[Echographic follow-up of the chamber angle in suspected simple glaucoma].

We have recently seen a number of patients with angle-closure glaucoma who had been treated for glaucoma simplex (primary open-angle glaucoma). The main cause of this misdiagnosis was gonioscopy, as its findings are subject to the impression of the examiners. For objective examination of the chamber angle and the iris configuration, B-scan ultrasonography was performed using a contact eye cup filled with saline. Two groups of patients were noted in particular. The first group contained relatively young patients, more men than women, with plateau iris and a central normal, deep anterior chamber…

Primary congenital glaucoma clinical presentation and evaluation of modified trabeculectomy.

Primary congenital glaucoma is a relatively rare condition. A study on this subject to observe the mode of clinical presentation in our country and to evaluate the role of modified technique of trabeculectomy was undertaken. 30 eyes of 17 patients having primary congenital glaucoma were studies. The examination under general anaesthesia included (a) Anterior segment examination under operating microscope, (b) Corneal diameter measurement, (c) Fundus examination (d) IOP measurement and (e) Gonioscopy. The patients were subject to modified trabeculectomy operation. The assessment of the surgery …

[Glaucoma in dogs and cats. Review and retrospective evaluation of 421 patients. I. Pathobiological background, classification and breed predisposition].

Glaucoma is diagnosed frequently in dogs and cats. A review of case records at the University of Utrecht Small Animal Clinic revealed that during a 4-year period glaucoma accounted for 8.6% of all ophthalmological diagnoses. Glaucoma is defined as an increased intraocular pressure, which sooner or later leads to loss of function of the eye. Primary and secondary types can be distinguished. In primary glaucoma an increased intraocular pressure exists in the absence of other primary eye disorders. By gonioscopy a discrimination can be made between open and narrow or closed anterior chamber angle…

Early to late explantation of Hydrus microstent MIGS device: A case series.

PURPOSE: The Hydrus microstent was approved by the FDA in August 2018 for use with cataract surgery to reduce IOP in patients with mild to m … a (CME) and uncontrolled IOP in the left eye after cataract extraction with Hydrus implantation. On gonioscopy, the implant was occluded and embedded in the iris. The patient underwent removal of the Hydrus implant 10 months after the initial surgery with canaloplasty to control IOP.Case 2: A 71-year-old male patient on dual anti-platelet developed intraoper …

Diagnostic performance of anterior segment optical coherence tomography in detecting plateau iris.

OBJECTIVE: To evaluate the diagnostic performance of manual grading of anterior segment optical coherence tomography (AS-OCT) in detection of plateau iris configuration (PIC) based on the presence of standardised ultrasound biomicroscopy (UBM) criteria in at least two quadrants; namely, clinical diagnosis of PIC (DxPIC). METHODS AND ANALYSIS: In this cross-sectional study, paired AS-OCT and UBM images were evaluated by three glaucoma specialists. AS-OCT was classified into two mechanisms, PIC versus non-PIC, of primary angle closure disease (PACD) and AS-OCT-PIC diagnostic performance was tes

Glaucoma Progression After Lens Extraction in Primary Angle-closure Glaucoma According to Angle-closure Mechanism.

PRCIS: Long-term prognosis of primary angle-closure was assessed after lens extraction (LE) according to different mechanisms of angle closure (AC). Patients with pupillary block (PB) had a higher probability of glaucomatous progression than patients with plateau iris configuration (PIC). PURPOSE: The aim was to investigate and compare the clinical characteristics and long-term prognosis of primary angle-closure disease (PACD) after LE according to different mechanisms of AC. METHODS: In this retrospective observational cohort study, 118 eyes with PACD that underwent LE (mean follow-up; 6.0±

[Primary anterior chamber angle closure: progression from suspect to glaucoma. Part 1. Frequency and rate of transition from suspected primary angle closure to true angle closure and primary angle closure glaucoma].

To review the literature devoted to the problem of primary anterior chamber angle closure (PAC) and the development of this pathology from glaucoma suspect to primary angle closure glaucoma. The paper includes a trend analysis of the studies concerning primary angle closure suspects (PACS). The concept of this review is conditioned by the conflicting strategies for treating patients with initial PAC without glaucomatous optic neuropathy. Solving the problem of angle closure plays a key role in preventing the development of PAC glaucoma, which is the world's leading cause of irreversible blindn

[Advanced closed-angle glaucoma with intercalary staphylomas].

The case of a 70-year-old woman displaying a quasiabsolute glaucoma with large scleral juxtalimbic ectasias at both eyes is reported. The gonioscopic examination shows a closed-angle glaucoma developing for about 10 years with subacute crises, neglected by the patient. Vision at the right eye 1/50 nc, intraocular pressure at the right eye = 34.4 mmHg vision at the left eye = 3/50 nc, intraocular pressure at the left eye = 41.4 mmHg. The ocular examination reveals a small anterior chamber, sectorial iridic atrophies, a mydriatic pupil, the camerular angle closed. The fundus of the eye shows the

Diagnostic evaluation of the patient suspected of having glaucoma.

From the preceding it can be seen that the clinician still has little to help him decide which of his patients with elevated intraocular pressure will advance to frank glaucomatous disease. There are no objective provocative tests that will reliably predict the future. Consequently, one must obtain as much data as possible about an individual patient and, combining all these facts, have either a high or low degree of suspicion. Once a patient is noted to have elevated pressure, however, he must remain a glaucoma suspect, perhaps for the rest of his life. Only the subsequent examination will de

Corticosteroid response in dominant congenital glaucoma.

The parents of six children with goniodysgenesis and hereditary infantile congenital glaucoma were tested with corticosteroid eye-drops. Their response on provocation demonstrated a clear trend: In five families one parent of each couple responded with a tensional rise of 9 mmHg or more, whereas the other parent responded with a rise of 9 mmHg or less. Of the six parents with the greater response, five had glaucoma in their ancestry and displayed a distinct goniodysgenesis. The findings support the previously presented view that hereditary infantile congenital glaucoma is caused by a dominant

Secondary glaucoma due to inactive congenital syphilitic interstitial keratitis. (With 1 colour plate).

35 cases of secondary glaucoma due to congenital syphilitic interstitial keratitis have been surveyed. In four cases which had no opacities in the cornea, but with positive serologic reaction for syphilis, the characteristic goniscopical features such as peripheral anterior synechia, pigment deposits in the trabeculum, and irregularity of iris configuration as observed commonly in cases with inactive congenital interstitial syphilitic keratitis, were found. In the clinical course and goniscopical findings, they were divided into two types: one was the angle closure type with acute glaucomatous

Iris lesions: A review.

Iris lesions encompass a broad spectrum of entities ranging from benign pigmentary variants to primary malignancies and secondary infiltrative or metastatic disease. Although many are discovered incidentally and remain asymptomatic, certain lesions may exhibit growth, seeding, hyphema, pigment dispersion, or angle involvement, leading to secondary glaucoma and vision loss. We synthesize contemporary evidence on melanocytic, non-melanocytic, and cystic iris lesions. We highlight a multimodal diagnostic approach incorporating slit-lamp biomicroscopy with serial photography, gonioscopy, iris tran…
