Treatment of Plagiocephaly With Orthotic Helmets: Prevalence, Costs, and Inequities in Massachusetts.

Positional plagiocephaly is a craniofacial condition resulting from infant positioning that applies pressure consistently to a part of the skull. This study analyzed the prevalence of, and costs associated with orthotic helmet treatment for positional plagiocephaly and assessed inequities in treatment between medically underserved areas (MUAs) and non-MUAs using health insurance claims data from the Center for Health Information and Analysis (CHIA) in Massachusetts for the years 2016-2021. The mean percentage of patients receiving orthotic helmet therapy was 4.3% (SD=0.49, 95% CI=3.88-4.91, P

Comparison of a modifiable cranial cup versus repositioning and cervical stretching for the early correction of deformational posterior plagiocephaly.

BACKGROUND: Deformational occipital flattening is the most common cause of abnormal head shape. Current treatment calls for repositioning and cervical stretching in infants younger than 5 months. The authors assessed the effectiveness of a modifiable cranial cup compared to repositioning and cervical stretching to correct early deformational plagiocephaly. METHODS: This nonrandomized, prospective clinical trial used historical controls. The treatment and control groups consisted of infants, younger than 4 months, referred for deformational posterior plagiocephaly. The control children were ma

Helmet therapy for positional cranial deformations; a 5-year follow-up study.

The aim of this study was to assess the long-term effects of helmet therapy in its use for positional cranial deformation, such as plagiocephaly and brachycephaly, by evaluating head shapes in children with a 5-year interval. These children were included at a neonatal age, with a deviating cranial shape classified as positional deformation, and received different forms of therapy, varying from none to physical and helmet therapy combined. Cranial shape was measured before and after therapy, using plagiocephalometry. Both positional plagiocephaly and positional brachycephaly decrease significan

Rate of craniometric change following suture release in patients with metopic and sagittal craniosynostosis.

OBJECTIVE: Craniosynostosis is a congenital disorder resulting from the premature fusion of cranial sutures in the infant skull. This condition results in significant cosmetic deformity and can impede neurodevelopment, if left untreated. Currently, rates of craniometric change following minimally invasive surgery have only been examined for sagittal craniosynostosis. A better understanding of postoperative skull adaptations in other craniosynostosis subtypes is needed to objectively categorize surgical outcomes and guide length of cranial orthosis therapy. METHODS: Eleven patients with sagitt

A case-control study of the effectiveness of bicycle safety helmets.

Bicycling accidents cause many serious injuries and, in the United States, about 1300 deaths per year, mainly from head injuries. Safety helmets are widely recommended for cyclists, but convincing evidence of their effectiveness is lacking. Over one year we conducted a case-control study in which the case patients were 235 persons with head injuries received while bicycling, who sought emergency care at one of five hospitals. One control group consisted of 433 persons who received emergency care at the same hospitals for bicycling injuries not involving the head. A second control group consist

Safe spontaneous breathing with helmet noninvasive ventilation in acute hypoxemic respiratory failure.

PURPOSE OF REVIEW: Helmet noninvasive ventilation (NIV) has gained attention for the management of hypoxemic patients, owing to physiological and potential clinical benefits. We summarize the recent advances on the topic. RECENT FINDINGS: Compared to facemasks, helmets facilitate application of higher positive end-expiratory pressure (PEEP) for prolonged treatments: this improves oxygenation and may mitigate injurious inflation patterns related to lung heterogeneity. The large, highly compliant interface reduces ventilator triggering performance, causing pressure support to be partially out o

All-terrain vehicle injuries in children and adolescents.

QUESTION: I know that all-terrain vehicles (ATVs) are commonly used for recreational purposes, and that recently an adolescent was injured while driving one. What are the dangers of ATV use among adolescents, and what are some preventative measures to recommend to families in my clinic to reduce ATV-related injuries in children? ANSWER: There is a disproportionately high rate of ATV accidents in children compared with adults, and the safety of ATVs has become a public health concern in recent years because of their association with increased pediatric morbidity and mortality. Children account

The association between Guardian Cap use during practices and sport-related concussion risk in high school American football players.

OBJECTIVE: Guardian Caps (GCs) have been widely implemented at all levels of American football participation based on laboratory evidence that they may reduce head impact forces. The purpose of this study was to determine if GC use during practice was associated with a lower risk of sport-related concussion (SRC) during practices and games among high school football players. METHODS: Data were collected on 2610 athletes from 41 Wisconsin schools that self-selected into GC or Non-GC cohorts. Athletic trainers recorded practice and game exposures and SRCs through the 2023 season. Generalised li

Internet of things based smart framework for the safe driving experience of two wheelers.

Several parameters affect our brain's neuronal system and can be identified by analyzing electroencephalogram (EEG) signals. One of the parameters is alcoholism, which affects the pattern of our EEG signals. By analyzing these EEG signals, one can derive information regarding the alcoholic or normal stage of an individual. Many road accident cases around the world, including drinking and driving scenarios, which result in loss of life, have been reported. Another reason for such incidents is that riders avoid wearing helmets while driving two-wheelers. Many road accident cases involving two-wh

Peaks and Distributions of White Matter Tract-related Strains in Bicycle Helmeted Impacts: Implication for Helmet Ranking and Optimization.

Traumatic brain injury (TBI) in cyclists is a growing public health problem, with helmets being the major protection gear. Finite element head models have been increasingly used to engineer safer helmets often by mitigating brain strain peaks. However, how different helmets alter the spatial distribution of brain strain remains largely unknown. Besides, existing research primarily used maximum principal strain (MPS) as the injury parameter, while white matter fiber tract-related strains, increasingly recognized as effective predictors for TBI, have rarely been used for helmet evaluation. High speed, high risk: the rise of e-scooter injuries in adolescents.

The popularity of eco-friendly electric scooters has led to a rise in significant injuries, especially in adolescents. Teens riding e-scooters are more prone to severe traffic accidents and head injuries compared to those using traditional scooters. Risk-taking behaviors, susceptibility to peer influence, limited road safety experience and a lack of protective headgear exacerbate the risks. These injuries impose a significant burden to patients and to the health care system. To mitigate these risks, age and helmet laws should be strictly enforced, with enhanced public education, safety trainin

Evaluation of an Elastomeric Honeycomb Bicycle Helmet Design to Mitigate Head Kinematics in Oblique Impacts.

Head impacts in bicycle accidents are typically oblique to the impact surface and transmit both normal and tangential forces to the head, causing linear and rotational head kinematics, respectively. Traditional expanded polystyrene (EPS) foam bicycle helmets are effective at preventing many head injuries, especially skull fractures and severe traumatic brain injuries (TBIs) (primarily from normal contact forces). However, the incidence of concussion from collisions (primarily from rotational head motion) remains high, indicating need for enhanced protection. An elastomeric honeycomb helmet des

Role of helmet ventilation during the 2019 coronavirus disease pandemic.

Coronavirus disease 2019 (COVID-19) has been declared a pandemic by the World Health Organization; it has affected millions of people and caused hundreds of thousands of deaths. Patients with COVID-19 pneumonia may develop acute hypoxia respiratory failure and require noninvasive respiratory support or invasive respiratory management. Healthcare workers have a high risk of contracting COVID-19 while fitting respiratory devices. Recently, European experts have suggested that the use of helmet continuous positive airway pressure should be the first choice for acute hypoxia respiratory failure ca
