 4 lg CFU/cm2, indicating dysbiosis; the lysozyme level was reduced, accounting for 25.4±8.5 µg/ml. After the radical hemi-glossectomy the reduction in the frequency and qu…

UNLABELLED: Tongue cancer significantly impacts speech and swallowing functions, necessitating comprehensive rehabilitation post-surgery. This study evaluates the rehabilitation outcomes of Indian patients who underwent tongue cancer surgery, focusing on speech therapy, nutritional support, and psychosocial interventions. A prospective cohort study was conducted from January 2021 to December 2022 at a tertiary care center in India. Fifty-five patients who underwent partial or hemi-glossectomy were enrolled. Baseline data, including demographic information, medical history, and nutritional stat…

This case report details the innovative use of transoral robotic surgery (TORS) for cricopharyngeal (CP) myectomy, a procedure that has never been documented for this condition. Our patient, who was previously treated for tongue cancer by total glossectomy, experienced persistent dysphagia and received gastric tube feeding despite multiple rehabilitations. After botulinum toxin injection confirmed CP muscle spasm, a TORS CP myectomy was performed using the da Vinci Si robotic system. The procedure involved a minimally invasive transoral approach, providing high-magnified 3D panoramic visualiza…

PURPOSE: Anterolateral thigh free flap (ALTFF) is a versatile option for tongue reconstruction after cancer resection. Compartmental tongue surgery (CTS) is a surgical technique whose purpose is to remove the entire oncological compartment with the pathways of tumor spread. Extended glossectomies (EG) follow the same surgical steps and anatomical concepts as CTS but extend beyond hemiglossectomy. The surgical defect following such resections often necessitates the use of a large free flap, with the ALTFF being the most commonly used.
METHODS: The CTS and EG are anatomically-based approaches t…

Dentists are aware of the extraordinary influence an active thrusting tongue can have on dental development. In the presence of the profound muscle hypertrophy characteristic of macroglossia in the Beckwith-Wiedemann syndrome, a compelling need exists for surgical reduction of the tongue as early in life as possible. This report describes our clinical experience with 3 children, 1 referred late and the other 2 treated within the first year of life.

The results of reduction of tongue size in connection with orthodontic and surgical correction of prognathic anomalies of the jaws are presented. It is shown that reduction of the tongue size combined with orthodontic treatment arrests of tendency to prognathic growth when it is performed early. The results further indicate that the effect of prophylactic reduction of tongue size to check relapse following surgical correction of prognathism or open bite can only be confirmed in cases of genuine macroglossia. Since there was only one relapse in our cases without reduction of tongue size, the in

A 45-year-old male with tongue cancer and obstructive sleep apnea presented for glossectomy and right neck dissection. He underwent a difficult nasal intubation assisted by a bougie and Glidescope. After an otherwise uneventful procedure, the patient was extubated and taken to recovery. Several hours later, he developed increased respirations and decreased oxygen (O2) saturation with decreased air movement on the right side of his chest. A chest X-ray confirmed a right pneumothorax. A chest tube was placed with immediate improvement of O2 saturation and breathing. Pneumothorax was presumably d…

Neuroendocrine carcinoma usually originates from lung. Few data exist in the literature regarding neuroendocrine carcinoma of the tongue. Patient data including history, surgical procedure, histology, and radiology investigations were collected and summarized. A 40-year-old woman was referred after partial glossectomy. Squamous mucosa with neoplasm and cells with round nuclei and light cytoplasm was reported in the tongue biopsy. Immunohistochemistry (IHC) staining was positive for cytokeratin, neuron specific enolase, synaptophysin and chromogranin and negative for leukocyte common antigen. T…

A 33-year-old woman sought medical care at our institution for macroglossia. She had been treated before with tracheostomy and gastrostomy due to an adenoid cystic carcinoma (ACC) of the mobile tongue, in clinical stage IVC. She was subjected to total glossectomy with larynx preservation, modified radical dissection of the right neck and of the left supraomohyoid muscle, as well as reconstruction using a free flap from the rectus abdominus. The histological report described a 15x11 cm solid type ACC of the mobile tongue (grade III), with infiltration to adjacent soft tissues, vascular and lymp…

Many methods have been reported to select the proper candidates for uvulopalatopharyngoplasty (UPPP) among obstructive sleep apnea patients. Polysomnography with intraluminal pressure recordings in the upper airway was found useful to predict obstructive site(s) and hence the success of surgery. According to the pressure analysis, 11 cases with high negative pressure at the mesopharynx were selected for UPPP, while UPPP combined with midline partial glossectomy (UPPP-MLG) was done in 18 cases proved to have high negative pressure difference between the mesopharynx and esophagus. A reduction of…

Mixed connective tissue disease (MCTD) is an autoimmune disorder characterized by overlapping features of multiple connective tissue diseases in addition to anti-U1-ribonucleoprotein (RNP) antibodies and Raynaud's phenomenon. A 53-year-old female patient with MCTD, interstitial pneumonia, and idiopathic thrombocytopenic purpura underwent partial tongue resection under general anesthesia. Perioperative management included steroid coverage and careful titration of the oxygen concentration. Although venous access was difficult, the anesthetic course was uneventful. MCTD can present perioperative

Operable oral tongue cancers are managed best with surgery followed by adjuvant therapy as and when indicated. The only factor that affects the prognosis, and is under the control of a surgeon, is the tumour margin. Often in cases with trismus, which is prevalent in tobacco and areca nut users, obtaining a clear margin intraorally can be challenging. Approaches described until now to obtain clear margins in these cases have a great impact on the postoperative recovery and morbidity. In our experience, lingual sulcus release is a safe and reproducible technique for these cases. The transcervica

Some classical textbooks of anatomy provided a detailed description of the lingual nodes, which are small inconstant lymph nodes in the floor-of-mouth and the upper neck. The clinical importance of these lymph nodes in cancer therapy, however, has been underestimated so far. We previously reported an extremely poor prognosis of oral tongue cancer patients who had lesions at the root of the lingual artery and assumed that metastases in occult lingual nodes might be responsible for such lesions. This case report clearly demonstrates the distinctive draining course of the lateral lingual nodes, w

Lymphangiomas are benign congenital tumors that frequently result in macroglossia. Tongue involvement may lead to airway obstruction, swallowing difficulties, mandible-dental deformities, and speech disturbances. This case report demonstrates the many difficulties in diagnosis and management. Reduction in tongue size with preservation of motor and sensory function is best accomplished by wedge resection of the tip of the tongue.
