Problem substance use among depressed patients in managed primary care.

This study identifies characteristics associated with problem substance use among 1,187 patients with either depressive symptoms (44%) or depressive disorders (56%) in primary care clinics of six managed care organizations. Sedative misuse (reported by 14% of all patients) was associated with greater wealth, social phobia, and misuse of prescription opioids. Cannabis use (11%) was associated with younger age, male gender, single marital status, white ethnicity, less education, recurrent depression, agoraphobia, and hazardous alcohol use. Hazardous drinking (11%) was significantly associated wi

Update on occupational dermatitis: reviewing toxic substances from OSHA standards.

Occupational dermatitis (OD) is an inflammatory skin disease stemming from exposure to specific substances within a work setting. As the second most prevalent occupational health concern in 2020, affecting 1.8 per 10,000 workers, OD poses a significant challenge to workforce well-being and imposes a substantial economic burden through lost wages, decreased productivity, and increased healthcare spending. Dermatologists emerge as pivotal figures in recognizing risk factors and delivering essential care to individuals with OD. This review focuses on chemical hazards and toxic substances regulate

Exocrine pancreatic cancer and living near to waste sites containing hazardous organic chemicals, New York State, USA - an 18-year population-based study.

OBJECTIVES: The etiology of exocrine pancreatic cancer (EPC) remains unknown except for family history and smoking. Despite recent medical advances, rates of pancreatic cancer incidence and mortality are increasing. Although existing evidence suggests a potentially causal relationship between environmental chemical exposures and pancreatic cancer, whether residential exposure impacts pancreatic cancer rates remains unknown. MATERIAL AND METHODS: The authors identified 28 941 patients diagnosed with exocrine pancreatic cancer in New York State exclusive of New York City for the years 1996-2013

Increasing UV-B radiation at the earth's surface and potential effects on aqueous mercury cycling and toxicity.

In the past two decades, a great deal of attention has been paid to the environmental fate of mercury (Hg), and this is exemplified by the growing number of international conferences devoted uniquely to Hg cycling and its impacts on ecosystem functions and life. This interest in the biogeochemistry of Hg has resulted in a significant improvement of our understanding of its impact on the environment and human health. However, both past and current research, have been primarily oriented toward the study of direct impact of anthropogenic activities on Hg cycling. Besides a few indirect effects su

Association between contact with mental health and substance use services and reincarceration after release from prison.

OBJECTIVE: People released from prison who experience mental health and substance use problems are at high risk of reincarceration. This study aimed to examine the association between contact with mental health and substance use treatment services, and reincarceration, among adults released from prison. METHODS: Pre-release survey data from 1,115 adults released from prisons in Queensland, Australia were linked with administrative health and correctional records covering a median of 787 days post-release. We constructed marginal structural Cox proportional hazards models, adjusting for pre-re

Using the Alcohol, Smoking and Substance Involvement Screening Test to predict substance-related hospitalisation after release from prison: A cohort study.

BACKGROUND AND AIMS: Poor substance use-related health outcomes after release from prison are common. Identifying people at greatest risk of substance use and related harms post-release would help to target support at those most in need. The Alcohol Smoking and Substance Involvement Screening Test (ASSIST) is a validated substance use screener, but its utility in predicting substance-related hospitalisation post-release is unestablished. We measured whether screening for moderate/high-risk substance use on the ASSIST was associated with increased risk of substance-related hospitalisation. DES

Alcohol and illicit drug use among young people living with HIV compared to their uninfected peers from the Kenyan coast: prevalence and risk indicators.

BACKGROUND: In sub-Saharan Africa, there is paucity of research on substance use patterns among young people living with HIV (YLWH). To address the gap, we sought to: i) determine the prevalence of substance use, specifically alcohol and illicit drug use, among YLWH compared to their HIV-uninfected peers; ii) investigate the independent association between young people's HIV infection status and substance use; iii) investigate the risk indicators for substance use among these young people. METHODS: Between November 2018 and September 2019, a cross-sectional study was conducted at the Kenyan c

Structured expert judgement approach of the health impact of various chemicals and classes of chemicals.

INTRODUCTION: Chemical contamination and pollution are an ongoing threat to human health and the environment. The concern over the consequences of chemical exposures at the global level continues to grow. Because resources are constrained, there is a need to prioritize interventions focused on the greatest health impact. Data, especially related to chemical exposures, are rarely available for most substances of concern, and alternate methods to evaluate their impact are needed. STRUCTURED EXPERT JUDGMENT (SEJ) PROCESS: A Structured Expert Judgment (Research Outreach, 2021) process was perform

Tobacco cessation prescription rates among Veterans living with human immunodeficiency virus who smoke.

Introduction: Cigarette smoking remains the leading preventable cause of death in the United States, with prevalence remaining dangerously high in at-risk groups, including people living with Human Immunodeficiency Virus (HIV). Military Veterans with HIV and receiving Veterans Health Administration (VHA) care have a high prevalence of smoking, and risk factors such as hazardous drinking and substance use disorder (SUD), despite access to smoking cessation treatment. Little is known about the prevalence of smoking and smoking cessation medication receipt among Veterans with HIV and co-occurring

Effect of substance use on premature mortality among severely hypertensive African Americans.

Low-income African Americans residing in impoverished neighborhoods confront myriad barriers to adhering to antihypertensive regimens. Substance use may thwart medication adherence and lifestyle modification efforts, which has implications for excess cardiovascular disease mortality. The Inner-City Hypertension and Body Organ Damage (ICHABOD) Study was a longitudinal cohort study that evaluated causes of mortality among African Americans who lived in urban areas, had severe, poorly controlled hypertension, and were admitted to a local hospital between 1999-2001 and 2002-2004. The authors emplo

Mapping outdoor habitat and abnormally small newborns to develop an ambient health hazard index.

BACKGROUND: The geography of where pregnant mothers live is important for understanding outdoor environmental habitat that may result in adverse birth outcomes. We investigated whether more babies were born small for gestational age or low birth weight at term to mothers living in environments with a higher accumulation of outdoor hazards. METHODS: Live singleton births from the Alberta Perinatal Health Program, 2006-2012, were classified according to birth outcome, and used in a double kernel density estimation to determine ratios of each outcome per total births. Machine learning for monitoring and assessment of potentially toxic elements in soils: a synthesis of spatial validation, explainability, and uncertainty.

Potentially toxic elements (PTEs) in soils pose persistent risks to ecosystems, groundwater, and food systems, creating a need for reliable spatial assessment tools. Machine learning (ML) is increasingly used to map PTE concentrations from environmental covariates, but many studies still rely on spatially naive validation, limited interpretation, and incomplete uncertainty reporting. This review synthesizes recent advances (2020-2025) in ML-based PTE mapping with emphasis on four requirements for monitoring-grade assessment: spatially honest validation, explainable artificial intelligence, unc

Emergency management and HAZMAT mitigation: Strategic planning for new developments.

In cases of accidental hazardous materials (HAZMAT) release, several emergency management responses can be taken to maintain the safety of the living environment. This study focuses on building a long-range planning, urban development model for hazards addressing where development zones should be located to mitigate exposure to HAZMAT and minimize evacuations. Interviews and meta-analytic manipulation of data were used to demonstrate flaws in three industrial cities' disaster management plans. These data were also used to influence development in plain model areas where hydrogen sulfide or oth

Evaluation of Measures against Exposure during Administration of Hazardous Drugs through a Feeding Tube.

Hazardous drugs (HD), which need to be handled with care, may be administered through a feeding tube using the simple suspension method. However, instrument contamination during HD administration with the simple suspension method remains unclear. Therefore, to minimize such contamination during the simple suspension method using an injector, we propose the following exposure countermeasures method: (1) Wear two layers of gloves. (2) Prepare injectors for administration and flushing. (3) Use caps. (4) Replace outer gloves after the removal of tablets from the press-through package (PTP) sheet.

A critical review of the application of polymer of low concern and regulatory criteria to fluoropolymers.

Per- and polyfluoroalkyl substances (PFAS) are a group of fluorinated substances that are in the focus of researchers and regulators due to widespread presence in the environment and biota, including humans, of perfluorooctane sulfonate (PFOS) and perfluorooctanoic acid (PFOA). Fluoropolymers, high molecular weight polymers, have unique properties that constitute a distinct class within the PFAS group. Fluoropolymers have thermal, chemical, photochemical, hydrolytic, oxidative, and biological stability. They have negligible residual monomer and oligomer content and low to no leachables. Fluoro
