Safety of conventional and wireless capsule endoscopy in patients supported with nonpulsatile axial flow Heart-Mate II left ventricular assist device.

BACKGROUND: Left ventricular assist devices (LVADs) are increasingly being used as a bridge for cardiac transplantation in patients with decompensated cardiac function. A known complication of these devices is severe and sometimes life-threatening GI bleeding, usually related to the presence of angioectasias. Endoscopy has been generally accepted as safe in patients with cardiac disease and implanted cardiac devices, when it is performed with appropriate monitoring. However, the literature is sparse regarding the safety of endoscopy in patients with LVADs. OBJECTIVE: Given the potential risks

Persistence of Cheyne-Stokes breathing after left ventricular assist device implantation in patients with acutely decompensated end-stage heart failure.

Three patients, who were admitted for acute decompensation of end-stage congestive heart failure (CHF), underwent nocturnal polysomnography (PSG) before, and from 7 to 80 days after, left ventricular assist device (LVAD) implantation. Moderate to severe sleep-disordered breathing (SDB) was diagnosed in all 3 patients within 48 hours of admission, consisting predominantly of Cheyne-Stokes breathing (CSB) with central sleep apnea. After LVAD implantation, despite improved hemodynamics and end-organ function, the patients continued to have moderate or severe CSB, although there was decreased time

Predictors of 30-day post-transplant mortality in patients bridged to transplantation with continuous-flow left ventricular assist devices--An analysis of the International Society for Heart and Lung Transplantation Transplant Registry.

BACKGROUND: Continuous-flow (CF) left ventricular assist devices (LVADs) are standard of care for bridging patients to cardiac transplantation. However, existing data about preoperative factors influencing early post-transplant survival in these patients are limited. We sought to determine risk factors for mortality using a large international database. METHODS: All patients in the International Society for Heart and Lung Transplantation Transplant Registry who were bridged to transplantation with CF LVADs between June 2008 and June 2012 were included. Risk factors for mortality within 30 day

Single-beat right ventricular-pulmonary artery coupling and residual heart failure in patients with a HeartMate 3 left ventricular assist device.

BACKGROUND: Contemporary left ventricular assist devices (LVADs) improve survival, yet residual heart failure (HF) remains despite optimized device support. The extent to which right ventricular (RV)-pulmonary artery (PA) interaction, measured by single-beat elastance-based indices, identifies residual risk is unknown. METHODS: We studied 70 adults implanted with HeartMate3 LVADs (2018-2022) who underwent pre-discharge right heart catheterization after echocardiographic and invasive LVAD optimization. Using a single-beat method, RV end-systolic elastance (Ees), PA effective arterial elastance

Rehabilitation Course Through Compassionate Use of JenaValve Trilogy Heart Valve System in a Patient With Left Ventricular Assist Device: A Case Study.

Aortic insufficiency (AI) develops in approximately one-third of patients 3 years after continuous-flow left ventricular assist device (LVAD) implantation. Transcatheter aortic valve replacement (TAVR) has recently emerged as a potential option. However, the use of TAVR in patients with LVADs is limited. There are currently only two transcatheter valves, J-Valve and Trilogy Valve, available under expanded access for aortic regurgitation, particularly for patients with LVAD. This case describes a 47 year old male with destination HeartMate 3 LVAD complicated by methicillin-susceptible Staphyloc

Heart Transplant Outcomes in Chemotherapy-Induced vs. Non-Ischemic-Dilated Cardiomyopathy: Pediatric and Adult Recipients in the Ventricular Assist Device Era.

BACKGROUND: Chemotherapy-induced dilated cardiomyopathy (CIDCM) has become an increasingly recognized indication for heart transplantation (HT) among cancer survivors with end-stage heart failure (HF). Advances in cardio-oncology practices, mechanical circulatory support, and refined immunosuppression strategies have improved outcomes; however, comparative data with non-ischemic dilated cardiomyopathy (NIDCM) in the modern ventricular assist device (VAD) era remain limited. Therefore, this study primarily aimed to compare post-transplant outcomes between CIDCM and NIDCM within pediatric and ad

A Learning Curve is Associated With Combined Hybrid Procedure and Single Ventricle-Ventricular Assist Device Insertion in Neonates With Hypoplastic Left Heart Syndrome.

ObjectivesA minority of patients with hypoplastic left heart syndrome (HLHS) are at extremely high risk for staged palliation and can be bridged-to-heart transplantation with bilateral pulmonary artery bands, ductal stenting, and single ventricle-ventricular assist device insertion (HYBRID + sVAD). The purpose of this analysis is to assess our learning curve associated with our first ten patients with functionally univentricular ductal-dependent systemic circulation who were supported with primary HYBRID + sVAD as bridge-to-heart transplantation.MethodsPatients were temporally separated into t

Hemodynamic Changes After Left Ventricular Assist Device Implantation Among Heart Failure Patients With and Without Elevated Pulmonary Vascular Resistance.

BACKGROUND: Left ventricular assist devices (LVADs) may reverse elevated pulmonary vascular resistance (PVR) which is associated with worse prognosis in heart failure (HF) patients. We aim to describe the temporal changes in hemodynamic parameters before and after LVAD implantation among patients with or without elevated PVR. METHODS: HF patients who received continuous-flow LVAD (HeartMate 2&3) at a tertiary medical center and underwent right heart catheterization with PVR reversibility study before and after LVAD surgery. Patients were divided into 3 groups: normal PVR (<4WU); reversible PV

Comparison of Evaluations for Heart Transplant Before Durable Left Ventricular Assist Device and Subsequent Receipt of Transplant at Transplant vs Nontransplant Centers.

IMPORTANCE: In 2020, the Centers for Medicare & Medicaid Services revised its national coverage determination, removing the requirement to obtain review from a Medicare-approved heart transplant center to implant a durable left ventricular assist device (LVAD) for bridge-to-transplant (BTT) intent at an LVAD-only center. The association between center-level transplant availability and access to heart transplant, the gold-standard therapy for advanced heart failure (HF), is unknown. OBJECTIVE: To investigate the association of center transplant availability with LVAD implant strategies and sub

Diagnostic Usefulness of Histological Examination of the Left Ventricular "Core" Excised to Insert a Left Ventricular Assist Device in Patients With Severe Heart Failure.

The left ventricular assist device (LVAD) has proven to be beneficial for patients with severe heart failure poorly responsive to anti heart failure medicine. To examine both grossly and histologically the portion of left ventricular (LV) free wall excised ("the left ventricular core") to insert a LVAD in 337 patients with severe heart failure from a variety of causes. We collected together all photographs of LV "cores" and the histologic sections prepared from them and reexamined both. Despite the fact that these LV cores usually weighed >100 times the quantity of myocardium available to exam

Contained rupture of a pseudoaneurysm of the descending thoracic aorta related to remnant outflow graft of left ventricular assist device after heart transplantation.

The Jarvik 2000 left ventricular assist device is inserted via a left thoracotomy with the outflow graft anastomosed to the descending thoracic aorta. Removal of the device during heart transplantation involves division of the outflow graft, resulting in a retained remnant. We describe the first reported case of a mycotic pseudoaneurysm of the descending thoracic aorta related to the remnant of a left ventricular assist device outflow graft in an immunosuppressed heart recipient complicated with systemic Pseudomonas infection. The pseudoaneurysm was temporarily treated with endovascular stent

Detection of anti-HLA antibody by flow cytometry in patients with a left ventricular assist device is associated with early rejection following heart transplantation.

BACKGROUND: Patients with a left ventricular assist device (LVAD) as a bridge to heart transplantation (HT) often have elevated levels of panel reactive antibodies (PRA). The clinical significance of anti-human histocompatibility leukocyte antigen (HLA) antibodies detected by flow cytometry in PRA negative patients remains unclear. METHODS: Eighteen patients who underwent LVAD placement as a successful bridge to HT had standard anti-human globulin complement-dependent cytotoxicity and retrospective flow cytometry assays performed to detect class I anti-HLA antibodies. Insulin Resistance Compromises the Pentose Phosphate Pathway and Impairs Left Ventricular Assist Device-Mediated Myocardial Recovery in Obese Patients With Heart Failure.

BACKGROUND: End-stage heart failure (HF) remains a major global health challenge, and left ventricular assist devices (LVADs) represent an important therapeutic option. LVAD-mediated mechanical unloading improves cardiac function and promotes myocardial recovery in many patients with HF; however, this recovery response is suboptimal in obese patients. The mechanisms by which LVAD-mediated unloading induces myocardial recovery, and how obesity alters these processes to blunt myocardial recovery, remain poorly understood. METHODS: Patients with HF receiving LVAD support were recruited to invest
