We present an electrocardiogram with triphasic QRS morphology variation and discuss the mechanism and clinical implications for this finding.

Catheter ablative techniques have introduced an exciting new chapter of interventional therapy for patients with drug-resistant cardiac arrhythmias. A variety of techniques are presently available, and proper choice depends on proper localization of the tachycardia focus and mechanism of the arrhythmia. These techniques should be performed only in laboratories with personnel experienced in invasive electrophysiologic procedures. Over the past 5 years, we have learned that catheter ablation of the AV junction for patients with drug-resistant supraventricular tachycardia has replaced the need fo

Sudden death is a frequent mode of fatal outcome in cardiac disease and does not exclude young people. The aim of this investigation was to establish whether and to what extent sudden death in the young may be ascribable to the substrate of underlying congenital heart disease. Among 182 young people (< or = 35 years) who died of cardiac sudden death and underwent postmortem examination, 58 (32%) had congenital heart disease. Seven showed an intrapericardial rupture of aortic dissection, in the setting of Marfan syndrome in two, isolated bicuspid aortic valve in two, and bicuspid aortic valve a

The number of patients undergoing transcatheter aortic valve replacement (TAVR) worldwide is increasing steadily. Atrioventricular conduction disturbances, with or without the need for permanent pacemaker (PPM) implantation, are one of the most common adverse events after TAVR. Among transcatheter heart valves (THV), rates of conduction abnormalities vary from less than 10 % to more than 50 %. Depending on the reported data referred to, historical data showed that up to one-third of the patients required implantation of a PPM following TAVR. Although generally considered as a minor complicatio

Verapamil is considered by many investigators to be the drug of choice for the acute management of uncomplicated PSVT. Several clinical investigators have demonstrated termination of PSVT in more than 90% of their patients within minutes following IV drug administration. The incidence of reported severe adverse reactions has been less than 1%. PSVT may be complicated by underlying heart disease, or by antegrade accessory pathway conduction in individuals with pre-excitation syndrome. Such conditions, or the prior use of beta-blocking agents, may contraindicate the use of verapamil. However, th

INTRODUCTION: The cardiac conduction system (CCS) is crucial for maintaining adequate cardiac frequency at rest and modulation during exercise. Furthermore, the atrioventricular node and His-Purkinje system are essential for maintaining atrioventricular and interventricular synchrony and consequently maintaining an adequate cardiac output.
AREAS COVERED: In this review article, we examine the anatomy, physiology, and pathophysiology of the CCS. We then discuss in detail the most common genetic mutations and the molecular mechanisms of cardiac conduction disease (CCD) and provide our perspecti
