OBJECTIVE: To describe the functional and anatomic heart abnormalities of patients with dermato-polymyositis.
METHODS: Thirty-two consecutive patients with dermato-polymyositis (DM/PM) were studied. Patient assessments included a questionnaire for cardiac symptoms, physical examination, muscular enzymes, standard electrocardiogram (EKG), chest radiographs, spirometry, Holter monitoring and color Doppler flow (CDF) studies. Systolic heart murmurs caused by chronic mitral valve disease are particularly common in Cavalier King Charles Spaniels (CKCS) in Great Britain. To determine if American-bred CKCS have a similar high prevalence of chronic valve disease, results of stethoscopic examinations on 394 CKCS were analyzed. In 7/340 patients (2.1%) examined by transesophageal echocardiography, aneurysms of the intraatrial septum were observed. In no patient were additional shunts present. In 3/7 patients mitral valve and in 2/7 patients tricuspidal valve prolapse was found. Intraatrial septum aneurysms seem to have a higher frequency than that indicated by transthoracical echocardiography. The incidence observed by transesophageal echocardiography corresponds to the incidence of pathological findings.

Although myasthenia gravis (MG) is frequently mentioned in standard textbooks and journal articles as a rare cause for pulmonary hypertension and right heart failure, no case can actually be found in the literature. The case described in this report is the first documented case of chronically decompensated MG manifesting itself as pulmonary hypertension, severe right heart failure, and functional prolapse of both the mitral and tricuspid valves. Interestingly, no hepatic biochemical abnormalities were present in spite of significant congestive hepatomegaly.
